Two bolter operators had finished bolting the mine roof, backed the roof bolter up and started to hang a section of exhaust tubing. A large piece of rock fell out from the rib striking one employee on the leg causing a deep tissue contusion. IE was unable to return to EE's normal work duties. EE was placed on restricted duty restrictions due to weight bearing on the calf muscle.

WHILE EMPLOYEE WAS ON A LADDER ATTEMPTING TO CLEAR A CHUTE, ROCK FELL FROM AN OPEN CHUTE DOOR. THE EMPLOYEE FELL FROM THE LADDER AND ROCK FELL ON TOP OF HIM. HE WAS KOCKED UNCONSCOUS AND TAK EN TO THE HOSPITAL.
