EE was removing the roof over the gas motor on the high frequency screen plant. The edge of the roof cut EE's tip of left index finger. EE went to urgent care and got six stitches in the finger. No work restrictions were issued.

On 6/16/10 EE was lifting a bearing into a shelf, when he experienced back pain. The ER doctor said it was a pulled muscle & told him to take Ibuprofen. Employee was loading a roll of conveyor belt onto the boom truck. After unhooking the hoist from the roll of belt, it began to roll, rolled off the truck, striking ee and landing on his foot. He completed his shift that day, but went to his family doctor the next day. The doctor ordered x-rays, finding a fracture in his foot bone.
