The IE was moving an L shaped piece of cut stone when part of the stone broke loose and fell towards the floor striking ee on the foot just above the safety toe protective area. The piece of stone struck only one foot.

EMPLOYEE WAS MOVING A 4X4 TIMBER WHEN A PIECE OF STONE FELL AND STRUCK THE 4X4. THE 4X4 WAS FORCE DOWN ON TO THE EMPLOYEES LEFT FOOT CAUSING BRUISING.
Bent down to pick up a rock and a sharp pain occurred in lower back area.

Employee was stacking product on a pallet. He was trying to force a piece of stone in between two others and the piece suddenly went between them. When the stone slipped in between them it caught his right ring finger pinching it at the nail. Employee didn't notify management until the next day and we took to the clinic.
EMPLOYEE WAS STACKING STONE ON PALLET WHEN THE STONE BROKE AND FELL ONTO HIS MIDDLE FINGER.
Employee was pulling frozen rocks when they came loose and jammed back onto his finger.
Contract worker was throwing away a piece of scrap stone into a rock bin and pinched the tip of his little finger off between the rock and steel bin wall.
Employee alleges he had his hand resting on a pallet and a rock sitting on the pallet fell over onto his hand. The stone that fell over was shaped like a tall building brick.
Removing rock from end feed of park industries splitter. Co-worker had placed 3 lb hammer in slot on conveyor. Smashed hand between rock and hammer head that was sticking up from the out feed belt.
Employee alleges he was taking a 25 ls rock from the 24" saw and placing if on a pallet when he felt a pop in his back and the pain started medical history ceviled he had the same thing happen 4 other times with former ee and he also has diagneitive disk diease in his lower back.
The contracted employee was installing 4" PUC pipe in the splitter plant overhead from a man lift approx 12 ft above floor and 4-5 ft from ceiling. Throughout the installation procedure PUC glue and cleaner was used. All garage doors open in the building. He passed out, had a seizure, and transported to hospital by ambulance.
EE ALLEGES SHE WAS CLEANING UNDER SPLITTER OUTFEED BELT WHILE IT WAS IN THE RAISED POSITION WITH A GLOVED HAND. THE OPERATOR PUT THE BELT IN THE LOWERED POSITION WITH HER HAND IN THE WAY.
THREE EMPLOYEES WERE CHANGING A 72' SAW BLADE. THEY PLACED THE BOTTOM OF THE BLADE ON A 4X4 AS USUAL BUT USED ONE THAT WAS ONLY 1' LONG INSTEAD OF AN 8' ONE LIKE THEY WERE SUPPOSED TO. THE WE IGHT OF THE BLADE TIPPED THE 4X4 UP & THE BLADE ROLLED ONTO THE TOP OF VICTIM'S FOOT JUST ABOVE HIS STEEL TOE.
THE EE ALLEGES HE WAS REACHING UP A PILE OF ROCK TO RETRIEVE A ROCK AND HIS ANKLE TWISTED.
