Employee was working on a machine and was moving crib blocks. While moving them, the tech felt a pain around their navel and there was some slight inflammation.

Note: The original date of injury was 8/6. The employee continued to work until November 22 at which time the results of an MRI revealed that time off work would be needed. Description: Employee smacked the drill steel against another piece of steel while rib bolting to get it unstopped. EE felt a sharp pain radiate from EE's shoulder up into EE's neck.

Note: The original diagnoses was an infection. It was not diagnosed as a hernia until after seeing a specialist on 5/28/2024. Description: While dragging a timber down the offside of 3C head EE felt a pain in EE's groin area. The timber measured 6"x6"x10'.

Employee was installing the fork back onto the forklift. While trying to get it into place EE's left-middle finger was caught between the back-clip of the fork and the cleat of the fork-frame. EE received 2 stitches in the left middle finger.

While bolting, the employee was placing a piece of mesh on top of the ATRS and adjusting it with EE's hands. A rock fell and caught the inby corner of the mesh and caused the mesh to spin around and strike EE in the mouth area and knock out 2 teeth.

While loading a top roller into a scoop insert, EE stated felt a pop in lower back.

Two employees were hanging bottom roller on structure during a belt move. The roller was being lifted simultaneously by each employee with the bottom belt laying loose over top. The injured employee felt a pop in shoulder. Note: The employee worked until 6/12 when employee was then required to have surgery to repair damage sustained during this action.

The employees were pulling in/out 1000' of belt. While the injured employee was pulling the redwing (AKA Duck-Bill) over by hand to keep the belt straight the redwing slipped off while under pressure and struck the employee in the forearm.

While in the process of changing scoop batteries when they were lowering one side of the lid their fingers were caught between the battery lid lock and battery tray.

Two employees were re-stacking a stopping that had been being used as the diesel regulator. There were 2-3 rows of block left attached to mine roof that became dislodged as the witness was straightening blocks already stacked in place. A block fell causing a fracture to the injured EE's left pinky finger.

While adjusting the forks on the # 21 Forklift the fork came off and struck EE's lower right leg.

Employee had bent over to pick up gloves and when they stood up they hit left ear on a piece of damaged roof mesh resulting in a laceration.

EE had removed a knot out of the miner cable and waterline. Afterwards, when EE reached down to pick up the slack rope to hook it to the CM EE felt a sharp pain in their back. EE continued to work until the date of August 9th when EE was put off of work from a doctor.

Employee was adjusting forks on forklift and pinched left ring finger between the forks and the slide bar.

Employee was handling the forks of a forklift and smashed finger between the fork and the frame

Employee was loading a stab jack for a roof bolter into a Wallace diesel mantrip with the help of another employee. The stab jack slipped out the employee's hand and caught left index finger between the stab jack and the side of the mantrip. This resulted in a fracture to the tip of the left index finger.

Employee was attempting to load a 20" X 6" I Beam. While doing so the beam rolled and caught employee's left index finger between it and another beam resulting in the amputation of the finger just behind the finger nail.

Employee opened the air lock door in the roadway on 3 D Belt for the grader. It has to be secured with a hanger. The rope hanger on the plate of the rib bolt came loose and the door struck and knocked them down. EE said they felt something pop in left knee. Due to events surrounding this injury it has been under investigation and deemed reportable.

Injured and a foreman were installing a steel beam. They had a chain on the end of the beam that the injured was holding. The foreman was trying to install the chain on the other end, when the beam slipped. The injured tried to hold the beam, but the weight pushed them to the ground. Injured stated they did not think they were hurt.

Employee was sliding an 6" I-beam into place for a belt head. Another I-beam tipped over and caught employee's finger between itself and the other beam.

The incident referenced herein, of an original date 9/20/2017 has been under investigation. It was determined on 10/4/2017 that this will be deemed reportable under the rules. Employee was unloading a drill pot from a 14 man mantrip. Employee stood the drill pot up on its end and it fell over pinching left pinky finger between the drill pot and the metal seat of the mantrip.

The above shows original date of injury. The employees first missed work day was 8/2/2017. Description: The employee was climbing in and out of the miner conveyor chain making repairs. Employee felt a pop in employee's knee.

The original date of injury reflects 4/6/17; The employee has continued normal duties. On 5-31-2017, the employee went to a doctor and was diagnosed with a hernia. Description: The employee was lifting a timber onto the ride and felt a pulling/stinging feeling in right groin area.

Employee bent over to flip over a loop of CM cable and felt a pop in shoulder.

While the employee was descending off of a piece of machinery, struck knee against the body of the machine. UPDATE: On January 17th the employee had a follow up visit to an orthopedic surgeon. It was determined employee had damaged MCL, and has been put off of work.

Note: The original date of injury was 11/29/2016, employee continued to work under normal duties. Consequently, EE followed up with EE's physician on 12/27/2016 and was referred to be taken off of work. Description: The employee was dragging a water line from the continuous miner to help assist in watering the roadways. EE states EE felt a sharp pain in EE's lower back.

Employee was carrying a timber to be set on the belt line due to wide bolts. As employee lifted and began to drag the timber employee felt a pain in lower stomach/groin area. The employee did not receive medical treatment until 12/2/2016, in which employee was diagnosed with an Inguinal Hernia.

Employee injured EE's right thumb on Sep 29 while replacing a ripper-head jack on a continuous miner. The jack slipped pinching EE's thumb between the frame of the CM & the jack. The employee did not report this injury until 10/11 at which point it was investigated and later determined it meet reportable criteria.

Injured was carrying top structure to place in scoop bucket. While walking EE fell into the bucket, catching finger between two pieces of structure.

The employee was attempting to pick up roof mats to put onto the scoop. Employee bent over to pull, and lift mats. The employee states they then felt a pop in back.

The employee was taking a motor down to unload into the core bin with the maintenance scoop. When employee rolled the motor off into the bin it bounced, and pushed ankle into the side wall of the bin.

The employee was going to pick up one pallet to load into the trash car. When he attempted to lift one pallet, two others were attached also. The employee states that he felt a pain in his abdomen. He went to a family doctor who referred him to a specialist on August 31. This specialist determined that it was in fact a hernia.

While the employee was walking across the section he hit his head on a flyboard, stubbing his neck. This report is being filed to terminate citation #8149175.

Employee was injured picking up 6 inch aluminum water line. When he lifted water pipe, he felt a pop and pain in his right arm from elbow to wrist.

Employee was preparing to unload a plate press panel from a bed of a truck when a large gust of wind blew three panels over, striking him in the low back and knocking him down.

Employee was unloading 600 pounds lift plates when wind blew three plates over hitting employee knocking him down.

The employee was removing a rub rail and it slipped off, and fell on his left foot. This report is being filed to terminate citation # 8149176.

To begin with this accident was not reported, nor was anyone aware of it occurring until 11/14/13. At which point we put it under investigation to determine its validity. The employee was changing a breaker in a power center and the breaker panel slipped and fell which caused a laceration to his right ring finger.

The injured was cleaning rock off the from the top of the miner. He and another employee was attempting to push the rock into the conveyor boom. As the rock started to shift into the conveyor it pivoted upward and caught his finger in- between the rock and the roof.

This incident originally occurred on 4/15/2012, he did not start missing work until 4/19/2012. The injured was helping to move a 8" I-beam that was 11' long, to position it for a belt drive. He strained his back in the process.

While the injured was installing a fly board with a conventional chuck bolt the hole was not drilled to the proper dimension. He was forcing the bolt threw the pre-drilled hole and once it popped thru the board sprung back and caused a curtain hook to cut his palm.

The injured and three others were going to recover some high voltage cable from old works, they had knocked an old stopping to gain access to the area, after they traveled thru the old stopping with a scoop they were attempting to hang a piece of curtain up and some block that was left on the roof had fell and struck the employee in the back of the head.

During a belt move the injured was trying to seperate two pieces of waterline. While pushing/ pulling he cut his hand on the edge of the waterline.

The injured was loading roof bolt plates onto the scoop and they cut through his glove causing a laceration.

Injured stated that after lifting block over his head to build a stopping his back started to hurt and continued to get worse until the point he could not continue to work.

The injured was trying to situate the cat pads into place under the miner to change them out and strained his back in the process.

The injured was helping to move the section tool boxes up closer to the section. When he was pulling the tool boxes up into the scoop bucket, he strained his back.

The continuous miner was hung up on a rock ledge. They cribbed up the ripper head so they could bow it down and raise the frame of the miner to get some wood under the cats of the miner, while the injured was pushing wood under the cats of the miner the cribs under the ripper heads crushed/rolled out and caught the EE's right thumb between the cat and the crib.

The employee had laid a bolt with a pie pan against the rib. The bolt tipped over and hit his face which caused a cut.

On the night shift, the electricians had moved the feeder toward the power entry to replace the conveyor chain. The injured was moving the old flights out of the way for the new. He was going to toss an old flight over the bed of the machine when it slipped and he tried to catch it, the flight smashed his left index finger against the bed plate.
