Employee was removing fluid coupling from 10 Aux drive. Fluid coupling slipped out from between the couplings and fell on employees hand. Employee received stiches and broken finger.

Employee was walking up set of stairs to check on repair status of transfer chute transfer screw when employee looked up and a particle of lime dust entered EE's eye
Dirty dust collector, Lime dust on filter bags, extreme heat, sweating, resulting in chemical burns on both arms and neck.
Employee was raking oversize rock from the grating at the discharge of the Dryer. As EE was prying rocks from the grating, a rock fell from the end of the dryer kiln, bounced off the grating, striking EE in the mouth. This resulted in the chipping of two teeth and loosening of four teeth. Employee will need dental work to repair teeth.
Driver was offloading at the Vertical when the line became plugged. EE shut down the air and went to work the plug. The cam lock came loose, and lime blew into EE's face. Driver was wearing goggles. Employee was prescribed medication for EE's eye.
While working on a cylinder on a diverter gate, employee leaned over to see if the shaft was going to clear when cylinder/arm broke striking employee in left jaw.
PROT Employee was cleaning RK2 Baghouse on 3-11 shift. At one point EE bent over near a cell discharge damper to pick up trash. As EE went to stand up with EE's hard hat on, EE struck EE's head just under the edge of the hard hat at a force hard enough to cut the back of EE's head requiring 3 staples.
When closing a gangway at hydrate loadout, hand got caught between the handrail and gangway.
The employee tapped a pipe on the floor and when EE did, material that was in the pipe flew up into EE's eyes. Operator had the PAPR on but had the shield up at the time since EE hadn't started bagging. Employee was taken to eye doctor and given a prescription.
Employee was stepping off ladder, after cleaning HRH loadout camera, EE felt a pain in EE's left knee. This pain got worse, which EE thought EE hyperextended EE's leg. EE then reported it to EE's supervisor. Employee continued to work until November 24, 2023, when EE had surgery to repair a tear.
Employee was working to remove a separator gearbox and felt a muscle pull. Employee was diagnosed with a hernia, however, initial restrictions did not interfere with work duties. Subsequent restrictions, however, issued on 9/12 interfered with work duties making this reportable. Employee will also receive surgery.
Employee felt a pop in their shoulder while lifting a sample bucket. Surgery occurred on 11/3/2023, making this a reportable incident.
One employee was injured but conditions are unknown at this time.
Employee took the lock off and while turning the disconnect back on, EE felt a sharp pain in the shoulder. Employee originally refused medical treatment, however, a MRI revealed a tear. Surgery occurred on June 28, 2023, turning this into a reportable injury.
Employee was super sucking out 002 outfall. Had hydrate dust in boots and socks from prior shift. When they got in 002 outfall water went over their boots causing lime burns to toes, feet, and legs. Employee did not see the burns until they took their socks off. Employee went to doctor on February 3rd and began missing work on February 4th, began missing work and reportable.
Employee strained arm and shoulder while pulling and pushing on a slide gate. Initially returned to work with no restrictions. Pain persisted, however, and surgery occurred on June 26, 2023, making this a reportable based on medical treatment.
