Employee was in the process of starting the last row of bolts in the #3 face on the #4 unit when a piece of slate approximately 1'L x 1' W, 1/4" T(Estimated weight of 6/12 lbs) fell from 6"-12" above EE's head contacting the back of EE's head and back resulting in muscle soreness.

Employee was walking beside of employee's parked miner on #6 unit in the #7 entry when a piece of slate struck employee in the top of the hard hat causing a laceration to the back of employee's head that required 3 staples.
