Crusher Operator was lifting impact rails about 25lbs over his head to co-workers. After lifting the fifth rail the operator felt pain in his right shoulder that started to radiate down his right arm to fingers. The operator returned to next scheduled shift, after a few months shoulder did not heal operator was seen by doctor whom suggested surgery.

Employee was lifting and carrying a loader tooth (992G) from bed of p/u to bucket. After installing tooth on shank and straightening up, he noticed pain in his lower left side of back. Was seen by a chiropractor.

NO SPECIFIC INCIDENT OCCURRED TO CAUSE INJURY, SORE BACK IS A RESULT OF REPETITIVE LIFTING AND CARRYING SAMPLE BAGS OF MATERIAL.

EE WAS CHANGING A HAMMER ON A DRILL STEEL. HE LIFTED UP ON IT TO GET IT STARTED ON THE STEEL. STRAINED BACK MUSCLES.

HE WAS LIFTING SOME CAP BOXES OUT OF THE BLAST PICKUP WHEN HE SAID HIS NECK POPPED. NO REAL REASON WHY HE POPPED HIS NECK UNLESS HE TURNED TOO QUICKLY.
EMPLOYEE WAS CHANGING HAMMER ON DRILL STEEL. WHEN HE MOVED A PIPE IT CAUSED HIS THUMB TO BE CAUGHT BETWEEN THE HAMMER AND THE CAROUSEL ON THE DRILL. LACERATION TO THUMB REQUIRED THREE STITCHES
EE WAS CHANGING A CRUSHER JAW LINER. THE LINER SWUNG AWAY FROM THE JAW CRUSHER AND STRUCK THEE'S LEFT ANKLE, CAUSING A BRUISE AND LACERATION.
EMPLOYEE STATES WHILE ASSEMBLING A ROD CONNECTIONS, THE OVERSHOT TOOL ROLLED OFF THE TOWER, STRIKING HIS LEFT MIDDLE FINGER. SUSTAINED CRUSH INJURY TO FINGER.
