While Traveling to the section on a 2 man Johnson. A drop of water fell from the mine roof and struck the employee in the right eye. The employee was wearing EE's safety glasses at the time of the incident.

Setting arches when co worker went to put a crib under support and let IE take the weight of the steel set.

EMPLOYEE WAS PICKING UP MINER CABLE TO HANG FELT BURNING PAIN IN LOWER LEFT BACK

The Employee was changing out a belt top stand on North A belt. As they lifted the top stand into place, they felt pain in the lower back.

EE was helping hang miner cable, when EE felt a pop in upper back.

Employee was working along the Section E beltline. Employee was dragging water/pipe line and tripped over other pipe line in the work area.

When throwing a crib block under a shuttle car ee smashed right ring finger between the crib block and the shuttle car frame. This accident caused a laceration to finger and broken bone in the finger also.

Employee was carrying a box of resin glue to put on the roof bolter, when employee twisted to put the glue in the glue tray employee felt pain in employee's lower back and pain going down employee's left leg.

The employee got left wrist pinched between the foot jack assembly and the frame of a roof bolter trying to put a keeper pin in the foot jack.

While filling the pod duster, the employee thought that the outside tank was empty as EE was not getting dust. EE then unhooked the fill hose from the duster and didn't know that the line was plugged and had pressure. The hose flew into EE's left hand causing a laceration. The employee was wearing gloves at the time.

The employee bent over to pick the mine cable and waterline up to throw out of the tram road when EE felt pain in EE's lower back and radiating to EE's groin area. EE was on restricted duty from 10/16/15 and ended 10/25/16. EE was placed on S/A on 10/27/16. At this time BWC has considered EE a lost time accident as of 6/7/17 since EE has not returned to work in any capacity.

While helping to clean stone off of the top of the continuous miner, a piece of sharp stone cut through the employee's glove and lacerated his right index finger. The laceration required 3 stitches.

While bending a roof bolt to push it in the hole, the employees hand slipped off of the bolt and he struck his Rt. forearm on the bolt plate causing a laceration requiring 3 stitches. the employee was wearing gloves at the time of the accident.

While working in the outside shop on a mantrip, the employee was removing a speed reducer and it slid off of the axle pinching his Rt. ring finger against the shop floor resulting in a laceration and a fracture to the tip of the finger. The employee was wearing gloves at the time of the accident.

Injured stated he was pulling down wire screen when a piece of wire went through his glove and cut his finger.

Injured employee stated he was walking and hit his head on a roof bolt hanging down with plate attached.

After oiling the hoist rope, the EE disconnected the hose on the oiler. The hose still had slight pressure on it and the oil sprayed on his right index finger. According to his doctor, the oil was absorbed into the skin causing an infection which had to be cleaned out by a surgeon.

The EE was bending a 6 ft. roof bolt and twisted his left knee. There were no lost or restricted days at the time of the injury. This injury became worse after time and the EE required surgery on 05/01/2013.

Employee was putting old rollers into the backend of mantrip when he hit his left hand with a roller against the frame of the mantrip fracturing his left middle finger,

While pulling a piece of stone off of the top of the continuous miner, the stone slipped in the employees hand causing a laceration on the right little finger requiring four stitches.

While getting on the Damascus personnel carrier the employee had a drill auger in his hand and put the auger on the machine against something solid and when he attempted to get on the machine his auger jammed him in the lower abdomen causing a large bruise.

While dropping the miner cable out of hi-line hooks, the miner moved and he caught his right index finger between the cable and the hook lacerating his finger. It required 7 stitches to close the wound. Note 4/24-25-26 2012 was regular scheduled days off.

While securing a block stopping, he was hitting a wooded wedge cap when it split and a piece of it hit him in the upper lip which required 4 stitches to close the laceration.

While trying to hold the shuttle car cable the shuttle car was energized - the cable pinched his hand between the cable and the frame of the shuttle car lacerating his right middle finger.

Employee was helping scoop operator supply roof bolter when his hand was pinched between the scoop car and ejector blade and side of the bucket. Employee received 3 stitches to the back of his hand.

Employee was suppling roof bolter when he backed away from the bolter rear tire and struck a 12" strap and hit the employee under left arm. Requiring 2 staples.

EE was removing shtone from top of continuous miner whe a p[iece of stone broke pinching his middle and ring finger of his left hand between c/m and stone. The results was breaking the tips of both fingers.

While changing a coupler on a belt drive, employee was struck on the left index finger by a fellow employee with a 5 x 7 wood block. He was driven to the hospital and required two stitches.

Employee was attempting to slide drag rail under shuttle car. He pinched his left index finger between the drag rail and the rib. This resulted in a broken bone.

While pulling a piece of brattice cloth that was parially covered with stone, felt pain and could hear a crack in right elbow. (EE did not lose any days of work until surgery on 11/16/07)

Dropping hiline during 2nd retreat a spad pulled out of the rib creating a domino effect. Hiline dropped quickly & a hiline hook caught EE's left middle finger. 13 stitches.

EE was loading a scoop car with 6" concrete blocks. He pinched the ring finger of his right hand between 2 blocks. The tip of his finger was lacerated.

Employee was in the process of cleaning coal from under belt take-up. A belt splice caught the shovel handle & the twisting action caused his left hand to strike the support structure, all guards were in place.

EE and another EE were in processed putting canopy on buggy. As they lifted the canopy over the mounts the canopy slippped & pinched his left index finger between the post & mount.

EE was walking beside stationary scoop with his hand on the edge of scoop bucket. His hand came in contact with a burr which resulted in a laceration of the palm of his right hand. He should have been wearing gloves.

EE was helping move a skid of roof bolt plates off a bundle of roof bolts. The skid rolled causing a laceration by the steel banding.

TWO EMPLOYEES WERE PREPARING TO HANG MINER CABLE. AS ONE LIFTED THE MINER CABLE HE CLAIMS SHARP PAIN IN LOW BACK AND NECK

EE AND ANOTHER EE WERE IN PROCSS OF SWITCHING OUT THE RAIL CAR (EQUIPED WITH A DRAWBAR). EE LOCKED THE RAIL CAR AND AS EE PULLED THE PIN TO RELEASE THE BAR, THE CAR DRIFTED SLIGHTLY, PINCHING FINGERS BETWEEN PIN AND RECUPLER

EE WAS CARRYING BELT STRUCTURE THRU LOW AREA AND STRUCK LEFT EAR AGAINST EDGE OF ROOF BOLT PLATE.

WHILE INSTALLING A SCRAPER ON BELT, THE SCRAPER SLID OUT OF BRACKET PINCHING EMPLOYEES THUMB BETWEEN THE SCRAPER & BELT STRUCTURE.

EMPLOYEE WAS IN THE PROCESS OF PLACING CRIB BLOCKS UNDER THE MINER, UTILIZING THE STAB JACK TO RAISE THE MINER OFF THE GROUND IN ORDER TO PERFORM MAINTENANCE. DURING THE PROCESS THE EMPLOYEE INADVERTENTLY PLACED HIS FINGER BETWEEN CRIB BLOCK AND THE CONTINUOUS MINER FRAME RESULTING IN CRUSHING INJURY TO FINGERTIP.

EE WAS CHANGING ROLLERS ON SLOPE BELT. ROLLER SLIPPED OUT OF HIS HAND AND PNCHED FINGER BETWEEN ROLLER AND SLOPE FLOOR. COMPLIANCE WITH REGS NOT A FACTOR. IN ATTENTION TO ACTIVITY A FACTOR.
