Employee was attempting to fill a water cart when the water fill-up hose slipped from hands. The pressure from the hose caused the metal end to strike lower lip causing a laceration.

Employee was trying to pin relay bar to shield. To help align the holes for the pin EE used another pin to let the relay bar roll along & push it over. When EE pushed the ram out, the pin popped out striking EE in the right foot.

Employee was removing a 42" bottom roller out of its bracket while the belt was locked out. EE was on knees beside the belt with right hand on the floor. When the roller came out of the bracket it struck EE on the right wrist and lower part of right arm.

I.E. was loading a water booster pump into an empty sled after a 3rd shift power move. As EE was assisting getting it in the sled, right forearm was caught between a rail of the pump & the sled. EE missed no work until Oct. 26th 2016. EE has went off for a surgery on hand that the Dr. says has to do with the incident in May.

Employee was inserting a tow bar into the socket on the shearer to remove a DA Ram that had been replaced. While trying to attach the DA Ram to the tow bar the employee pinched left hand, below the thumb between the tow bar and the ram. This caused a small laceration that resulted in 3 stitches.

Employee was pulling a piece of 8" plastic pipe in the mine. The rigging loosened up that secured the pipe to the mantrip and the pipe slid forward when the operator slowed and struck EE in the right forearm.

Employee was disconnecting shuttle car cat heads for the power move. They started to pull the middle car cable up and didn't realize it was still connected to the distribution box. When the cable got tight it turned the distribution box. Employee's left ankle was caught between the distribution box and the C.O. cable sled. This resulted in a bruise on EE's left ankle.

Employee was building cribs on the longwall recovery face. EE set a crib block on finger smashing it between two crib blocks. Crib blocks being used are 6"x6"x30" with an estimated average weight of 20-25 lbs.

Employee was pulling on 60" belt with 5/8 steel braided cable and shive wheels. While they were pulling, a double molly became lodged. They then let the pressure off from the scoop side to unlodge the cable. When it became unlodged the tail end of the molly caught employee's glove and pulled EE's thumb into the shive wheel causing a laceration to EE's left thumb.

Employee was attempting to load an oxygen cylinder for welding into a scoop bucket. EE was carrying the cylinder and stepped on a loose rock. EE slipped, buckling right knee. The cylinder fell across the back of right leg and right knee. EE felt a "pop". Results of the injury reveal a torn meniscus.

Contactor was entering the guard shack and grabbed the door handle with right hand. A gust of wind jerked the door from ee hands. Contractor felt a strain/sprain in lower back.

Roof bolter operator was trying to line the top and bottom drill steels to drill his bolt hole. The bottom drill steel fell from the drill chuck. The operator then dropped the top drill steel which fell approximately 5' striking the right ringer finger.

Setting timbers. Went to guide a timber someone was placing and finger got caught between the two timbers and smashed.

While roof bolting, employee turned to retrieve a roof bolt wrench from the material tray and stuck his left elbow on roof plates that were stacked on the tray. The result was a laceration requiring 5 sutures to close.

Employee stopped to assist other employees installing belts on the B Pump Vessel pulley. When they rolled the pulley his thumb caught on the belt and rolled his thumb into the upper sheave. This caused a laceration to his thumb requiring stitches.

I.E. was washing at A tail. I.E. stated he just shut the water off to add another joint of fire hose. When I.E. turned the water back on he lost control of it and dropped it. This resulted in the fire hose striking I.E. on the back of his left hand and wrist causing a slight fracture to his left wrist.

I.E. was changing a bottom roller on the clean coal load out belt. While I.E. was changing the roller he felt a piece of dirt fall in his right eye. I.E. was wearing safety glasses. The roller was above his head and the particle fell from the roller and entered into the top of his glasses then into his right eye.

Employee was raising lid on battery pack on scoop tractor and before he could secure it in the upright position, he dropped it on his right hand, fracturing his right middle finger and also requiring sutures to close a laceration to the same finger.

Employee was helping another employee change a bottom belt roller. The employee's right middle finger was caught between the belt roller bracket and belt frame resulting in a fracture and laceration to the tip of his finger. The employee received 7 sutures.

Employee installing 60" structure. Employee was trying to get a pin installed in a belt rail when the rail flipped & landed on his hand which was on a ladder. His hand was between the rail & the ladder which caused the injury.

Employee was descending steep narrow spiral staircase while carrying parts and felt pain in left knee. Employee tried to work thru pain but had arthroscopic knee surgery on 09/10/2015 and returned to work on 09/14/2015 with a full medical release. Employee had no lost work days.

I.E. was placing a set of bobcat forks in the bucket of scoop #6, when one slipped striking I.E. on his right hand.

I.E. was picking up a 42 inch top roller to install. As soon as he picked it up he said he felt a pop in his right shoulder. Top roller is approximately 50-60 LBS.

Employee struck in face with tubing ring. Went to dr. 1/30/15.

Employee was going to tail to grease tail. After passing shear employee stepped on to a shield that was already pulled in. The automation come through this area and tried to lower and then re set itself. Employee was standing on shield during cycle, it dropped down causing the employee to overextend in an already bent over position

At the end of the shift the injured contract employee was unloading bad rollers into the scrap metal bin on the surface, when one became caught causing him to catch his left pinky and ring finger between the roller and a rail resulting in a fracture and laceration to each finger.

Shearer was mining towards the headgate, when a rock was thrown out with the drum hitting I.E. in the bottom part of his left leg. I.E. continued to work thru 1/19/15 before taking off. This resulted in I.E. getting an infection.

I.E. picked up an acetylene tank and placed it on the tool sled. I.E. felt a sharp pain on top of his right shoulder. This resulted in a separated AC in I.E.'s right shoulder.

While unhooking rope, hook swung around and caught employee between the index and middle finger.

While cleaning off the top of the miner, a rock hit employee on his right foot.

While employee was holding screen wire for roof bolter operator, (he was on ground) the wire slid down coming in contact with employees lip. Employee continued to work the shift. Next morning decided to go to doctor and received 5 stitches in lower lip. Employee was helping to install wire on ribs.

Bent down to remove rock and felt pain in back. First day missed was 11/17/14.

Employee was throwing sequence roller on top of feeder prior to belt move when roller fell off striking him on the face. Employee received 5 stitches and returned to work following shift.

Employee was constructing scaffolding and got finger pinched in cross brace. The employee was diagnosed with a fracture in the 5th digit finger of the right hand

Employee was working on the bottom deck scalping screen in the prep plant when he reached into the area and cut his arm on the screen. This was a file only until he had to go get the coragulated skin removed for healing purposes. He required 5 stitches due to the procedure. Employee has not missed any work due to injury

While loading 42 inch top rollers employee smashed thumb.

While loading trash onto a trailer, a 2x2 board slipped and pinned employees leg inside cab of bobcat.

Employee was picking up a knuckle for a shield. He was trying to turn the knuckle to put it on the relay bar when he heard a loud snap in his right arm. He tore the bicep in his right arm.

EE was washing the shear off prior to servicing. the shear had more than normal amount of rock on it than normal so he hooked up a half inch hose to the emulsion and turned it on wide open. The hose slipped out of his hands immediately and hit him repeatedly before pressure could be reenergized. This is not a normal operation procedure

EE was in the process of setting up a longwall tripper drive @ x-cut 94 in entry #1 on Headgate 3. EE was jacking up an I-Beam to position it when the beam turned over onto his right foot. It broke 2 bones on the top of his foot.

Employee was carrying a piece of pipe and twisted his left knee.

Employee was pulling down a curtain when he felt a tear/pain in his right bicep.

While employee was spreading loop on choker he popped his right shoulder.

Was helping build stopping when he went to put in last concrete block it fell out striking him in the face. Employee received stitches to his face.

HE WAS WALKING THROUGH A PARTIAL OPENING IN A HOLLOW STOPPING WHEN STRUCK ON THE RIGHT HAND BY A BLOCK THAT FELL FROM above. the block striking was estimated to fall from 12-24 inches. middle finger was fractured.

While hanging a continuous miner cable the employee felt pain in his stomach. On 6/30/2014 he had surgery for a hernia.

The miner was pulling cable off the shuttle car reel so he could hang it. While pulling cable off he felt pain in his left shoulder. He was taken out of the mine to be looked at.

Employee states he was carrying chicago air line and felt a pain in his right hip. He went off work on 01/05/2014 and is under a doctors care.

A piece of rock hit employee on the thumb.
