EE hurt lower back moving a scoop tire.

EE was splashed on the face with hot oil while moving it out the way.

EE was lifting the shield on miner-felt burning sensation in lower back. EE worked regular duties until 10.30.13 EE started missing work on 10.31.13

EE was loading bolt supplies and said he felt pain in lower right side

Employee was putting up center lines crawling, he kneeled on left knee/twisted causing pain

EE was lifting out a battery from the mantrip approx. 40lb when felt a sharp pin in lower back.

EE was setting stab jack and slipped when stab jack came down on left foot. Contusion-Left Foot--Metatarsal Boots in use.

EE WAS WORKING ON WIPER AT #4 REF BELT HEAD, HE DROPPED A BOLT, WHEN HE BENT OVER TO PICK IT UP HE FELT SHARP PAIN IN RIGHT KNEE

EE complained of left shoulder hurting while shoveling #1 head and lifting parts that day. EE starting missing work on 2.25.2014 for possible RTC repair (acromioplasty)

EE was trying to move a 5 gal bucket to pick up a dropped solaris spotter. While attempting to move the bucket EE stated that he had hurt his neck-back and had pain in bolt legs. EE was transported to Logan Hospital via Ambulance

EE was pulling belt head cable when he felt sharp pain in back

EE was hanging miner cable and felt a sharp pain in the groin area

EE was loading airlock doors in scoop bucket and felt sharp pain in left shoulder and arm. NOTE: EE did not begin missing work until 9.6.12

EE was taking the rub rail off miner to wash it when the rail was loosened it fell on the right foot. Metatarsal boots were in use. Right foot was bruised.

EE was pushing 12" pipe away from vessel with two other EE's- the left thumb was caught between pipe and I Beam-smashing the Left Thumb

EE was driving the 2 man carrier when he lost grip of the steering wheel and it hit his right hand. EE finished shift doing regular duties.

EE was loading glue onto roof bolter and twisted to load glue and hurt back.

Employee was helping get a rock off the miner -had pain in lower back.

EE was bolting top and moved a heavy rock, Supervisor stated that it looked like a hernia in the left groin area. EE refused medical attention and refused ambulance, he also continued to work regular duties thru 8.31.2011. EE notified us that he will have surgery on 9.1.11

Employee was loading cribs onto ride and twisted lower back-causing pain.

Employee was attempting to move a pump out of the roof bolter, while lifting he had a pain in his groin area. He finished his shift. He not lost any time due to his strain.

Employee bent over to pick up belt structure to load in the scoop, hurt lower back.
