While loading blast holes. EE was moving side to side, left to right, and front and back. Allegedly strained left muscle calf. EE sought medical attention on 9/13/2021
While removing bolts on the top of the tail pulley guard, the guard slid down and cut into employee's left ring finger
Working on an oil tank, Crusher #3, when a hose not connected, swung and hit employees head. Area struck, swelled up to approximate size of a golf ball.
Miner was throwing a chain over a beam while standing on a step ladder. Chain slipped, yanked their hand. Miner's left hand hit the ladder forcefully resulting in a laceration of the top of left thumb.
While moving trougher on C-2, trougher slid off C-2 frame and hit 3 fingers on employee's left hand.
Miner was trying to lift a full tank of acetylene in to the bed of service truck when they strained lower back.
While Lifting a 35 gallon grease barrel from the ground onto the flatbed truck (#290), employee strained right shoulder. Barrel weighed approximately 80lbs.
EE saw co-worker at Screen 15 with the hood opened 3/4 of the way. The hood was blocked by screens stacked in front of it. In an attempt to lay the hood flat, EE crouched under the hood to lift/push screens out of the way and ee felt pain in lower back thereafter. Pain did not go away, EE out on injury from 3/15/17 and sought medical attention on 3/17/17.
On 7/6 EE and Mansand crew was changing out conveyor 60 belt. EE claims the manual tugging & pulling of the belt might have caused lower back pain that was felt later that day. Pain worsened the next morning, but EE ignored it. Pain continued on for the next 2 wks. EE REPORTED INJURY on 7/20/15.
While working at the Hazemag crusher, just finished turning the blow bar and started to remove brackets. Co-worker, proceeded to push the bracket back and ee left pinky finger was struck by the bracket
Mounting steel plate to conveyor belt on the shop floor. EE lifted a 4'x 8' steel plate off the floor to stand it up and slide pallet jack under it, EE felt some strain. Later that day, while mounting 2nd steel plate & moving plate by hand with pry bar, EE felt a sharp pain in lower back.
On 11/4/14 EE reported that on 10/30/14 he had a hard time lifting the cover for crusher 4 hydraulic controls that was covered with material. Several days later he noticed a lump by his belly button and went to see a doctor on 11/4/14. His doctor referred him to a specialist, on 11/17/14 diagnosis of hernia confirmed. No lost time on this injury as of yet.
