Employee was attempting to reset the manlift by pulling rope. When employee pulled down he scraped his hand over sharp metal and resulting in two finger lacerations. Employee received medical treatment and returned to work full duty.

Employee was opening a can of welding rod and his right hand slipped causing his ring finger to get cut on the lid of the can. The cut went through his glove

Employee was turning condensate water valve, reaching with arm above water filter canister. As employee turned valve on, canister cover blew off, striking employee's arm. Employee was treated at hospital for burn and buildup of air/fluids in upper arm. EE was able to return to work full duty.

Operator was adding 50 lb. bags of additives to tank. After picking last bag off of pallet, operator felt a sharp pain in the center of mid-back. Operator had soreness in back and was seen by doctor.

Employee was dumping waste cans into back of dump truck. One can was very heavy, so employee stepped up on hitch of dump truck to help lift can and heard knee pop. Employee received medical care.

Injured lower back while lifting and turning.

Employee was replacing side arm on a screen in the Crusher Building, and finger was pinched with equipment as it was being positioned. Employee received sutures.

Employee was adjusting make-up water to cooling tower when a hose came out of the top of the tank and sprayed employee with hot process water. Employee received burns on right side of his body.

Employees was dismantling scaffolding, a piece came loose and smashed his right 3rd finger, causing fractured bone and laceration

The EE was working in the coal storage area that day and had no complaints. EE went to work next day and was in the control room when he felt something in his right eye. EE and supervisior washed out his right eye, but could not get the coal out of it.

EE was going up a silo in a man basket. As he reached the work level he prepared to put on his blast hood. He noticed his rope grab was sitting on the basket & his life line had not gone through. As he started to feed the life line through it whipped back & hit him in the right eye. The strike ruptured blood vessels and tore his retina. He was not wearing his safety glasses.

Injury was lifting and moving asbestos waste bags and twisted wrong while holding the bag of about 30#s. Lumbar Strain

Employee was moving miner cable with a crutch (against the rib) and felt a sharp pain in his left shoulder. Employee was able to continue working with no restrictions. Employee received surgery on shoulder in May and was unable to work on scheduled shift beginning May 29, 2009.

Employee was replacing the gear reducer on the drive of a screw conveyor and moving the suspended load. The load hit bumped toe board and pinched right pinky finger. He was able to return to work.

Employee stuck welding rod in his hand while welding, received a puncture wound that got infected.

Employee was crouching under a pipe when his knee hit the lanyard and the lanyard swung up and hit his tooth.

While conducting a wire rope test at the drum end of #2 Shaft North Skip Rope, contractor employee put his left leg through a window cutting leg below the knee.

Employee was lifting rebar, twisted, and strained back.

Employee was removing the bottom base bolt on a pump and felt a pop in his left shoulder. Employee was able to continue working. On 5/1/08 employee received medical attention for the shoulder and Dr. restricted work activity.

Employee was standing on east side of TRM II mud line chipping out a plug of frozen mud. The frozen plug got a hole in it and cut loose. An unknown amount of pressure forced the frozen plug out along with mud hitting his head/face.

Employee was carrying door & frame up stairs. When employee raised load, he felt a pull in his knee. On 12/18/07 employee received medical attention and required restricted work activities.

Employee was lifting large hose, which was full of ore, twisted and felt pain in upper arm. Received medical treatment and had shoulder injury.

Employee cut his finger at home before work. When he was helping change brake shoes, he lost his footing with brake shoe in his hand & the brake shoe smashed his finger between the brake shoe and rail re-opening the cut on his finger that he received at home before coming to work.

While retrieving 3" water pipe from cont miner (CM01) using lift truck, pipe hung up on J-hook & fell into lift truck. Pipe bounced when it hit & struck ee's hand. EE rec'd sutures in right middle finger & ring finger. Ligaments in middle finger were torn.

Employee picked up ore sample in 5 gal bucket to bring to Lab. Bucket was fuller than usual & was very heavy. Employee tried to use one hand, but couldn't lift the bucket. Then EE used both hands to lift bucket. A day later, employee felt pain in shoulder. **Employee returned to regular duty. On May 22, he was unable to work. Dr determined it to be result of 4/22/07 incident.**

Employee was working at the bottom of Shaft #3, repairing a pump. Warning light to alert others (on door) was not illuminated, because pump and light were on the same circuit. Another employee opened up top door and inundated employee below with rush of air & dust. He received medical treatment for dust in eyes.

While shoveling rock and dirt onto a conveyor (7C26C), employee twisted the shovel to empty it onto the belt, and felt a twinge in his shoulder. Employee was able to continue working.

Employee was performing maintenance work on a continuous miner (CM04), dropped valve spool under the miner. Employee knelt down on right knee to retrieve spool. When he stood up and put weight on right leg, he felt pain in his right knee.

Employee was replacing guard on feeder Stamler #7. Guard was bent and did not slide into hanger hardware easily. While trying to force the pin into the hanger, the pin slid and pinched ee s finger between pin and hanger. He received a laceration which required sutures and a fracture in his finger.

Operator was dumping a dissolving mill (5ML01B) and had valve 1/2 open. There was low flow. When the hot liquor broke free, it sprayed out on operator, went down his foot and burned right foot.

Employee was removing materials from a utility trailer. He stood up and struck the door header against the top front of his hard hat.

EE was lifting a small piece of sheet metal (2'2"x 4'x 1/2") on a table, assisted by another EE. EE picked up his end of metal almost waist high and stopped. EE felt light headed and asked to sit down. EE was taken by ambulance and was unable to return to work.

Moving sections of 3" diameter pipe into the pipe rack, one section bound up on another and as he moved it he caught his left middle finger between them and took the tip of the 1st joint off.

Employee was tying "sta form" - As he was tying he removed his gloves. He then reached to stretch "wire mesh" tight. As he reached his hand came in contact with the sharp edge. He suffered from a laceration approx 1" long x 1/4" wide. He received 7 stitches to left (dorsal) hand.

WHILE INSTALLING A NEW 4" MUD LINE, EMPLOYEE GOT SOMETHING IN HIS EYE WHICH WAS LATER DETERMINED BY A DR. TO BE RUST.

EMPLOYEE BENT OVER TO WASH OFF BOTTOM OF MINER BOLTER BOOM & STRUCK HIS LEFT KNEE ON A METAL GUARD.

EE was performing maintenance work on maintenance truck #16. Ee felt a pain in his groin area when he sat up after working under maint. truck.

WHILE WALKING DOWN STAIRS FROM 4T-04, EE FELT PAIN IN RIGHT KNEE

Ee was raising a rail car loading chute lid when he felt a twinge in his back.

EE was standing on pipes in pipe rack attempting to close a washout valve that was leaking. EE's foot slipped under condensate stream and condensate filled his boot.

EMPLOYEE WAS CLOSING LIDS ON TOP OF A RAIL CAR. LID HUNG UP ON ANOTHER LID AND EMPLOYEE FELT A PAIN BEHIND HIS LEFT KNEE. *PAIN WENT AWAY BEFORE NEXT SHIFT BUT RETURNED ON JUNE 30, 2004. WE RECEIVED NOTIFICATION ON 7/5/04 THAT THE EMPLOYEE HAD TORN CARTILEDGE & WOULD NEED SURGERY, MAKING THIS INCIDENT REPORTABLE.

EMPLOYEE WAS WASHING THE DECK BY 4T62 (TANK) WHEN HE WAS SPRAYED WITH HOT CONDENSATE.

Employee pinched index finger on right hand while pushing magnet drawer back into place on discharge of 5C02 classifier.

EE WAS ASSISTING WITH THE LOWERING OF AN 8" SECTION OF PIPE AND THE PIPE SWUNG ON HIS LEFT FOOT. THE PIPE WAS APPROX 11 FT LONG AND WAS BEING LOWERED WITH A CHAIN.

EMPLOYEE WAS MOVING SHUTTLE CAR ONCHOR AS HE WAS RAISING THE ROPE TO PLACE IN CLEVIS, HE TURNED AND STRUCK THE ANCHOR AND HIT HIS MOUTH. A TOOTH WAS BROKEN DUE TO THIS CONTACT.

ON 11/29/02, EMPLOYEE WAS STEPPING DOWN FROM SHUTTLE CAR CAB AND TWISTED HIS RIGHT KNEE. EMPLOYEE WORKED SEVERAL SHIFTS, BUT STARTED MISSING DAYS ON 12/27/02. KNEE WAS STRAINED WITH FLUID BUIL DING.

CLEANING OUT A HEAD FRAME, WALKED PAST AND BANGED RIBS ON A PIECE OF ANGLE IRON.

EE WAS SLIDING BUNDLES OF BOLTS OFF A MANTRIP WHEN HIS GLOVE CUFF GOT CAUGHT AND PULLED HIM FORWARD, OVER-EXTENDING HIS ARM AND SHOULDER.

WHILE PULLING THE RING OFF A FILTER LEAVE, EE EXPERIENCED A PAIN IN HER THUMB.

ON 9/24/01, EMPLOYEE WAS PULLING ON A BELT TO MAKE A SPLICE WHEN HE FELT A PAIN IN HIS ELBOW. HE WAS SEEN BY THE COMPANY PHYSICIAN AND RETURNED TO REGULAR DUTY. ON 10/18/01 EMPLOYEE WAS EXPERI ENCING PAIN IN HIS ELBOW AND WAS SENT TO SEE COMPANY PHYSICIAN. HE WAS RELEASED TO WORK WITH LIMITED LIFTING REQUIREMENTS.
