Employee claims he re-injured his back from another injury at another one of our locations in the past. This injury was never reported at the time. We can find no evidence to support employee's claim that he sustained a work-related injury. We have statements from other employees and supervisors stating that at no time did they see or hear employee say he strained his back.

WAS SIPHONING GAS INHALED FUMES & GAS HAD TROUBLE CATCHING HIS BREATH WAS TAKEN TO HOSPITAL AS A PRECAUTION DOC EXAMINED & RELEASED HIM SAMEDAY SAID HE WAS FINE
