EE was shoveling at the plant and felt a burning sensation in lower back. EE worked two days before reporting incident, EE also worked till 03-29-2024. EE did showing up for work on 04/01/2024. As of 04/09/2024, the extent of injury is still unknown.

Mostrando los 17Employee was working on screen #4 repairing a drive motor when a piece of consolidated limestone dust fell down onto employee from conveyor #33 located above screen #4. Conveyor #33 was not running at the time of accident. The piece of consolidated limestone dust struck employee in the back.
Employee was working on conveyor 7. While removing the chain from the conveyor drive system employee's left hand thumb & forefinger were pinched and lacerated by the chain.

EE strained a muscle in his left thumb while lifting a pail of nuts and bolts. This became a reportable injury on 3/23/10 when his doctor requested physical/occ. therapy and temporary restricted use of his left thumb.
EE was fusing pipe together, while in the process, he injuried his knee. **This was not diagnosed until 7/23/08.
While dismounting from the trackhoe, he strained his lower abdomen.
Employee was cutting a piece of steel that two other workers were supporting. When he broke through the steel it slid, getting away from the other two workers. The steel plate slid down hitting employee's foot, bending it downward, putting strain in his back.
Employee was installing steel wear plate in the crushing mill. The load shifted and he twisted trying to control the load, injuring his lower back and leg muscle.
EE WAS HELPING REPLACE A SHAFT IN THE CRUSHER. AS THE SHAFT WAS SWUNG INTO PLACE HE GOT HIS HAND PINCHED BETWEEN THE SHAFT AND HAND RAIL.

EE DRIVES AN 85 TON DRESSER 325M HAUL TRUCK. WHILE WAITING TO LOAD 200 FT AWAY FROM LOADING FACE, EE CLIMBED DOWN OFF OF TRUCK. FLOOR WAS DRY AND WEATHER WAS SUNNY. EE WENT TO CLIMB BACK INTO TRUCK AND LEFT KNEE HAD SHARP PAINS. TRUCK HAS NINE STEPS
INJURED WAS REPLACING ROLLER FRAME ON PLANT SURGE BELT. INJURED HIS BACK AND LEFT LEG.
OPERATOR STOPPED ON HAUL RAMP TO REMOVE LARGE ROCKS FROM HIS PATH. HE KICKED A ROCK OFF THE RAMPAND IN DOING SO STRAINED HIS KNEE
EMPLOYEE WAS REPLACING A WEAR PLATE AT THE CRUSHING PLANT. HE LIFTED A PIECE OF STEEL & INJURED HIS LOWER BACK.
