While working to assess for repair, bolt broke that holds clip securing safety chain of belly pan causing metal to impact LEFT index finger. On 3/06/24 met w/ employee and employee stated on 3/01/24 while performing maintenance of 390 bucket they felt irritation/soreness to RIGHT eye. Employee was taken to local Urgent Care for observation, sent them to eye doctor and diagnosed with foreign body to RIGHT eye.
