Pulling shot hose out of face - working on shot crew.

While repairing the head drive with safety glasses on, dirt and rust got in both eyes

Assisted coworker lifting hoses 8/30. Noticed L side pain next morning. Went to ER 9/1. Diagnosed w/abdominal wall strain. No treatment. Continued to work until 9/12 when off for unrelated elective surgery. Contact foreman 9/27 that ee had surgery 9/26 for hematoma thought to be caused by strain.

Employee was helping with a concrete driveway pour. When EE stepped in wet concrete to spread it, the concrete raised higher then the boots, causing it to be in contact with EE's skin.

While attempting to lift a heavy plate to secure on the machine, the plate slipped of the machine and employee tried to catch it. This resulted in a strain to the lower back.

Cleaning around the tail roller of a de-energized belt, the load on the belt shifted, causing the belt to turn. This action pinched his left arm between the roller and the belt. Injury was a broken left forearm.

While clearing a rock jam out of the bridge dolly, a rock flipped back, on the back of the operator's right hand and broke two small bones in his right hand.

Employee was closing the door of the scaler with his right hand. He inadvertently closed the door on his left thumb. His thumb is broken in 5 places.

Hauling in crib block down the intake slope. The crib block banded together sitting in the scoop bucket was cut by metal arch roof support. The top layers of the crib block were frozen together and slid into the operators lap. Attempting to protect himself he raised his forearm in defense and received a puncture wound to his right forearm requiring two stitches.

Employee was in the man basket hanging 480 cable on roof bolts. He started to feel sore during the day. That night the pain got worse and the next day he went to a doctor.

Employee was cleaning and lubricating brake pedal on IT24 Cat Loader when a piece of rust became lodged in his right eye.

Bolter operator was installing a roof bolt and felt a pain in his back as he straightened the roof bolt. Worked the rest of the shift with some discomfort.

Miner was throwing belt structure into a scoop bucket, when the bottom stand of the structure he threw bounced off the bucket of the scoop and hit him in the left eye, causing swelling around the eye socket and redness in the eyeball.

Employee was helping do a belt move on section; when he reached to pick up a stand for low style belt structure, throwing it into place he felt a pain in his lower back. Injury to lower back.

EE was helping load conveyor structure into the scoop bucket as part of retreating from old section. As he was throwing a stand into scoop bucket, he felt something pop in his back . His back stiffened quickley went to Dr next morning.

THE MINER WAS HELPING TO DO A BELT MOVE AND WAS THROWING BELT STRUCTURE NEAR THE BELT TO INSTALL. AFTER RELEASING A PIECE WHILE THROWING THE MINER FELT A PAIN IN HIS BACK. NO HORSE PLAY OR SAFETY VIOLATION WAS THE CAUSE. COMPLIANCE WITH RULES AND REGS NOT A FACTOR. PROTECTIVE ITEMS OR TRAINING NOT A FACTOR.

EE WAS IN HOPPER PUTTING A CHAIN ON A ROCK THAT WAS STUCK IN THE HOPPER. WHEN CLIMBING OUT, FELTTHE RUPTURE

EE WAS ASSISTING IN REPLACING A GUARD ON A MOBILE BRIDGE CARRIER FOR THE CONVEYOR CHAIN. IN ATTEMPTING TO LINE UP THE BOLT HOLES OF THE GUARD A PIECE OF CRIB BLOCK WAS PLACED UNDER THE GUARD A ND DOWN PRESSURE APPLIED TO HELP LINE UP THE BOTTOM HOLES IN THE GUARD AND THE MACHINE-THE CRIB BLOCK KICKED OUT PINNING HIS HAND BETWEEN THE GUARD AND THE CRIB BLOCK.

WHILE ASSEMBLING RIGID CONVEYOR BELT STRUCTURE EE WAS ATTEMPTING TO PUT A SIDE RAIL INTO THE STAND, THE STAND MOVED CAUSING THE RAIL TO PINCH HIS MIDDLE FINGE BETWEEN THE RAIL & THE STAND.

EE ATTEMPTING TO MOVE TAIL PULLEY OF A CONVEYOR, WHICH HAD COME LOOSE AND MOVED OUT OF POSITION. THE PULLEY WEIGHS APPROX 150 LBS. HE WAS MOVING ONE END WHEN A VERY SHARP PAIN OCCURED IN HISRT CALF.

FORMAN WAS SHOVELING ALONG BELT LINE, NOTICED SLIGHT TWINGE IN RIGHT SIDE WENT HOME AFTER HIS MIDNIGHT SHIFT WENT TO BED WOKE UP WITH SWELLING ON LOWER RIGHT ABMOMEN.

WHILE SERVICING THE MINER, EMPLOYEE STRAINED ADDOMINAL AREA RESULTING IN SWELLING TO GROIN AREA, BY LIFTING A HYDROLIC OIL CAN.

EE WAS CHECKING WATER LEAK ON 4 INCH PIPE LINE WHEN COUPLER BLEW OFF CAUSING JET OF WATER TO STRIKE HIM AND KNOCK HIM OVER AGAINST A COMPANION CAUSE SURGE IN WATER PRESSURE.

EMPLOYEE GATHERING TRASH. PICKED UP OFFICE CONTAINERS (20 LBS) AND HAD IN LEFT HAND UNDER LEFT ARM. SAT DOWN IN OFFICE TO DISCUSS MATTER WITH OTHER EMPLOYEE. WAS IN TWISTED POSITION AND WHEN A ROSE TO LEAVE, A SHARP PAIN OCCURRED IN HIS LOWER BACK.

A NO. 12190007 FORM WAS FILLED OUT 1/2/90 ON THIS. EMPLOYEE OFF WORK 5 DAYS AND THEN RETURNED 1/2/90. AFTER VACATION WEEKS OF 4/2/90 AND 4/9/90, HE PRESENTED DOCTOR'S STATEMENT, SO BEGINNING 4 /16/90, HE WAS AND CONTINUES TO BE OFF WORK. EMPLOYER CONSIDERING ACTION TO CONTEST ALLEGED ACCIDENT.

EE LIFTED A 55 GALLON DRUM FROM A HORIZONAL POSITION TO A VERTIZAL POSITION IT WEIGHED APPROX. 100 LB TO 150 LB AND DISTANCE LIFTED WAS THE HEIGHT OF DRUM APPROX. 3.5 FEET, WHEN HE FELT A SEVE RE PAIN IN HIS LOWER BACK.

WAS IN CRUSHER BLDG MOVING PIECE OF STEEL WHEN REINJURED BACK.
