Mostrando los 13Employee was removing the hose from the Hurricane 755 Industrial Vacuum, when EE was pulling on it the hose came lose the hose hit EE in the face causing EE to have to have 4 stitches above EE's left eye.
Employee was loosening a jam nut on a Rotex arm when the jam nut unexpectedly broke free causing the employees hand to slam forward into the rotex arm.
Miner was closing the doors on a Rotex machine. While pulling down on the latches for the door the miner felt pressure and pain in the shoulder and forearm.

A large piece of frozen sand caught on half on the belt and half off causing plugging. Employee removed the chunk of ice off the belt and placed on the ground. Employee did not feel any pain, but yet felt something that wasn't right in the groin area.
Employee was shoveling sand onto a conveyor belt. Employee was cleaning underneath the conveyor. As ee was lifting the sand to put on the conveyor ee felt left shoulder pop. This causing pain in shoulder.
While changing bucket teeth a metal fragment struck Miner's arm.
While wheeling a 5" roller into place for as a replacement part for a belt press the roller came off the cart causing the individuals finger to be caught between the roller and floor grating. The individual suffered a break in the middle finger.
While lifting a 20# LP Tank the individual felt a pain in the lower back region. This individual was then suffering from a lower back strain.
The individual was clearing material from the scalping tower location. While doing this clearing the individual attempted to pick up a large rock to remove it from the area, while lifting this rock it caused the individual back pain.
The miner was injured while shoveling sand into a 5 gallon bucket and lifting the bucket from feet level to eye level. Contributing factors include twisting and bending while lifting the bucket.
Driver was adjusting tailgate chain and box was slightly raised, chain moved pinching his two fingers.
