Operator was exchanging a motor 40lbs. and gear box 50lbs. when they lost grip, lost balance and the item fell and landed on their left hand. Basic first aide was given, employee requested further evaluation. Operator was transported to the hospital at 8:00P.M. Left hand was bandaged. Operator has a fracture and surgery was performed on 10/24/17.

The employee was using an 8 ft 2x4 to thumb on a bin to loosen a possible bridge over of material. He was holding the end of the 2x4 in his right hand supporting the middle in his left hand. While striking the side of the bin he felt a pop in his right shoulder.
EE WAS WIPING OUT A GLASS BREAKER (SMALL) WITH A PAPER TOWELL USING HIS RIGHT INDEX FINGER AND MIDDLE FINGERS. THE BREAKER BROKE CUTTING THE TOPOF HIS INDEX FINGER BETWEEN THE FIRST JOINT AND KNUCKLE.
EE WAS SLIDING A WOODEN PALLET DOWN OFF OF TALC STOCKPILE. THE PALLET WAS USED TO ANCHOR A TARP COVERING THE STOCKPILE. WHEN HE LET GO OF THE PALLET SOMETHING CONTACTED THE INSIDE OF HIS LEFT FOREARM CUTTING IT.
