Removing pipe from pump - the pipe wrap fell hitting right thumb.

Employee aligning large pipe (12" to 14") diameter to install a tee in the pipe line - was using a chain and Komatsu-wa2oo, put self in twisted postion to assist in the lift.

Employee strained his back unloading the cable from the pallet on the tailgate of the truck.

Bag house #1 tube had coal fines builing up and falling down chunks into the Jig, EE was reaching in and pulling out the chunks of coal fines and strained his neck.

EE overloaded shovel, it was heavier than he expected due to the difference in material (coal,fines,rocks,clays). EE was cleaning material from the conveyor where it was clogged up.

EE was sitting on structural steel, reached down below the floor level to lift a chain hoist.

Moving an oxygen cylinder out of a hoisting cage, the second cylinder fell striking the left little finger of the employee.

Employee was bending down to pick up plastic pipe. Employee stated that he was not doing anything out of his routine duties. Nothing strenuous or unsafe. He said he felt the pain when he bent over.
