Employee was helping a mechanic work on a D-11 dozer when the belly pan fell off of the dozer and struck employee on top of his left foot. He went to the hospital and was examined and diagnosed with a deep stone bruise. Employee was released and returned to work the same day as the accident occurred. He has missed no work due to his injury.

Mostrando los 3The operator got out of the 992G loader to move some wet hole bags from the spoil pile when a rock rolled down the spoil and struck him in the calf of the leg. This incident happened about 8:30 PM. He reported it to the foreman about 12:00. He refused medical attention and transport to the hospital. He went to the hospital after his shift.
Employee called on 3/3/10 stating that he hurt his back on 3/2/10 while loading a shot with wet hole bags. He stated that he was going to the doctor. He returned to work on 3/4/10 with no doctors excuse. He said the doctors office was full and that he did not wait. He worked the 4th, 5th, and 6th. He called on the 8th stating he could not work.
