Electrician was working on pulling cable out of conduit when he felt a warm burning sensation in his groin area. Employee was treated and released. H & S was notified on 3/6/15 that the injury was indeed a hernia, which made this a reportable injury to MSHA.
While installing an oil header the employee was holding the header in place while another employee was threading the union, header slipped and employee caught the header and felt a pop in his right elbow. Currently the employee is on restricted duty.
Employee was moving an Acetylene bottle with a crane with the chocker slipped, EE was guiding the bottle and it jerked injuring his back. Patient was treated and released from the hospital. The company was not notified until 11/4/14 that the injury was an MSHA recordable injury.
employee that was fixing a fence that had been blown over behind our building on the FMC property. As the employees were lifting the top horizontal bar and repositioning this bar into a fence post coupling, the top horizontal bar pinched his left hand at the thumb between the post and the fence post coupling resulting in a laceration on his left hand thumb.
Employee was assisting in moving a piece of equipment, through a door. equipment was being moved with a cherry picker, equipment hung up on a door jam, employee tried to free it and his hand got pinched between the equipment and the door jam.
EE bent down felt a slight pain in lower back, while moving valve. EE reported to the onsite medical facility the next day. Treated and released without restriction. EE was later treated by Specialist, prescribed medication for pain relief which made this recordable on 7/24/14. Company was not informed that it had become reportable until a later. (Date Unknown)
Smashed tip of right middle finger between pump shaft and hand rail.
EE was cutting the linkage on the cage of the crusher when a rock came down the chute bouncing off the rotor and through the opening striking the EE on the head, causing a laceration which required stitches, and/or staples.
Employee was lifting steel weights for a gravity belt take up to remove them. At the end of the shift he reported pain in the abdomen. He was referred to a doctor, FMC received the medical information on 10/22/2013 to show this hernia was work related.
Operator strung out water hose that was twisted. Hose slipped out of his hands and sprayed his leg with scalding hot water. Injured area became blistered and resulted in a 2nd Degree burn on upper right thigh.
Employee was arranging steel bars on a rack, one bar fell and pinched finger between two bars, pinching caused laceration which required four stitches.
Employee hit his head on a low pipe, and injured his neck. Received an X-Ray from chiropractor and treatment.
Operator was adding Sulphonic acid and D-90 in a mixing tank when the liquid splashed into his right eye. He was wearing safety glasses but the fluid splashed up under the glasses.
A spud wrench impaled the injured employee in the back of the calf when another employee working above him dropped the wrench.
Employee was pulling a 55 gallon barrel off of a pallet when the pallet broke, tipping the barrel on its side. The employee bent over to roll the barrel out of the way when he felt a pain in his back. He was treated by a chiropractor.
Employee was moving a large steel plate about 500# and had set it on the ground to re-rig it to move again. Initially the plate did not move but when EE stepped up to it to re-rig it fell over striking EE's left ankle.
Employee was lifting a railcar lid. The lid jammed half way up and the employee strained his back. Company was notified of work restrictions on 10/31/11.
Employee was aligning the load spout to the Railcar. He was aligning the spout with his right leg. He was supporting his posture and gripping the hand rail. As he pushed with his right leg. He felt a sharp pain run through the back side of his right upper leg which resulted in a torn hamstring muscle.
Employee was moving a 6" vacuum hose and felt pain in his shoulder. Employee was prescribed Physical Therapy.
Employee was manually handling filter leaves and felt a pop and then pain in right arm. Employee diagnosed with detached bicep tendon.
Checking tail of CB-2216; stood up & hit head on a beam near the tail pulley(approx. 5 1/2' high). The impact caused dizziness and head and neck pain.
Worker on his own decided to assist moving the material being lifted away from the wall by pushing the lift cable instead of the pallet of material. Had his gloves come into contact with top of the tugger pully pinching his left hand (little and ring finger) worker was wearing heavy leather gloves.
Lifting a piece of pipe to install with a come-a-long; pipe hung up - felt a pop in left shoulder & pain soon after.
Carrying two 5-gal. buckets of caustic to recharge tanks, while walking up stairs turned & felt knee pop. After review of medical documentation on 11/15/10 it was determined that this was a work related injury.
Employee states he was calibrating a bag packer weight scale with a 50# weight; the weight fell off the spout hitting his knee. The 7000-1 is being filed now because Safety Dept. became aware on 10/26 that the lost time was related to work.
He was moving pipe (about 10" X 36") from a pipestand to the bed of his mantrip when he felt a pain in his back. Thought he may have pulled a muscle. That evening he didn't rest well but thought it was getting better. On Friday, as the day went on, the pain continued to increase.
Inhalation of Chlorine fumes while preparing ICB. Scooping granulated chlorine into 5 gallon pail & adding to ICB. Inhaled fumes of the pail and/or measured chlorine.
Building a hand rail. Piece of metal went into left eye; wiped it out & went back to work. On way home eye became painful; he went to doctor who removed a small piece of metal from left eye. He was released to full duty.
Employee was pushing on a broken section of rail on a mantrip when it broke. The employee lunged forward scraping left forearm against the area where the rail had broken from recieving 2 lacerations, each requiring sutures.
Lifted box from the work bench (approx. 15 lbs.); felt sharp pain on top of right shoulder. Went to Medcor for evaluation & then returned to work.
Cleaning settled product from the top of the Rotex Gear Box. Had broken up the material with the handle of his channel locks & proceeded to wipe it of the gear box with his hand. Cleared off the front & then moved to side sliding his hand under the cover where his finger was struck by the cam.
Employee reported on 3/22/10 that he injured his back on 3/19/10 when lifting panel for an audio booth. Employee under chiropractic care consistently prior to this incident.Remains unclear if this is a new injury. Will know when medical records are received. In light of the elevated enforcement, this report is being generated and may be requested to be removed from the record.
Employee was working on extending the hydoclone line at the tailings impound. He said when he pushed up on the pipe his back went into spasms. He was unable to work his next scheduled shift.
On 1/5/2010 employee felt burning sensation in right shoulder while doing filter replacement; pain continued. Employee missed worked beginning 3/12/2010.
Assembling a locker, bent over to pick up the next piece of the locker & felt a pain in his back. He had not yet picked up the piece of material. Unable to get up without assistance.
Assisting in repair of discharger; pushed the movable assembly which allowed the plow guide to release from its stationary position & fall thru the housing impaling a trip bolt into the employee's left forearm.
Mechanic injured right thumb while moving belt parts. He was provided a brace and instructed to return to work with no use of R thumb. He was to be operating a picker which would not have been a restriction, however his job duty was altered and he wasn't operating a picker and was also not able to do his routine job duties. This information was discovered in February 2010.
Removing pin from the tail rope keeper on the south 4 Shaft skip. Once pin was removed the pin smashed his right hand ring finger between the keep and the pin.
Raising clothing basket by chain; it slipped out of left hand & fell on head cutting scalp. Sutures were required.
Replacing 60" parts on the 9th 60. Lifting a top part up into place and felt a pain in shoulder, neck & hand. Wasn't reportable until 8/26/09 when it became a lost time incident.
Restacking a pallet of 50# bags. While restacking he felt a pain in his back. Didn't become recordable until 8/17/09 when he started lost time.
Cut out the tailgate headed back to the headgate; there was a rock lying on the bretby channel up on the top of pan. Employee went to move rock; as he grabbed it to put it into the conveyor he felt pain on his left side upper abdominal area.
Employee was assisting others in positioning a tilting pipe deck on the forks of a forklift. They were lifting the deck when employee felt a tear in his right shoulder.
Working on roof bolter changing out bad hose & fittings. Was pulling hose towards himself to loosen a fitting & felt twinge in back. Continued to work; went to lunch around 1:00 pm & pain in his back got worse. Notified coordinator & was taken to Medcor.
Moving the knuckle for a shield from its shipping position forward to attach it to the push ram. Had to cut wire holding knuckle & slide the knuckle forward along the relay bar until it was at the front end of the relay bar. While pushing on the knuckle it slipped back & pinched employee's left long finger between the knuckle & the end of the tunnel on the shield.
Cleaning south tunnel @ Mono loadout & removing flexible 8" hose. Hose was bound in hard pipe. While attempting to remove hose, employee was pulling the hose when it because dislodged, whipped up & struck employee in the upper lip.
Employee was changing out hoist rope on 2 shaft manlift using bailing wire to attach old rope to new; as he was wrapping the ropes together the free end of the bailing wire caught on something & released causing it to spring towards the employee. Wire flipped under the bottom of his glasses into his right eye cutting into the cornea.
Employee was removing impeller from pump. Reached up & grabbed impeller with left hand & while pulling down on impeller cut his left little finger down to the bone. Was wearing a pair of jersey gloves.
EE was pulling cable off the reel of the roofbolter. The cable reel stuck, he pulled harder and felt pain in his lower back. Note: EE RTW Dec. 15, 2008 but company was not aware till Dec. 29.
Employee was getting into the operator's cab of a cherry picker and banged his knee. This resulted in a torn muscle and lost time.
