Employee was helping to tear down a set of steps at the mine office when a board came loose and the nail that was in the end of the board punctured his lower leg. He went to the hospital to have it checked and returned to work the next day.

Employee was bent over installing a belt guard and hit his head while raising up causing a sprain to his head and neck. He did not miss any work. This accident was not reported to the company. The company was not aware of this accident until we were contacted by our Workers Compensation provider.

The employee strained his lower back while pulling the cable of the #1 Mobile Roof Support out from under some gob.

Employee's knife was open in his pocket. He cut his right hand when he put his hand in his pocket. He had to get 2 stitches but did not miss any work.

EE's were lifting a miner tram idler to load it on the three wheeler at the end of the track. IE felt a sharp pain in his lower back while lifting.

The employee was letting down the cable of a MRS and a rope hanger struck him in the right eye.

EE was pulling down old ventilation curtain and strained his lower back.

Employee was helping to move the miner cable and strained his lower back.

EE was loading pallets on a track flat car and strained his lower back.

Employee bent over to pick up a piece of belt and strained his back.

EE was shoveling on #7 head and pulled a muscle in his lower back. He worked to the end of the shift.

Employee was lifting a piece of belt on #1 tailpiece to put in for skirt rubber when he felt something pop in his back. He reported this accident on 07/27/09 and is scheduled for an MRI on 08/04/09.

Employee strained his lower back while putting the rub rail back on the miner. His doctor has requested an MRI scan.

Employee was taking a steering jack off of the 488 scoop and smashed his right middle finger against the machine. He worked to the end of the shift. He went to the doctor the next day. The doctor told him he fractured the end of his middle finger.

The employee was loading rock dust from the supply car into the bucket of the unahauler, when he felt something pull in his lower back.

The employee was lifting the hydraulic valve chest out of the continuous miner and strained his lower back.

EE was shoveling the beltline. He hit his head against the mine roof as he was walking along the beltline causing a mild sprain with some swelling to his neck.

EE was shoveling on the belt line, when he turned in an awkward manner. This caused him to pull a muscle in his lower back.

The employee was moving some metal in the supply yard and dropped a piece of metal on the middle finger of his left hand causing a deep cut. He went to the hospital and received 12 stitches.

The injured employee was helping to finish the belt move. He was swinging a timber to knock in a bottom belt stand and twisted his lower back resulting in a bulged disc.

The employee was building a stopping and as he continued to stack the stopping blocks, a block fell from the stopping striking him in the lower right leg and right foot.

EE STRAINED HIS NECK, WHILE TRYING TO FREE A ROCK THAT WAS HUNG IN CHUTE.

WHILE EE WAS PULLING MOBILE ROOF SUPPORT CABLE FROM UNDER RIB ROLLS HE STRAINED HIS LOWER GROIN

EE STRAINED LOWER STOMACH AS HE PICKED UP A TIMBER.

WHILE EMP WAS SHOVELING #2 TAILPIECE HE INJURED LOWER BACK.

EE'S BACK WENT OUT WHILE SHOVELING TAILPIECE.

WHILE EMPLOYEE WAS BUILDING A CINDER BLOCK STOPPING, HE DROPPED A CINDER BLOCK ON HIS RIGHT FOOT.

WHILE EMPLOYEE WAS PUTTING A GUARD ON THE CRUSHER, THE GUARD SLIPPED OUT OF HIS HAND RESULTING IN A LACERATION TO HIS LITTLE FINGER ON HIS RIGHTHAND.

WHEN THE EMPLOYEE WAS ASSISTINGOTHERS IN CHANGING A 10 S/C SHUTTLE CAR TIRE. HE STRAINED HIS ABDOMEN MUSCLES.

THE ENPPLOYEE WALK INTO A BROW HANGING ONTO THE RIB WHILE HE WAS CARRYING BLOCKS. THIS RESULTED INJURING HIS NECK.

WHILE EE WAS WORKING ON THE FEEDER CHAIN HE STRAINED HIS LOWER BACK THIS OCCURRED WHEN HE LIFTED A CONVEYOR FLIGHT

WHILE THE EMPLOYEE WAS WORKING ON THE MINER HE GOT HIS FINGER HIT. BY A CRIB BLOCK. THIS CAUSEDA LACERATION TO HIS INDEX FINGER. THE EXTENT OF MEDICAL TREATMENT WAS NOT REPORTED TO THE OPERAT OR UNTIL 6-22-93. HE DID RECEIVE STITCHES.

A METALLIC FOREIGN OBJECT FLEW INTO THE EMPLOYEE'S EYES WHILE SPRAYING OFF MINER. THE EMPLOYEE DID NOT REPORT THE EXTENT OF MEDICAL TREATMENT TO THE OPERATOR UNTIL 3-5-93.

WHEN THE EMPLOYEE WAS PULLING ON THE TRAILING CABLE ON THE CONTINUOUS MINER.HE FELT A SHARP PAIN IN HIS LOWWER BACK AND STOMACH.

WHILE THE EMPLOYEE WAS BUILDING A CINDER BLOCK PERMANENT STOPPING, HE DROPPED A CINDERBLOCK ON HIS FOOT.

WHILE CRAWLING UNDER THE MINER TO CHECK PLANETARY HE TWISTED HIS RIGHT KNEE.

WHILE EE WAS LYING TRACK HE STRAINED HIS LOWER BACK.

EE WAS INM THE SUPPLY YARD ATTEMPTING TO LIFT A PIECE OF SHEET METAL WHEN HE STRAINED HIS LOWER BACK.

WHILE LIFTING A OXYGEN TANK HE STRAINED HIS LOWER BACK.

HE WAS LOADING CINDERBLOCKS INTO THE SCOOP AND STRAINED HIS RIGHT ELBOW AND LOWER BACK.

EMPL PULLING ON MINER CABLE TO MOVE IT OUT OF ROADWAY,STRAINED LOWER BACK.FIRST DAY MISSED WAS 5-10.REPORTED THIS TO OPERATOR 5-17.SAID EMPL HASNT RETURNED TO WORK.

WHILE LIFTING A MINER TRAILING CABLE, TO MOVE CONTINOUS MINER.

WHILE LIFTING BELT TO INSTALL STRUCTURE HE STRAINED HIS LOWER BACK THIS ACCIDENT WAS REPORTED TO THE OPERATOR BY THE INJURED EMPLOYEE UNTIL 03-25-91 & WAS REPORTED BY TELEPHONE

EE WAS ASSISTING IN MAKING A BELT SPLICE AND HAD STEPPED OUT ONTO THE BELT TO HOOK UP THE HOIST AND THE BELT BROKE. THIS CAUSED THE BELT & THE CLAMP TO HIT HIM ON THE LEG. THUS, RESULTING IN A LACERATION TO HIS RIGHT LEG.

WHILE HE HAD HIS HEAD INSIDE A PANEL OF A MINER, THE SWING DOOR CLOSED AND HIT HIM IN THE BACK OF THE HEAD. THIS CAUSED HIM TO HIT HIS CHIN AGAINST THE FIRING PACKAGE, RESULTING IN A LACERATIO N TO HIS CHIN. THE EMPLOYEE DID RECEIVE STITCHES TO HIS CHIN.

WHEN EMPL WAS UNLOADING RAILS HE STRAINED MUSCLES IN ABDOMEN RESULTING IN RUPTURE. THIS ACCIDENT WAS REPORTED TO OPERATOR ON 1/04/91 BY TELEPHONE BY EMPL.

THE INJURIED EE WAS REMVOING A HYDRAULIC HOSE FROM THE BOOM JACK ON A LOADER AND ASSUMBED THE PRESSURE HAD BEEN RELEASED BUT IT WASNT. WHEN HE THE HOSE WAS LOOSENED AND THE PRESSURE RELEASED H ITTING THE INJURIED IN THE EYS, AND MOST OF HIS BODY. THIS RESULTED IN A CORNEAL ABRASION TO THE EE.

SAID PERSON WAS CLIMBING LADDER TO BOARD SERVICE TRUCK AND REACHED TO PULL HIMSELF ONTO THE TRUCK BED WITH BOTH HANDS AND STRAINED HIS UPPER BACK.

WHEN HE WAS TRYING TO LIFT A PIECE OF ROCK FROM THE CONVEYOR OF A SHUTTLE CAR, HE STRAINED HIS LOWER BACK. NOTE: THE OPERATOR RECEIVED INFORMATION ABOUT THE ACCIDENT ON 11-7-90 BY TELEPHONE .

WHEN THE INJURIED EE WAS REPLACING A SHEILD ON THE CONTINUOUS MINER THE SHEILD FELL HITTING HIS FINGER. THUS RESULTING IN A FRACTURED FINGER.
