Mostrando los 25While setting timbers beside track along North Mains between mouth of 1 and Glory Hole, a member was holding timber and injured EE bent over to pick up a glut, when a piece of draw slate measuring approx 22"X28"X2" thick shaped triangular struck his hard hat and head area and right hand in which 3 stitches in head and 3 in his hand.
Member was in the process of putting drill steel against the roof to start drilling a hole, when a piece of rock measuring 0-1/2" thick, 4" wide and 12" long, fell out and struck him on the side of his face, causing an abrasion to right side of face from below right eye and cheek and a laceration to his chin that required 4 stitches.
Injury had started the continuous miner to prepare for bading shuttle car. The shuttle car opr. signaled for her to check her cable slack on the reel. After shutting miner off, she walked to shuttle car to check the cable slack when a piece of slate rock measuring 1' wide, 2' long, and 1" thick struck the top of hard hat causing staving but not knocking her down.
The employee was installing a roof bolt when a piece of rock and coal fell striking him on the back.
The employee was out of the shuttle car cleaning out the deck when he was hit by a piece of coal in the back pushing him into the shuttle car injuring his right knee. Was diagnosed on 2/10/09 by a specialist after a MRI with a hyper-extended knee and remains off at this time.
Rock fell from roof measuring 48" x 27" x 4 1/2" thick striking member on hard hat driving it down on nose. It then struck member on right foot.
Shear was at #50 sheild with britby broken. Employee repaired it and was walking through the last open break when a piece of draw rock fell striking him on the left shoulder causing him to fall to the ground.
EE WAS WAITING ON HELPER TO SPOT A BOLT BEFORE ENTERING #2 LEFT TO BOLT WHEN A 12" BY 12" PIECE OF ROCK FELL BETWEEN BOLTS STRIKING OPERATOR ON THE HARD HAT BOUNCING OFF OF HAT STRIKING HIM IN THE RIGHT FOREARM CAUSING A LACERATION ABOUT 4" LONG ON ARM. RECEIVED 17 STITCHES.
EE WAS INSTALLING THE OUTSIDE BOLT WHEN A PIECE OF ROCK FELL MEASURING APPROX 2' X 2'. THE ROCK WAS VERY THIN AND HAD SHARP EDGES. THE ROCK BOUNCED OFF HIS CANOPY AND CUT HIS LEG AT THE TOP OF HIS THIGH (RIGHT LEG). HE MISSED SHIFT ON 9/16/05 DUE TO HIS WIFE HAVING BABY.
Employee was located in the #6 face installing roof bolts. A piece of rock fell striking the drill mast and then bouncing up lacerating his left cheek. This accident occurred on the North Mains Section. Employee was on holiday weekend on 9/3 thru 9/5.
EMPLOYEE WAS INSTALLING ROOF BOLTS IN THE 3RD RIGHT CROSSCUT. HE ADVANCED THE BOLTER TO INSTALL ANOTHER ROW OF BOLTS. WHILE STANDING AT HIS DRILL STATION WITH HIS RIGHT HAND ON THE DRILL STEEL TRAY A PIECE OF ROCK, 32" X 24" X 3" FELL FROM THE MINE ROOF STRIKING HIS RIGHT PINKEY FINGER RESULTING IN A FRACTURE AND SIX SUTURES
INJURED PLACED RIGHT HAND ON TRS FRAME TO HELP PULL HIMSELF UP INTO A WALK THROUGH BOLTER. HE HEARD ROCK ABOVE HIS HEAD START TO BREAK LOOSE FROM ROOF. HE STEPPED BACK AS THE ROCK FELL, THE ROCK GLAZED HIS RIGHT SCALP BUT CAUGHT HIS RIGHT HAND AGAINST FRAME.
Injured was helping install an 8' timber - when the timber touched the roof - a piece of rock measuring 2" x 5" x 15" fell striking him on his miner's hat.
EMPLOYEE WAS ON A LADDER PUTTING A LIMESTONE BLOCK IN CORNER OF A STOPPING WHEN A PIECE OF ROCK MEASURING APPROXIMATELY 8" X 4" X 25' FELL & HIT HIS LEFT HAND ABOVE WRIST.
EE WAS HELPING INSTALL A BRATTICE CURTAIN. HE WAS ATTEMPTING TO SEAL THE BOTTOM OFF WHEN A PIECE OF ROCK MEASURING 12" X 12" X 4" FELL FROM ROOF AND STRUCK HIS LEFT HAND.
EE WAS REPLACING BROKEN BITS OF SHEAR DRUM WHEN ROCK FELL FROM ROOF STRIKING HIM ON LEFT FOOT.
HE WAS HELPING INSTALL A HEADER BOARD (FLY) WHEN A PIECE OF ROCK MEASURING 21" LONG X 18' WIDE X 6" THICK FELL FROM A ROCK BROW AND STRUCK HIM ON HIS LEFT BOOT.
EE WAS HELPING REPLACE A HYDRAULIC HOSE AT 149 SHIELD WHEN A PIECE OF ROCK MEASURING 3'X17"X2" FELL STRIKING HIM ON LEFT SHOULDER
TWO MEMBERS WERE AT THE TAIL LOADER AREA ON LW15 PREPARING TO SPLICE A PUMP CABLE. THE VICTIM, UNDER SHIELD #176, WAS BENT OVER TOWARD THE TAIL ENTRY LOOKING FOR A CABLE CRIMPER WHEN A ROCK FE LL FROM THE ROOF AND STRUCK THE EDGE OF THE ROCK GUARD AND BROKE IN TWO. THE MIDDLE PIECE (63" X 15" X 14") ROLLED OVER INTO THE WALKWAY AT #176 AND STRUCK HIM. HE WAS FLOW TO THE HOSPITAL.
EE WAS WALKING AROUND THE MINER ON THE HG-16 SECTION AT THE #3 ENTRY INTERSECTION. A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS, STRIKING HIM IN THE BACK OF THE CALF, CAUSING HIM TO TWIST HIS ANKLE WHEN FALLING. THE ROCK MEASURED 44" X 18" X 3". HE WAS SEEN BY A SPECIALIST ON JULY 3.
EMPLOYEE WAS HELPING LOWER #3 BELT WHEN A PIECE OF ROCK FELL FROM BETWEEN BOLTS STRIKING HIM ON HIS RIGHT ARM AND SHOULDER.
EE WAS UNDER THE #5 SHIELD CHECKING THE SLARK IN THE LONGWALL FACE CHAIN A ROCK FILL FROM THE TOP, HIT THE SHIELD BROKE OFF SWING BACK UNDER THE SHIELD HITTING. EE AND OTHER AGAINST THE PANLIN E. THE ROCK WAS 6" LONG, 4' WIDE AND 6 TO 8 INCHTHICK.
EE WAS HELPING MOVE A SHUTTLE CAR THROUGH A SECTION OF AIR LOCK DOORS. HE PUT HIS RIGHT HAND ON THE FRAME AS HE CHECKED THE CLEARANCE. A PIECE OF ROCK FELL FROM THE ROOF STRIKING HIS HAND.
SHOVELING LONGWALL, RIB ROLLED STRIKING HIM. WAS NOT INFORMED OF ACCIDENT UNTIL 6-20-00. SAFETY DEPT. FINISHED SHIF, NEVER MENTIONED HE WAS HURT UNTIL THE NEXT DAY WHEN HE CALLED IN.
