During innertube retrieval, side loading occurred while the helper attempted to free the stuffing box by pushing/pulling on it. When released, the stuffing box deflected slightly outside the perceived red zone and the 90° pump-down hose contacted the helper's right ankle, causing a fall and ankle sprain.

Mostrando los 3After pulling the inner tube with the latch head, the helper walked to the tube rack. While placing the tube, weight shifted toward the latch head, causing EE to lose balance slightly. EE's right hand lowered, and EE's ring finger was caught under the latch head as it contacted the rack. EE was wearing impact gloves.
Employee was drilling vertical holes above head with a Hilti gun using a 1 ½" bit to install anchor bolts to secure a 10' pipe. Employee was taking a break and wiped their face off with a rag, and got a piece foreign object in their right eye. EE then attempted to flush it out. Foreign object was later removed by optometrists.
