Mostrando los 6EE stated reached above the hose crimping machine and lifted the die to place in the machine. At this time EE felt pain in left shoulder. The die is estimated at 25lbs. The resulting diagnosis was a partial tear of the rotator cuff.
Stated he was not sure what he did. States he did pick up a piece of metal that he realized he couldn't pick up by himself. He got another employee to help him & had no problem until an hour later when he started to hurt. X-rays negative. Diagnosed as an illness - tenosynovitis.
While repairing a machine employee was holding a drive line in one hand while starting the bolts with the other felt a pop in left shoulder
Employee stated he was working on the breaker placing belting around the bottom of the hopper. When he pulled he lost grip & felt a sharp pain in his left elbow. He did not strike anything with the elbow. Accident reportable because nurse practitioner said no to grip with left hand.
EE STATED HE WAS LIFTING A 1/4" SECTION OF METAL 4' X 8' WHEN HE FELT PAIN IN THE LOWER BACK. THE PAIN WORSENED AND HE WENT TO THE DOCTOR ON 8/24/05. EE RECEIVED PRESCRIPTION & WAS RESTRICTED FROM LIFTING OVER 20 LBS. THE RESTRICTION WAS LIFTED ON 8/29/05. (NO LOST WORK DAYS.)
