Washing the lube truck and went to climb the step ladder in the back of the truck and calf muscle popped. Muscle strain. It was unknown EE was going to miss time from work until 4/28/26 and wasn't scheduled to come back to work until 4/30/26

Removing track adjuster from 349F excavator, removed mounting bolts and let adjuster down onto roller, went to move adjuster to let fall out of frame, adjuster fell and crushed middle finger on right hand between track and adjuster. used pry bar and lift adjuster off of hand, remove glove and found bad laceration to middle finger.
EE sustained a partial amputation at the distal portion of the left middle finger-(middle of the nail bed) when the finger was caught between two pieces of metal grating. Injury involved loss of tissue and bone at the fingertip. EE received medical evaluation and will received surgical surgery to smooth fingertip
Got on the forklift to do a pre-check. Finished the pre-check. Door was latched open to the body of the forklift and when EE reached back to pull the door close EE heard a pop in EE's bicep.
Worker was splitting a dozer track, when the track came apart EE's left ring finger was pinched in-between track pads causing injury to finger which required sutures and the tip of the bone was fractured
An employee became ill after experiencing irritation from ammonia event. The employee was evaluated and was unable to return to work the following day.
A Fixed Maintenance employee assisting with re-installation of a return roller for a conveyor belt. While lifting roller back into position, the supporting bracket weighing 7lbs - 10lbs slipped out of place and fell approximately 29 inches. Bracket struck the back of the employee's left hand resulting in a fracture of the 4th finger on the left hand.
Employee was changing out the water jug on the water dispenser when the bottle made contact with the employees chin causing a tooth cap to fall off.
two employees were putting a floor jack onto a flat bed one was lifting the other was on the truck lifting with a strap when they got it on the bed and went to set it down the jack shifted and the handle hit the employee on the truck in the head on the ear giving them a cut.
An employee sustained a toe fracture while working underground during diamond core drilling operations. The helper was in the process of lifting a core barrel into the drill head vertically when core barrel fell onto EE's foot.
EE was attempting to remove a drill bit from a drill basket when it became jammed. When the bit finally came loose, it struck the employee's right index finger, resulting in a chipped fracture and laceration that required stitches.
On 12-6-24 Employee lifted a jack leg into the back of a pickup truck when felt a light pinch in groin. Employee got home 12-8-24 and felt some pain in groin and went to the ER and has an inguinal hernia. Employee will need to see a surgeon to see if surgery is needed.
While changing a flat tire on pickup truck, employee injured back while lifting the flat tire.
The employee was lifting a chain anchor and twisted to put it on the barge. At that time EE had no pain. Later that night EE felt left lower back was a little tight. When EE woke up Thursday 3-28-24, EE's back was tight so EE went to the ER and the doctor said it was a severe back muscle strain. The doctor said it was a chronic back problem.
Miner was on a tractor and stepped over to sit on the seat and felt pain in left knee on 2-1-24. Still bothered miner and got it checked 4/6 release full duty got checked in home state 7-17 no work release until miner gets knee scoped. LTI will start 7-17-24
A haul truck driver, was standing on the emergency ladder platform located on the lower deck of a haul truck. EE was attempting to reach the air filter when they lost footing and fell approximately 4 to 5 feet to the ground. As a result of the fall, they suffered a fractured tibia.
An employee was moving a fire hose cabinet from a skip pan. Skip pan was also loaded with uni strut frames at the time. The employee did not see a unit strut frame leaning on and behind the cabinet. While moving the cabinet, the unit strut frame struck employee's finger.
The employee was in the process of removing a stuffing box from a vertical pump. The employee lost the grip with the right hand and tried to grab it as it was falling. The stuffing box flange crushed two fingers on the right hand between the concrete below.
Employee was pulling wire. Employee overexerted self resulting in a hernia.
Employee was working in the drill Conex, placing drill bits into the designated bin. After task was completed, employee noticed some discomfort in EE's abdominal area. After seeking medical evaluation, employee was diagnosed with an abdominal hernia. Employee stated that EE was not sure if it happened at work or home.
Teammate was attempting to throw a pickup tire mounted on a wheel, approximately 75lbs. onto a flatbed truck. The tire/rim struck the side of the bed forcing the tire back to the ground. The teammate attempted to stop the motion of the tire and push it onto the flatbed when EE felt the pain in their left arm.
Employee was attempting to break a plug loose on a final differential on the 692 excavator, when the plug broke loose the employee felt a sharp pain in the right shoulder.
While trying to reposition haul truck drive shaft, the shaft slipped, pinching the finger between shaft and yoke on the transmission.
Employee was attempting to push 24" pulley onto forklift when it rolled backwards towards EE and EE was caught between that pulley and another one directly behind EE.
While lifting an O2 tank into Ambulance felt a pop and tear in left elbow/bicep region
While lifting and turning to place a partially filled 5 gallon bucket of sample material into the back of a pickup, employee strained lower back. This has been a first aid case until 7/15/2020 when employee was prescribed Physical Therapy treatments. Employee has not missed any work to date.
While lifting a 50 pound bag over their head to put into a splitting bin, the employee felt a sharp pain in the neck and back just to the right of the spine. This injury occurred on 4/23/2020, however the employee was reexamined and prescribed physical therapy which began on 6/9/2020, which made the event reportable at that time.
miner was helping install dywidag bolts on the 1860 level. As EE lifted the rock bolt on to the driver, EE's shoulder popped. This resulted in discomfort and lack of mobility.
While stocking UB01 with plates a metal sliver struck through glove on right hand embedding a sliver between index finger and thumb.
Employee was manually moving a 6 inch poly pipe on 10/2/19. When the employee put the poly pipe down EE felt a sharp pain in EE's back. Due to minimal improvement in condition, the employee started Physical Therapy on 12/12/2019 which changed the status of the event from a First Aid event to a reportable Medical Treatment Case.

Employee was jackleg bolting. While grabbing jackleg, bolt fell out and hit Employee on the forearm.
Employee hyper extended left arm while moving a 2x4.
Employee felt a popping, pain and stiffness in lower back while lifting.
During rescue training a team member was pulling on a rope hooked to a basket. When ee felt pain in shoulder. EE didn't want to see a doctor at that time. In August shoulder was still sore ee was seen by a physician and released full duty. In Oct ee still sore & was referred to Ortho for evaluation. An MRI showed rotator cuff requiring surgery Oct 15th 2018 becoming a recordable.
Miner was moving CO2 bottles when ee experienced pain in lower back.
While offloading a drill tool, the driller caught right pinkie finger in a pinch point. The driller received a laceration to the finger requiring sutures.
Removing chains off loader tire
Employee was shoveling rock and dirt under the crusher conveyor guard when they heard a pop in their wrist. Employee continued to work the remainder of the shift as well as the next shift.
The employee pinched left index finger between two pieces of 12" pipe amputating the tip of finger.

While walking between two four-foot culvert pipes and carrying a nuclear test gauge in right hand subject, in anticipation of an uphill step, shifted weight to right leg and the nuke forward creating a rotation on the planted leg which caused EE's patella to dislocate and EE's leg to give out.

Employee was removing guard from the belly pan of a 16M Motor Grader. A chain hoist and jack were being used to support the guard. Employee was guiding it out when it slipped off the jack smashing left thumb between the guard and the concrete floor.
An operator was trying to clear out the blocked chute, they slammed their hand on the top part of the chute.
During the day, an employee stated EE was experiencing pain in EE's right, upper abdomen. EE could not remember any event that brought on the pain. After seeing a doctor the next day, EE was diagnosed with a hernia.
Employee was struck on left hand by a shackle under tension.
While carrying sample bags down a flight of stairs, employee noticed a sharp pain in groin area.
