The employee was walking up the Mill stairwell for lunch break and looked up. When the employee looked up, a piece of dirt fell into the employee's right eye, causing irritation. The employee was treated with a prescription eye drop at the on-site medical clinic.

A contract supervisor was inspecting the work area. The supervisor who was wearing fall protection stepped onto a section of floor grating which had been left unsecured, it fell 25' striking a Mill operator on head. The injured Mill operator walked away from the incident scene after preliminary medical assessment and flown offsite for further evaluation.

Drill shack roof support failed. Roof strut and 3/8" plywood fell onto Driller's hard hat. Driller was knocked unconscious. Driller recovered later but has continuing hearing loss in left ear. Transported for further evaluation.
