Employee dislocated a shoulder and was able to set it back in place. However, after several week, EE complained of ongoing pain. After MRI, it was determined that surgery was required for tendon tear.

Normal working conditions. No outside contributing factors to the injury.

Outside nightwork, dry, cold conditions

While throwing out shards of metal a piece of metal got caught on the employee's glove, ripping it causing a small piece of metal to become embedded in employee's hand

While performing maintenance work in normal work conditions around the plant the employee got a foreign object in employee's eye that employee was unable to flush out requiring a medical professional to remove the object.

Worker was performing maintenance on their excavator. While doing so, EE was struck/pinned between the grapple attachment and excavator track. The worker sustained severe injury to the left hip, upper left leg and lower left leg.

Borehole tracking with rodded bore Trak; suffered cramping in right bicep and noticed spasms and lump on right bicep in the evening.

Dry conditions, night time work utilizing tower lights. While removing pulley employee's wrist got cut on a sharp piece of metal of pulley.

Dry condition, daylight time. Employee received burns from the boiling water inside the slurry pump while attempting to repair it.

Employee was attempting to place take-up bearing onto shaft then the bearing slipped and pinned miner's middle finger between concrete floor and bearing resulting in a fracture of the tip of the miner's middle finger. No surgery necessary, light duty accommodated.

The employee was taking a nut off the head of the crusher when the nut fell hitting EE in the right forearm below the elbow causing a cut requiring 8 stitches.

Employee was guiding a piece of equipment into place during a repair and pinched the skin between the thumb and forefinger. He received a relatively small laceration that required 4 stitches to close.

Miner was changing a tooth on a CAT 990 loader bucket when it dropped on his foot, striking it along the edge of the steel toe. The miner suffered a broken toe on his left foot.

Employee went to enter into his tool truck to retreive a tool. As he stepped into his truck, he twisted his knee, feeling a pop and pain.

Employee was changing the backup alarm on the PC-400 Komatsu excavator and was under the machine when he stood up and hit his head. He received a small cut on the top of his scalp that required four stitches.

Employee was welding when he needed to chip away some slag from his weld. A peice of slag flew into his eye. He claims he was wearing safety glasses.

THE MAN WAS WORKING AT REMOVING THE BEARING CAPS ON THE PRIMARY CRUSHER. HE STEPPED FROM ONE PART OF THE CRUSHER TO ANOTHER AND TWISTED HIS RIGHT KNEE. HIS KNEE HURT HIM THRU THE NIGHT AND HE WENT TO THE HOSPTIAL THIS MORNING 5-27-94 FOR TETREATMENT. HE WAS EXAMINED BY A DR AND TOLD HE HAD WATER ON THE KNEE. THE DR TOLD HIM TO STAY OFF THE KNEE AND REPORT TO HIM ON 5-31-94 TO SEE I

TWISTED LEFT KNEE WHEN CLIMBING FROM BIN ONTO CATWALK.

EMPLOYEE WAS CLEANING UP SCARP IRON, ONE PEICE OF THE IRON SLIPPED OUT OF HIS HAND FELL, AND STRUCK HIM ONTHE TOE OF LEFT FOOT.

EMPLOYEE WAS HELPING UNLOAD A 55 GAL DRUM OF OIL OFF A PICK UP TRUCK AS THEY WERE SLIDING THE DRUM OFF THE TAILGATE IT SUDDENLY FELL TO THE GROUND CATCHING EMPLOYEE LEFT HAND BETWEEN THE DRUM AND TAILGATED OF PICK UP TRUCK EMPLOYEE HAS A BROKEN BONE IN TIP OF RING FINGER AND THE TIPS OF 2 OTHER FINGERS BRUISED.

EMPLOYEE WAS HE5PING CHANGE SCREEN CLOTH IN MILL WHILE LIFTING A SCREEN CLOTH PANEL THE PANEL SWUNG SIDEWAYS STRIKING EMPLOYEE ON RT EYE BROW EMPLOYEE WENT TO EMERGENCY ROOM AT HOSPITAL AND HAD SEVERAL ST3TCHES IN EYEBROW

MAN WAS HELPER REPAIRING BEARINGS ON VIBRATING SCREEN.SCREEN WAS BEING RAISED BYY HOUSE JACK AND MAN DIDNT MAKE SURE PRESSURE WAS OFF.THE JACK WHEN HE LET GO OF HANDLE ALLOWING HANDLE TO FLY U P AND STRIKE HIM OVER LEFT EYE, CAUSING SMALL CUT ON EYEBROW AND SWELLING .MAN TAKEN TO LOCAL HOSPITAL.

WORKER WAS CLEANING SPILL AROUND CONVEYOR WITH A LONG HANDLE SHOVEL AS HE TURNED AROUND TO GET RID OF THE MATERIAL ON THE SHOVEL HE HIT A STEEL SUPPORT BEAM WITH THE BACK OF HIS LEFT HAND.
