The employee was helping to move the rack wheel cover from the cable trough to the shield toes when his finger was pinched between the cover and the leg check on the #11 shield.

The employee was helping to move a piece of steel for a chute repair into position when his right middle finger was pinched between the new steel and the old piece.

On 8/8/14 the employee was unable to return to work at full capacity due to an injury that occurred on 6/1/14. On 6/14/14 the employee attempted to insert a roof bolt into the drill hole and shove the bolt through the resin cartridge by hand. The action strained his right shoulder.

The employee attempted to pick up a bundle of roof bolt plates that had been spilled to the ground. A bundle of plates on top of the pile moved and pinched the employees left index finger. The employee was working his last shift before an extended leave of absence but was released to full duty.

The employee was setting a concrete block under a battery charger frame. The tool box on the charger fell off which overbalanced the charger. The charger pinched the employee's hand between the frame and the block.

On 8/1/13 the employee reported to the mine operator that while the employee was loading vent tube into a Ram car he felt a pain in his lower back. The extent of the injury is pending further investigation.

On 1/3/14 the employee elected to have his left little finger removed. The finger was injured on 5/28/13 when the employee removed the U-bolts from the axle of a welder trailer and his hand was pinched between the trailer frame and the axle hub. The ligament damage to the finger has not healed properly

On 9/16/13 the employee had surgery to reduce a hernia that may have been started on 3/14/13 when the employee was lifting bags of Tek-Seal onto the pump for mixing.

On 3/15/13 the employee failed to report to work as a result of injuries that may have occurred on 3/10/13. The employee claims to have suffered a lower back strain while packing a crib block to load it into a pickup truck.

Lifted an oxygen bottle to put on his shoulder when he felt a pop and pain to the back of his left shoulder. Employee missed his scheduled shift of 8/10/12 for shoulder pain.

Employee chipped his front tooth while he raised the drill steel to his mouth and blew through to see if it was plugged or not. The steel came in contact with his front tooth and chipped it.

The employee was loading roof mats onto the bucket of a diesel scoop. When he attempted to move the bundle of mats it slid and pinched his right little finger against the scoop bucket.

On 3/1/12 the employee underwent surgery to reduce a hernia that appears to have occurred on 2/8/12 when the employee attempted to lift an engine cover into place on a diesel scoop.

When the employee attempted to hang a ventilation tube the end of the #9 wire hanger flipped up under his safety glasses and scratched the left eye.

The employee attempted to remove a steel band from around a bundle of bolts. The end of the band flipped up and cut his arm.

On 11/17/11 the employee was placed on restricted duty as a result of a back strain suffered on 10/31/11 when the employee lifted a compressed gas cylinder off a truck and carried it to a disabled scoop.

While attempting to check the oil in an engine the employee did not secure the engine hood. The hood fell closed and struck the employee in the head. The employee struck his forehead on the machine frame.

The employee attempted to help lift a belt line starter controller box.

The employee attempted to lift a roll of brattice out of a box and felt a muscle strain in his lower back.

On 3/2/12 the employee underwent surgery to repair damage to the left shoulder that may have occurred on 4/1/11 when the employee attempted to hang a miner cable.

The employee was engaged in building a block stopping. He cut open the back side of his arm on a piece of steel rib mesh that been cut to allow the construction of the stopping.

The employee was attempting to install a drive line in a Ramcar. In an attempt to align the u-joints the machine was moved. The movement of the driveline lacerated and fractured the employee's left ring finger.

The employee attempted to remove a roof bolter drill steel from a bolt hole by shaking and withdrawing it by hand. When the steel came loose, it pinched the left little finger against the drill head.

The employee was attempting to install a belt top roller bracket onto the belt rail when the bracket slipped and pinched his finger

The employee attempted to catch a Duke that was falling over. The duke pinched his finger against the face conveyor pan.

On 6/16/10, the employee underwent surgery on his left shoulder as a result of an injury on 8/2/09. On that date, the employee attemted to carry a jackleg down the longwall face and complained of pain in his left shoulder.

In the act of setting a wood wedge to tram the feeder breaker the employee brushed his arm against a steel edge of the feeder.

This employee stated that he had his right hand lying on the edge of a shark tank to brace while he held a loop of bull hose with his left hand. The hose slipped and swung over against his right little finger pinching the tip off.

The employee stated that he threw a wash hose over a scoop. The valve opened and the hose recoiled back and struck the employee in the face.

Employee reported this injury on Nov.14, 2008. He states he was hanging cable when it uncoiled striking him in the back.

Employee was changing hyd. hose on continuous miner when panel he had opened fell catching his fingers.

The employee stated that a roof bolt bundle steel band flipped back and struck his right hand.

Employee bent over pulling vent tube onto respirable dust sampler and felt pain in rib.

The employee attempted to move a tool box. He reportedly suffered a double hernia. Surgery to be scheduled.

On 04/21/08 the employee reported that he had suffered a hernia on 04/02/08. He believes the hernia resulted from lifting himself out of a scoop operator cab. Surgery is scheduled for 04/24/08.

The employee attempted to lift a barrel set in the mine floor. A sharp edge cut through his glove and finger.

While moving equipment parts through a mandoor, the shaft slipped and pinched the employee's right index finger.

On 5/15/06 a medical exam of the employee revealed a right side hernia. Employees stated that on 5/13/06 he had been lifting belt structure during a belt move. Surgery to be scheduled

The employee rolled a piece of rock and pinched his left middle finger between a water line and the rock

On 3/27/06 the employee notified the mine operator of lost time due to an injury sustained on 12/27/05. The employee attempted to lift a crusher inspection door and strained his right arm, torn tendon.

Employee was lifting a guard on a conveyor belt take-up, and felt a pop in his lower back (lower back & leg strain). Employee did not seek medical treatment at the time. His injury was later diagnosed as a disc herniation, which eventually required surgery.

Employee was pulling rock off track on trackhoe. Rock hooked on cuff of glove. Pulled Arm causing injury to Left shoulder.
