EE was tasked with other employees to change screens. When placing screens on forklift they were off center and slid, pinching EE's finger causing a laceration.

EE had been using a pry bar on a head pully to rotate it and it slipped when they gave it a good tug.

EE was trying to clean the metal off of the magnet that leads to the VSI. They grabbed a piece of metal to pull off and strained their arm.

While removing rocks from side of crusher, a rock rolled down and pinched EE's left index finger against another rock.

EE was attempting to hang a 480 volt live cable. When EE lifted the cable, EE received a second degree thermal burn on hand, from being in proximity to an arc flash.

While employee was moving angle iron from truck bed to man lift employee pinched left index finger between iron and toe board of man basket resulting in laceration that required sutures

Employee was removing a large rock from the bin feeder and EE's hand was smashed, causing injury.

Mechanic was rotating vee belt sheave checking for operational clearance between sheave and gear box bushing mounting bolts. Head of bolt had a sharp edge, as mechanic was rotating sheave sharp corner of bolt came in contact with mechanic's finger causing a laceration on the end of middle finger on left hand.

Bruised foot

Employee was working on a conveyor when a roller was stuck. EE pulled on the roller and it pinched a hand between the roller and the frame of the conveyor, pinching the hand. This caused a laceration that required 5 stitches.

Removing water hose and twisted back, felt a pop. Treated with basic first aid but was taken to a clinic on 9/16/19 which the doctor started Physical Therapy, Medication and modified duties

Hit head on a water pipe and received a laceration that required staples.

EE was pulling the guard from the cone to replace the V-belts. EE hooked the cone above EE's head in the belt frame. There are two holes to hook the cone to, the chain on the cone would not release so EE jerked on it a couple times and it came out of the hole and struck EE in the face. This caused a laceration of the upper lip that required stitches.

Heber crew was changing screens. Employee and another employee were putting the strap iron to hold the potato belting on the back of the screen. Employee was holding the bolt laying down on the bottom deck with their legs sticking out when the piece of metal fell and struck employee's leg.

Employee was trying to cover an electrical panel with a tarp during a storm. The employee pulled on a rope that was attached through an eyelet on the tarp to secure the cover. The eyelet ripped out of the tarp causing the employee to cut self on the left arm with a knife the employee was holding in right hand.

Employee was working on wash screen and felt discomfort in eye. EE thought some dirt was in it from sweat and was wearing safety glasses. EE used the wash station at work and went home at 2:30 am. At 8 am that same morning EE still felt discomfort and called EE's supervisor for treatment. EE was treated for foreign debris in eye.

While stepping up into skidsteer, lost balance knocking hard hat off and while pulling up hit head on cab causing scalp laceration requiring staples.

Employee was removing skid steer bucket and buckets pins would not release. He and another employee curled the bucket to access the bucket pins to tap them in all the way. While working on pins, the bucket became unhooked and fell two feet onto employee's foot above his steel toe.

Employee was getting concrete blocks for a project and a rock got into the loader bucket. When he tried to push the rock out, it slipped back and smashed his finger.

When removing a sheave, he dropped it causing his finger to get caught between sheave and the catwalk

Reaching up to turn off hose and felt a pop in his back.

Went recordable 3/18/12. Employee had stepped onto the first rung of a ladder and reached up to grab up further on the ladder and shoulder popped.

Helping a vendor un-strap the truck. Loosening up the binder, when it came up and struck the EE in the face.

Changing jaw belly belt and cable caught troughing roller frame and broke hook that was attached to the belting. Cable broke loose and struck leg.

While changing screens employee bumped elbow on equipment. Employee did not have any medical symptoms following incident. Employee later had symptoms evolve and sought medical care without notifying employer. Injury did not require medical treatment, until later visit when employee had fluids drained and received Steroid Rx treatment.

Used metal plate as a spacer and it broke free and hit the employees hand causing a laceration.

EE had been experiencing pain in shoulder after shoveling and lifting.

Employee was installing a belt on a screen, got finger pinched between the sheave and the gear box bolts on the side of the screen.

EE was walking oxygen bottle off rack toward him and exerted more force with his right arm.

Climbing into loader by pulling himself with his arms. Injury date 4/24/08, seen by Dr. 5/7/08, went recordable 6/5/08 with an injection of cortizone. No time off from work or work restrictions.

An employee was removing a motor from a sand stacker and needed assistance. The employee called another over to help him. The employee lost his hand grip on the motor and injured tried to catch the motor (appx 200 lbs)& pulled it toward his body with his knees bent. The motor hit into his leg just above the knee causing swelling and a contusion.

Employee was assisting in the repair & replacement of the hub & flights on the rock scrubber at the wet plant. Equipment was locked out & tagged out. Two employees were lifting the hub into the rock scrubber with a loader & were removing the hub into the scrubber. Hub was heavier than expected & his finger was pinched between the hub & frame of scrubber causing a laceration.

Shoveling out belly of screeen, while doing this his back was against metal hopper wall. The EE slid against the hopper wall, scraping his back. Area was in a tight area to work in.

Employee was changing a screen, went back to the truck to get more bolts and washers. He found an empty bucket in the old service truck and started to pull it out from the shelf. The bucket must have been stuck, so he pulled on it some more until it came loose and hit him in the lip. It was dark outside and he is not aware of how and why it happened.

Removing tail pulley, shaft dropped and his hand was underneath, caught the end of his fingertip and smashed the end tore the nail bed off.

Employee climbing off the dozer, when his wedding ring caught on the side of the dozer on a hinged housing unit. This caused the finger to become cut requiring stitches.

Working in dust cover on jaw, looking at die, hard hat knocked some dirt loose into his face and eyes. Was wearing safety glasses but dirt still went in eyes. did not flush his eyes, went all day with irritation and rubbing them. After 14 hours he went to clinic and right eye was scratched with dirt embedded in eye.

EE was lifting/moving a crusher screen that is 5'X6', weighing approx 40lbs by himself. After lifting the screen, he began to experience pain and swelling at the left elbow.

EMPLOYEE WAS WORKING WITH A CHROME BEARING WHEN A SLIVER OF CHROME CAME OFF AND ENTERED HIS HAND ON THE PALM SIDE. IT IS NOW BETWEEN THE BONE AND TENDONS.

Rocks were clogged in the jaw crusher, he moved a rock with his hands and another rock rolled down and crushed his left index finger.

EMPLOYEE WAS CARRYING A SCREEN ACROSS THE YARD WHEN HE STEPPED OVER A PIPE ON THE GROUND. WHEN HE DID THIS HE FELT A TWINGE IN HIS BACK THAT OVER TIME THE PAIN INCREASED.

EE WAS HOLDING ONE END OF FLAT METAL STRAPS, WHILE ANOTHER EE WAS UNHOOKING THE CHAIN THAT WAS HOLDING THEM TOGETHER. THE CHAIN SLIDE THROUGH THE EYELET CAUSING THE LOAD TO DROP WHILE STILL IN THE EE'S HAND. EE HAD HIS HANDS IN A CRADLE HOLD POSITION,BUT WAS NOT WEARING GLOVES

CO WORKER HANDLING DOWN HYDRAULIC PUMP TO EE. EE REACHED TUP TO GET THE PUMP, HE FELT A SHARP PAIN IN HIS BACK.

EE WAS LIFTING/PULLING ON A BROKEN SCREEN TO REMOVE IT FROM THE SCREEN DECK WHEN HE BEGAN TO EXPERIENCE LOWER BACK PAIN.

EMPLOYEE WAS CHANGING THE CUTTING EDGE OF HIS LOADER, STANDING CLOSE TO THE CUTTING EDGE, WHEN IT FELL, STRIKING THE EE ON THE TOP OF HIS RIGHT FOOT.
