Bolt crew had set ATRS preparing to bolt the face of #4 entry. Operator was starting to swing drill boom out to install 1st Roof bolt. A piece of slate 0-8" thick x 30" x 60" fell between T-bar and last permanent support. Rock hit operator on head & shoulders & hit bolter canopy. Canopy had not yet been raised. Neck was jammed up & shoulders bruised.
EMPLOYEE WAS BOLTING TOP IN NO.2 ENTRY & A PIECE OF DRAW ROCK FELL BETWEEN THE 1ST & 2ND ROW OF BOLTS WHICH HAD BEEN INSTALLED. ROCK HIT EE IN THE LOWER LEFT SIDE OF BACK. COMPLAINT OF PAIN IN LOWER BACK, LEFT HIP AND LEFT LEG. TRANSPORTED TO HOSPITAL, RELEASED AFTER X-RAYS.
OPERATING SHUTTLE CAR, ROCK FELL FROM TOP, HITTING RIGHT ARM.
EE WAS GETTING PLASTER IN THE OLD WORKS, AND A 8" FLAT ROCK FELL AND HIT HIM ON THE HEAD.
SMALL PIECE OF DRAW ROCK FELL FROM ROOF HITTING. VICTIM ON THE HEAD ROCK WAS 22 IN. X 26 IN. X 38 INCH X 1 1/2 IN THICK ENTRY WAS 64 IN HIGH 19 FT WIDE.
