Stone stacker flipped a fieldstone over on the conveyor and thought their hand was clear. The stone caught the tip of the right index finger and smashed the tip.

Operator stacking stone on pallet, when operator picked up a stone it broke in half not giving the operator time to move out of the way. Steel toe boots were in use, but did not give the bridge of operator's foot protection.

Operator off loading stone off conveyor belt, lost grip on the stone and dropped it on foot above the steel toe boot. Operator expressed just sore then 2 days later went to Dr who diagnosed fracture on top of the foot.

HE WAS LOADING TIRES METAL AND PARTS ON TRAILER FROM SHOP AREA.

WAS SCOOTING ONA 55 GAL BARREL OF OIL AND FELT A STRAIN IN BACK.

ON 7-15-98 HE TOLD HIS SUPERVISOR THAT HE HAD HURT HIS NECK AND COUPLE OF WEEKS AGO WHEN HE WAS HELPING PUSH A ROCK DOWN THE CONVEYOR. SAUL HE FELT A BURNING IN HIS NECK.

WAS PALLETIZING ROCK-LIFTED ROCK FELT SOMETHING POP IN MIDDLE OF BACK.

WAS LOADING CONVEYOR WITH ROCK FOR THE CHOPPING OPERATION. ROCK SLIPPED AND SMASHED FINGER

REMOVING EXCESS DIRT WITH SHOVEL. WHILE INDIVIDUAL WAS USING SHOVEL HE HAD A STRAIN IN BACK.

WORKER WAS LIFTING A PIECE OF STEEL & PULLED A MUSCLE IN HS BACK

DROPPED ROCK ON FT AND BROKE BIG TOE, HAD TO AMPUTATE IT , HALF OF IT

UNLOADING STONE FROM FORKLIFT TO ROLLER CONVEYOR STONE FELL ON FINGER IN WHICH GOT CAUGHT BETWEEN STONE & ROLLER CONVEYER TIP OF HIS FINGER WAS CUT
