Employee was attempting to close a seal water line but the valve was stuck. While attempting to close it, he felt a pop and pain in his wrist while trying to close it. He was sent to the clinic. Diagnosis was a left wrist fracture avulsion.

Mostrando 50 de 54Employee was removing a bolt with an impact wrench. When the bolt spun without loosening, he reached into the engine compartment to find out why the bolt wasn't coming loose. Sharp edge, out of sight in compartment then sliced through his glove cutting his right index finger.
Employee was pulling a water hose across the grade floor of the fines crusher, "F side" just on the south side of 9F conveyor, under the #4 rod screen area, when the collar of the nozzle suddenly got stuck in a section of the floor. Causing discomfort to the employee's back. Employee has been seen by a Chiropractor.
Employee states he was partially lifting mud flaps and turning when he felt a pain in his low back and left hip area. Employee stated at clinic visit on 11/2/09 this is due to 12/3/08.
Using 8" clamp to compress brake caliper on light truck 9590 stated he felt a sharp pain on the top of his right hand. He sought treatment on 10/29/09. Also injured shoulder requiring surgery.
Employee was servicing 9513 pickup. He had vehicle in raised position. States he noticed the blockheater plug had come undone. He reached up to plug the cord back in said dirt fell off the outlet & got between his glasses & face entering his right eye. First responders flushed out eye but later sought treatment at the clinc. Cleaned out eye with Burr and wrote out prescrption
employee was lifting cover off of 3c slide chute heard a pop in the right shoulder. He determined he should visit clinic but did not notify company until 8/5/09 of his intentions.
Employee opened crusher up a notch because it was set too tight, after which he moved the swing rams that open and tighten the crusher setting back to its normal position. While moving one of the swing rams he felt a pain in his low back. He did not treat nor was an incident report entered into the system until 7/15/09 when he called to state he wanted to seek treatment.
HIT IN RIBS WITH PIPE WHILE PUSHING COOLER DUMP ARM ON
While opening the Traxol drum a small amount of the contents "puffed" from the opening and hit him in the face close to his left eye. He wasn't sure if it got in his eye or not but he flushed out his eye with water. He did not feel any irritation or burning and had no pain, but it was a possible that some got in his eye, so we sent him into the clinic to get checked out.
EMPLOYEE WAS GRINDIND A NEW DOOR OPENING FOR A HATCH TO BE ABLE TO ENTER THE DRYING MANIFOLD. A NOTHER EMPLOYEE STOP BY TO ASK A QUESTION AND EMPLOYEE FLIPPED UP HIS SHIELD AND DUST SLID OFF HIS SHIELD AND WENT BETWEEN HIS SAFETY GLASSES AND FOREHEAD AND ENTERED HIS LEFT EYE.
EMPLOYEE WAS LIFTING A SECTION OF CONVEYOR BELT IN ORDER TO RIG IT TO BE LIFTED WITH THE CRANE. HE BENT DOWN TO LIFT THE BOTTOM OF THE BELT UP TO SLIDE A CHAIN UNDER IT AND FELT A PULL IN HIS LOWER ABDOMEN/GROIN AREA.
Employee and coworker were trying to position a barrel onto a cradle when he felt a snap behind his left knee. Was initially released back to duty no restrictions, during vacation on 6/1/09 a different Dr. took off work until next appt. on 7/2/09, Due to his vacation status and companies madatory vacation status he reported the aforementioned information to us on 6/15/09.
EMPLOYEE HAD JUST FINISHED UP GRINDING ON A LOADER BUCKET. HE WAS CLEANING UP THE SURFACE FOR THE ADAPTER TO BE INSTALLED. WHEN HE FLIPPED UP HIS FACE SHIELD DUST ENTERED HIS RIGHT EYE. THE DUST HAD COLLECTED ON THE BRIM OF HIS FACE SHIELD WHILE GRINDING. WHEN THE SHIELD WAS FLIPPED UP THE DUST WENT BETWEEN HIS SAFETY GLASSES AND HIS EYE BROW AND GOT INTO HIS EYE.
While shifting filters on chest high pallet, warehouse person felt pain in lower back.
Employee was underneath Cat pushing starter up in place. He experienced pain in his back and legs started to go numb. He came into dispatch to report his incident and the guard gate was called to bring him in for medical treatment.
Hose was buried in mud, the employee tried to pull out the hose in the mud, felt pain in his left shoulder. Sought medical treatment 7/20/08. Physician has taken him off work at this time until MRI can be done.
Employee reported sore shoulder after lying on engine compartment deck reconnecting diesel engine. Clinic diagnosed tendonitis on 7/1/08
Employee was removing ladder from ladder storage area and dirt fell from above bounced off safety glasses frame and into right eye. Received discharge papers on 4/14/08 stating the employee receiving prescription eye ointment for precaution.
Employee was removing a hydraulic cooler line inside frame at right rear of loader. Ackward position hard to manuever hydraulic line caused shoulder strain. No medical treatment at the time. Employee subsequently had an appointment at clinic doctor recommended surgery when appropriate. Surgery scheduled 7/9/08.
Removing ice on the ground near a 2" electrical conduit sticking out of ground. Grabbed ice with ungloved hand cut left thumb on sharp edge of ice.
Employee was backing down out of the engine compartment of 9174 when he bumped his left ear on the edge of the exhaust tube cutting lower ear lobe. Received 7 stitches and also had a small cut behind his ear that received 2 stitches. There was a sharp edge on the exhaust tube that caused the lacerations.
Employee noticed arm itching. He believes this was caused by a new chemical EXP11 being tested on I filter. He had contact possibly when cleaning the filter. Received doctor narrative on 8/14/07 stating that this is a chemical burn and probably due to the EXP11 exposure.
Employee was inspecting work he had completed earlier in the day. Gathering his tools he had left behind. When he left the filters he felt his back start itching. The itching could have been caused by new chemical being used in filters EXP11. Narrative received from doctor is stating that it is a rash and may have been exposed to chemical at work
Employee lifting tool box when latches opened up causing him to hurt shoulder. Medical received on 7/31/07 indicates a strain/spasm of dorsal paraspinal.
Employee was unplugging center launder going into classifier. Material started flowing to classifier, backed up hot pellets from hopper hit the water, causing excessive steam & water to back into the discharge pipe and spray out. Employee tried to escape the hazard, but ran directly into the hot pellet pieces that were spilling out burning his upper back, shoulders and wrists.
Employee was moving a used pallet. He was lifting the pallet and reported having a sore back. He has a strained low back. This is a reoccurrence of an old low back injury.
Individual was changing the steel end liners in a rod mill when his index finger on his right hand was pinched.
Employee removing conveyor return idler laying on back and working in akward position. Walway end of roller was chained in place. While manually lowering inboard end, roller slipped and got away from employee. As he attempted to grab it, his right thumb was pinched between roller and conveyor gallery floor resulting in laceration requiring stitches.
EE was removing nozzle from the 1.5" hose. He was bent over and handed the hose between his legs. When he twisted the nozzle off he felt a slight pain in back. His back started to stiffen up.
strained upper body by repeatedly lifting rear wheel locks into place on 240 ton production truck installing top wheel locks stretching out and hanging wedges on studs using one hand also replaced cylinder heads and installed kits on 9190 water truck. Had not treated immediately. Now treating, physicial therapy. Also, this is recur of old injuries.
Working on horizontal pump. Attempted to roll suction side of pump case by hand when it tipped. Mechanic index finger got pinched between the case and suction side of case.
Employee installing fuel tank on 9578 pickup on hoist. Due to design of bed on the truck a straight up lift is not possible. Employee and helper were manually fitting the tank into position which is dfficult. This appears to be a recur of an old injury same body part injured - investigation completed 9/22/05 appears to be work related.
Employee was trying to upright a strong hold cabinet off a four wheel cart in a store room by the warehouse. He felt a sharp pain in his low back as he was performing task. This is a recur of an old low back injury from 2003, 1999 and 1998
MOVING PRO TRUCK TO READY LINE AT CRUSHER. LIGHT BULB HAD FALLEN OUT FROM UNDER DASH. HAVING HEAD BENT OVER UNDER STEERING COLUMN TRYING TO FIND LIGHT BULB. MOVED HEAD BACK HELLMUT STRUCK STEERING COLUMN CAUSING HIS MOUTH TO HIT BOX ON HEATER VENT SYSTEM RESULTING IN CHIPPED TOOTH WHICH REQUIRED REPAIR
CHANGING 16" BIT ON DRILL, EMPLOYEE FELT PAIN & NUMBNESS IN HIS LOWER BACK. RECEIVED MEDICAL REPORT ON 6/27/05 STATING A BACK INJURY & IT WAS WORK RELATED. EMPLOYEE IS CURRENTLY OFF WORK.

EE WAS CHANGING CLASSIFIER SHOES ON OLD SIDE LINE 2. WHEN CLIMBING INTO CLASSIFIER, EE STRUCK HIS HEAD ON CONDUIT. RECEIVED NOTES ON 4/13/05 INDICATING EE IS ATTENDING PHYSICAL THERAPY (NECK STRAIN).
Injured was installing duct hanger and hanger bracket. The bracket spun on the hanger pin trapping his right index finger between the hanger rod and bracket, resulting in fracture.
EE was changing wiper on #10 conveyer North, while attaching wiper belt to the weighted arm the arm slipped and struck ee in mouth hitting teeth. Teeth broken and will be repaired.
Driving truck, inhalation of WD40 aggravating bronchitis - used WD40 on sticky heater flap. Investigated - pulled work orders. Diagnosis from physician as respiratory irritation.
REPLACING MOTOR ON HEATER PUSHING ON MOTOR WITH TWO FOOT PRY BAR AND TRYING TO TIGHTEN BOLT WITH OTHER HAND, FELT BURNING IN LEFT SIDE OF ABDOMEN. RCVD DIAGNOSIS 12/20.04, NOT REPAIRED AT THIS TIME
CARRYING PIECE OF METAL TO TABLE, DIDN'T LIFT METAL HIGH ENOUGH AND RIGHT HAND PINCHED BETWEEN TABLE AND METAL CAUSING LACERATION TO RIGHT MIDDLE FINGER REQUIRING STITCHES.
EMPLOYEE WAS PLACING HEAVY ELECTRICAL DEVICE ON MOVABLE CAR. THE DEVICE SHIFTED & CART MOVED. TO PREVENT THE DEVICE FROM DROPPING HE STRUGGLED TO MAINTAIN POSITION CUTTING HIMSELF ON ANGLE IRON FRAME ON TOP OF DEVICE. LACERATION REQUIRED 7 STITCHES.
INJURED EMPLOYEE WAS CLEANING UNDER THE ROUGHERS ON CONCENTRATOR LINE 1 USING A HAND SHOVEL, WORKING IN TIGHT AREA WITH LIMITED HEAD ROOM USING TWISTING MOTION. HE DEVELOPED PAIN IN LOW BACK THAT HAS NOT RESOLVED. SENT TO DOCTOR ON 10/26; MRI DONE ON 10/27.
Pulling pendents onto top of overhead crane felt strain in right forearm. Reported at end of shift.
WORKING ON #8 TRIPPER CAR RAIL IN FINES CRUSHER. ON SUNDAY HE SAID HIS RIGHT THUMB BECAME SORE. AS THE WEEK WENT ON IT BECAME MORE PAINFUL RESULTING IN A CLINIC VISIT AND DIAGNOSIS OF TENDONITIS ON 8/30.
TAKING PELLET SAMPLE, EMPLOYEE EMPTIED SAMPLE BUCKET (14 QT PAIL) USING UNDERHAND TOSSING POSITION. WHILE HOLDING BUCKET HORIZONTALLY AFTER DUMPING THIS BUCKET, HE TOOK ANOTHER SAMPLE. WHEN HE STARTED TO CARRY BUCKET, HE FELT PAIN IN LEFT GROIN AREA.
ASSISTING WITH MOVING EQUIPMENT IN AND OUT OF PELLET PLANT FREIGHT ELEVATOR. EMPLOYEE LIFTED UP DOOR BY PUSHING ON UPPER HALF OF DOOR & HE EXPERIENCED LOW BACK PAIN. REVIEW OF OLD INJURY INVESTIGATION COMPLETED 8/10/04.
