Employee was trying to throw the flop gate at the direct ship hopper, when the chain ratchet free spooled letting arm of the flop gate pinch his left hand between the flop gate arm and beam causes cuts and a fracture to his index finger of the left hand.

Mechanic was removing the wheel cover from a loader when the cover slipped contacting the middle and ring fingers on his right hand and causing a laceration requiring 6 stitches on his middle finger.

While setting up the wet hole trailer, I lifted a crib block & threw it under the trailer.

Mechanic was installing a new heater hose on a Cat D9R dozer. While moving the hose through a tight spot he received a laceration on the back of his left hand. He was taken to the hospital where it was glued back together.

Mechanic moving pins for a loader into storage, picked up a pin (approx 45 lbs) and strained his back while moving. Sought medical attention from family physician.

Operator was dismounting and mounting a Volvo articulating truck throughout his scheduled shift. Later in the evening, EE noticed pain in his knee and notified supervisor via text message that evening and sought medical tratment the following day.
