EE stated reached above the hose crimping machine and lifted the die to place in the machine. At this time EE felt pain in left shoulder. The die is estimated at 25lbs. The resulting diagnosis was a partial tear of the rotator cuff.

Stated he was not sure what he did. States he did pick up a piece of metal that he realized he couldn't pick up by himself. He got another employee to help him & had no problem until an hour later when he started to hurt. X-rays negative. Diagnosed as an illness - tenosynovitis.

While repairing a machine employee was holding a drive line in one hand while starting the bolts with the other felt a pop in left shoulder

EE was assembling the breaker gear box. The puller bar was supporting two 3' threaded rods, which were attached to the drive coupler for the gear box. EE was in process of removing the bolts from the bottom of the treaded rod, the puller bar shifted causing it to slip out of the nylon strap, falling approx 2' striking EE's left hand. Sutures to R little finger/restrictions.

Employee stated he was working on the breaker placing belting around the bottom of the hopper. When he pulled he lost grip & felt a sharp pain in his left elbow. He did not strike anything with the elbow. Accident reportable because nurse practitioner said no to grip with left hand.

Two employees were in the process of replacing refuse flume screen panels. They lowered the panel (2' x 4', 150 lbs) to the deck by hand. At this point, the middle and ring finger of his left hand were caught between the deck and the screen panel. He was referred to an orthopedic surgeon where it was determined he had incurred fractures to the tip of both fingers.

Employee stated he was repositioning the gear box for the radial stacker which was located on the shop welding table. His left hand slipped and struck a small burr on the gear box resulting in a laceration requiring 12 sutures to the ring finger of the left hand.

EE STATED HE WAS LIFTING A 1/4" SECTION OF METAL 4' X 8' WHEN HE FELT PAIN IN THE LOWER BACK. THE PAIN WORSENED AND HE WENT TO THE DOCTOR ON 8/24/05. EE RECEIVED PRESCRIPTION & WAS RESTRICTED FROM LIFTING OVER 20 LBS. THE RESTRICTION WAS LIFTED ON 8/29/05. (NO LOST WORK DAYS.)

EMPLOYEE STATED HE WAS PULLING A HYDRAULIC PUMP FROM UNDERNEATH A TABLE WHEN HE FELT PAIN IN LOWER BACK AREA. HE WAS DIAGNOSED WITH A SPRAIN BACK - LUMBAR SPINE. (THREE DAY RESTRICTION).

EMPLOYEE WAS ASSISTING TWO OTHER EMPLOYEES ROLLING A 20 FT LONG 48 INCH DIAMETER PIPE INTO POSITION. EMPLOYEE WAS POSITIONED ON THE END OF THE PIPE. AS THE PIPE ROLLED, A JAGGED EDGE OF STEEL FROM THE PIPE PENETRATED THE SIDE OF HIS LEFT HAND. FOUR SUTURES REQUIRED.

EE WAS REMOVING LINES FROM HELIUM TANK AFTER CHARGIN STRUTS ON R-190. PRESSURE WAS STILL ON LINES WHEN REMOVED. THIS CAUSED A LOAD NOISE NEAR HEIS RT. EAR. EE COMPLAINED OF PARTIAL HEARING LOS S, HOWEVER, IT SEEMS TO BE IMPROVING DAILY.

THE EMPLOYEE WAS DISMOUNTING THE 777 CAT HAUL TRUCK AND AS HE WAS LEAVING THE CAB HE CAUGHT HIS RIGHT RING FINGER ON THE EDGE OF THE DOOR RESULTING IN A LACERATION.

EE WAS PUSHING A LARGE ROCK OFF DECK OF HIS TRUCK WHEN HE FELT PAIN IN HIS RT ELBOW. STRAIN MUSCLES ELBOW.

THE EMPLOYEE ALLEGEDLY STRAINED HER RIGHT KNEE WHILE DISMOUNTING 50 TON EUCLID END DUMP TRUCK.

WHILE DOING MAINTAINENCE ON END DUMP APR EE RAISED HEAD AND STRUCK IT AGAINST BOTTOM OF BED.

WHILE IN PROCESS OF MOUNTAIN MACHINE EE PULLED MUSCLER IN CALF AS HE MADE FIRST STRIP FROM THE GROUND.

WHILE DRAINING WASHBOX ON PREP PLANT OPER,THOUGHT COALDUST HAD BLOWN INTO EYE FROM WIND.AFTER RUBBING EYE,DISCOMFORT STOPPED UNTIL NEXT DAY WHEN DISCOMFORT RETURNED AND INFLAMMATION BECAME VIS IBLE.DR FOUND PIECE OF RUST.
