Foreign body/dirt in left eye

5200-184 Rock on face - went to pull it off, slipped and fell forward, rock slipped off face and caught middle finger of right hand between two rocks.

3200 Grizzly pulling boulders down off grizzly slope, felt belly button pop.

5200-206 glue back hole with dwyidag, PVC pipe slipped out of hands of miner; striking upper lip, laceration.

Miner wash down raise, rock rolled off bulkhead striking miner on nose and left cheek

4300 Diamond Drill station - move gopher drill forward when base slid out, drill rocked ahead, miner tried to catch it and caught right arm on cable guard. Laceration to inner right elbow.

4900-121 Shop, move full 55 gallon drum of oil off timber truck, barrel slid causing left little finger to be fractured between two barrels.

On August 14th, 2017 miner was lifting 4' x 8' sheets of plywood, strained abdomen. Came in Tuesday, December 10, 2019 - wanted to see doctor concerning the injury in 2017, medical have taken miner off work till ultra sound had been completed.

Employee lifted 4' x 8' sheet of plywood resulting in hernia on August 14 2017. Went to surgeon in January 2020 - insurance accepted diagnosis and surgical repair as of February 6, 2020

5200-165 using jackleg, leg kicked out miner tried to catch it and felt burning in lower abdominal area. Went to ER on Saturday, Sept 24th. To specialist in Coeur d'Alene

4900 Chiller - was moving circulating pump, pump tipped over - EE tried to catch it. Strain to mid abdomen. Reported accident on Monday, Sept 26th. To specialist.

3400-51W - bent over to pull jackleg out of lifter, felt like tear to abdomen. Was seen on Monday, August 8th. Will be seeing a specialist in Post Falls, ID this week.

Men's Dry, clean men's dry, lifting buckets and mopping, abdominal strain - hernia surgery scheduled for Friday, July 8th.

Working on 4900 Ramp, during shift got something in left eye

Miner load powder to blast round. A slab was on collar pipe couldn't load hole without moving slab. Moved slab and broke free, sharp end cut through glove and cut right palm.

Taking down sand line on 5200-058, the pipe shot back behind employee's head causing shoulder to dislocate.

Miner was performing pre-shift inspection on mucker. The mechanic closed the hood. The hood fell and struck the miner on his right hand.

Pulling rods out of drill hole to change tubes, turned to stack the rod, felt back "instantly " hurt

Lowering cage deck after repairing; deck fell pinching right back arm between deck and cage structure. Replacing shoes on cages at Coeur Mine.

Turning loose hose roller for Alimak on timber truck, it caught part on other timber truck - felt pain in right shoulder. Went to doctor on May 27th, waiting to schedule MRI for shoulder

Load round w/pressure prill pot, turned pot on - hose blew out peppering face with prill

Employee lifting slusher drum onto shaft about waist high, felt sharp pain in back

The employee was bolting when he experienced pain in the groin area and noticed what he referred to as swelling. He stated that he has had similar pain occasionally for several years both on and off the job but it was more significant than in the past. He was diagnosed with a bilateral hernia. He could not correlate the event to any specific activity.

The employee was inspecting a 2yd LHD which was parked in the fueling area of the 3700 ramp. There were two 10"x10"x10' timbers leaning against the rib beside the LHD. The timber in the rear fell and bounced off the rib or other timber and hit the employee in the right side of the head as he was leaned over inspecting the LHD.

Put up "I Beam - 4600 Galena, felt pain in stomach. To clinic diagnosed-pulled abdomen muscle. Complained of pain-October 29, 2014, to clinic, diagnosed pulled abdomen muscle. Came back complaining of persistent abdominal pain, back to clinic - diagnosed-hernia. On restricted duty November 11 scheduled for surgery November 19th.

The employee was moving a pile of rubbish. A metal band approximately 10' long by 3/4" wide was protruding from the pile. He jerked on the band and when it came free it recoiled striking him in the eye, causing a laceration to his cornea. He reported that he was wearing safety glasses as per Co. policy but the band somehow got around them. An investigation is continuing.

EE was handing lagging up to his partner on the floor above. As he slid the lagging up a small particle of rock was dislodged and entered his eye through the gap between the top of his glasses and his forehead. The rock caused an abrasion to his cornea. He was given a prescription antibiotic as a precaution against infection. He was given a release to work his next shift.

Employee carried 60 lbs. pump down stairs -felt pain to right forearm - went to local clinic on Feb 24 2014; was told if it still bothered him to come back - employee complained of pain and range of motion - returned to clinic on October 10 - placed on restricted duty.

The miner was pulling on a jackleg drill trying to get the steel out of the hole in extremely gravelly ground. The drill did not want to come out and the miner pulled on the machine to hard and strained his lower back.

Pouring cement cell in mechanized heading. A pump was not working correctly and the employee was going back and fourth through a water and cement mix over his boots which resulted in burns to both feet.

Employee was attempting to remove a 6" header from a charged airline in order to add additional 6" pipe. He shut off the 6" valve and began to remove the Victaulic fitting. With out paying attention he was removing the vic fitting on the pressurized side of the valve. This caused a violent release of the header and valve which were still pressurized on that side.

Miner was positioning a 6"X10' section of drisco pipe into the pipe welder clamp. Employee did not move his hand out of the way when the pipe was pushed forward by other employees causing his right middle finger to become caught between the clamp and the pipe.

Employee was breaking a rock that was stuck in the chute lip when a separate piece of material came down and caught employees finger between the rock and the chute lip. This resulted in a laceration requiring sutures.

The employee was attempting to remove a rock that was caught between a utility pipe and the rib in a drift. When He lifted the rock up another rock that apparently was resting against it fell and struck him on the back of the right hand causing a laceration which required sutures.

The employee was inspecting a tailing pipe on the surface which was located on an embankment. He was descending while holding a safety line. He stepped over the pipe with his left leg and when he lifted his right leg to step over it his left fibula fractured. He did not slip or trip and he was not in an awkward position. He also stated that he had good footing.

Employee was tasked with loading a 22lb pump into a man basket for transportation to an underground heading. Employee lifted pump alone, and sustained an injury to her back. Employee was diagnosed with acute lumbar back sprain, she received x-rays and an office visit. Employee to have follow up on 4-09-13 and may be tasked with light duty on 4-10-13.

Employee was pulling the overshot & core tube out of the rods. He had the overshot supported with his right hand extended & raised over his head. The overshot came unlatched which caused his weight to shift all to his right arm and stress his shoulder resulting in his injury.

Breaking bands on bundle of split sets with pipe wrench. Band broke & forearm slid across broken band & cut forearm.

The employee was operating a valve which opens and closes a chute to fill cars with muck. He had just emptied the chute and a rock which must have been stuck to the side of the chute broke loose and fell, contacting the chute lip or check boards. It then bounced out of the empty chute lip and struck the employee on the right side of the abdomen causing a laceration/sutures.

A round had been blasted on the previous shift which partially buried the grizzly. Employee was attempting to work the muck through the grizzly. The muck started to flow and a rock that had been buried in the muck pile slid off the rib contacting his knee and causing a laceration which required sutures.

Miner was climbing up a raise to clean down muck from previous blast. Miner was up about 30 feet on the ladder when ladder swung away from the rib. Ladder had loosened up from precious blast and allowed muck to fall from above about 8 feet and struck miner on left shoulder, arm, and back. 12/17/12 placed on authorized time loss until MRI in 2 weeks.

The employee was pulling muck in the 4000 pocket. The chute gate is operated by a pneumatic cylinder and when he threw the lever to open the gate a particle was blown into his eye. This was caused by an air leak which had developed in a steel supply line that was directly beside the employee. He was wearing prescription glasses w/ side sheilds.

Employee attempted to bend over a piece of hog wire with his hand to prevent it from snagging on someone. Wire sprung back and punctured palm of left hand through employee's glove. This resulted in employee having to go to the hospital ER and having wound cleaned and 2 sutures to close wound.
