Employee's finger was cut on the index finger of the left hand between the first knuckle and finger nail. Employee was taken to emergency room and then sent to a larger facility where a pin was inserted into his finger.
Transferring parts from an old pallet to a new pallet he felt a sharp pain in his back. The employee refused medical attention at the time of injury. However he decided to seek medical attention for this injury at a later date and did not notify his employer until Wednesday August 20th, 2008.
