EE was changing a bearing on the tail pulley of the conveyor. EE was cutting the old race off of the shaft and accidently cut the cable holding the shaft. This caused EE's finger to be pinched between the take-up and shaft.

Mostrando los 12IE and another employee had moved the guard on the surge bin out of the way to access and disassemble the gearbox to replace the seals. The guard swung down and back into place, smashing IE's pinky finger.
Had hand resting on secondary crusher as the hood was mechanically opening. A piece of angle iron came down on the tip of middle finger. Received three stitches on finger nail from ER and returned to full duty at work the next day. Went to a specialist a week later on 08/27/2018 and they said that the tip of finger is broken. EE is on light duty work now.
The crew was putting on a new belt on a conveyor. EE was pulling on the drive belt to help turn the head pulley to help the new belt go on. While pulling on the drive belt, ee caught the tip of ring finger between the belt and the pulley. The very tip of finger was broken.
Transmission pump being installed struck employee in the mouth chipping front teeth.
While changing drill steel out to a different length it misaligned causing operator to need to physically move it by hand while doing so the operator dropped the drill steel and it landed on ring finger tip right hand

EE WAS CHECKING FUEL TANK FOR LEVEL OF FUEL. EE CUT BACK OF LEG ON A PIECE OF METAL WHEN GETTING DOWN.
DIGGING ROCK OU TO CRUSHER & ROCK FELL ON FINGER.
HAD BEEN GRINDING AND WELDING ON THE JOB. RUBBED EYE WITH GLOVE AND GOT METAL IN EYE FROM GLOVE.
EMPLOYEES ENTERED PROPERTY AND PRIOR TO SHIFT OR SUPERVISION IGNITED TRASH USING GAS IGNITING THE CLOTHING OF 1 AND THE OTHER EMPLOYEE HEPED TO GET FIRE OUT. STOP DROP ROLL RIP OFF CLOTHES. BURNS TO1 CHEST, ARM, 2 HANDS, FACE, SIDE. TAKEN TO ST. FRANCIS BURN CENTER. NOTE: BURNING TRAS WITH GAS AND AT LOCATION IS NOT A PRACTICE OF COMPANY.
OPERATOR WAS SERVICING MACHINE. RIGHT KNEE POPPED WHEN STEPPING UP ONTO MACHINE. THE KNEE HAS REMAINED SORE & STIFF SINC.
