Employee was at the block hole on 1C belt between the track and scoop road. The employee was hand loading block when EE struck EE's left pinky finger between the corner of the scoop bucket and the block in EE's hand. it was approx 5'6'' high and damp in the area. the employee received 5 sutures and pain medication.

"Employee Stated EE has experienced knee pain in the past and a few days ago the employee stood up and felt a more severe pain in EE's right knee unaware of any single event occurring to cause this. Employee is scheduled for an MRI

The Employee was removing a wall panel off of the old 2E rock box when it became dislodged striking the employee in the right shin. The area was approximately 8 foot high and damp. Fracture of the anterior tibial pilon. This is where the tibia meets the foot (on top in the front).

Employee was removing a rock that was stuck between the spreader bar and the chain of the #5 S/C. The rock fell back striking EE's left ring finger against the frame causing an approximate 1 inch laceration. The area was approximately 6.5 high and damp. The rock measured 2.5' long X 1' wide X 18'' thick. The employee received 10 sutures.

The employee was was trying to stand a S/C tire up off the mine floor when EE strained their back. The area was approximately 65" high and damp.

Employee was loading machine and felt a sharp pain in back and right leg

Employee smashed EE's finger between the scoop door and the frame of the machine while exiting the scoop.

At approximately 3:50 a.m. an owl shift employee was helping with the belt move process and as they picked up a top belt roller and started walking they felt a strain in lower back. The employee was transported to the hospital where they were later taken off of work.

Employee was working on a new belt head set-up and as EE climbed on the drive roller to hang a chain the drive roller started to turn. The employee tried to stop the drive roller with EE's legs causing a strain in EE's left knee. Employee is currently off of work and EE's return is unknown.

Employee was struck in the left pinky finger while stacking belt structure onto a flat car. This resulted in a fracture and sutures in the employees finger.

Employee was trying to remove a piece of drill steel that was stuck in a drill steel chuck from a roof bolter and as the employee was beating on the chuck employee smashed a finger between the back of the roof bolting machine and the drill steel chuck. Employee received four sutures and returned to work on next regular shift.

Employee was dropping continuous miner cable and waterline and as employee raised up with the cable on employee's shoulder employee felt pain in lower back. The employee is currently off of work.

An employee had been building cribs under the feeder and as EE was trying to keep balance, EE's right index finger got between two pieces of metal near the jack area smashing the finger. The employee was brought outside and taken to the hospital where it was determined EE would lose part of the finger.

Employee was cleaning loose coal from the wear plate in the feeder when the sharp edge of the wear plate cut EE's left index finger. The employee was taken to the hospital where EE received 5 sutures.

Employee was dragging line curtain in the #7 entry when they tripped on a rock landing on their right shoulder. Employee was seen by a specialist where it was determined that they had a fractured scapula. Employee is currently off of work.

Employee was loading roof bolts on the roof bolting machine and as employee turned left handed employee's left forearm struck a roof bolt plate causing a laceration. Employee went to Raleigh General where employee received 9 sutures.

Employee was loading full oil cans in the scoop bucket when ee felt a pop in left hand/elbow/forearm. Employee finished shift and then went to the hospital where they reported ee may have some ligament damage so they suggested ee wore support. Employee received a full release and returned back to work the next day.

EE stated was beating an oil can on a cinder block to get dried B Bond out and EE's hand slipped cutting thumb.

Employee stated that they flopped the miner cable over & out of the way & felt a pain in their groin. Employee thinks they pulled a muscle in the groin area but finished the shift and reported the incident to the boss @ 12:30 (end of shift) at the bottom of the shaft. Employee didn't want, and refused an ambulance, stating they would apply ice to the area at home.

Employee was mixing B-Bond to plaster a stopping with when B-Bond splashed under employee's safety glasses in employee's right eye. Employee received eye drops and antibiotic cream for employee's eye.

Employee was putting in a top roller while moving belt when EE noticed that the belt clamp was stuck. When the employee reached down to free the belt clamp another employee grabbed the roller and caught EE's finger between the top rail and the belt roller smashing and cutting ring finger on right hand.

Employee was getting a piece of rock off of the continuous miner and when ee tried to flip the rock in the pan it caught right ring finger and right pinky finger on right hand causing fractures to the tips of both. Note: Employee returned to work on the next regular scheduled shift.

Employee was located near the bottom of the Service Hoist on the motor side of the Service Hoist. EE and another employee were hooking up a draw bar to the locomotive and EE got EE's left thumb caught in the coupler of the draw head. Minor fracture to EE's left thumb.

Drill steel was stuck together and while employee was pulling the steel apart it broke loose causing him to strike himself in the nose with his hand and drill steel.

Employee was trying to remove a rock from the ATRS and as he shoved it off the ATRS his left hand was pinched between the top of the rock and the mine roof as it fell forward towards the face.

Employee was attempting to change batteries on a scoop while working outby and the battery plug fell and struck his right middle finger. Employee finished his shift and received x-rays two days later that revealed the middle finger was broke.

Employee stated he was pulling dust hose from the pod duster to the #3 belt line and felt a pain in his lower back. Strained a muscle in his lower back.

Employee was removing the part from forklift jacks to place on Service Hoist when the part slipped and caught his left hand breaking his pinky finger. Employee finished his shift and reported incident at end of shift to management at approximately 7am (work 12 hour shift).

Employee was sampling flowable fill concrete at the Affinity Mine. Sometime during his shift he was splashed on the chin by two specks of wet concrete and did not notice until he got home that night. On 12/30 he sought medical attention and was diagnosed with two small second degree chemical burns on his face and prescribed oral antibiotics and an antibiotic cream.

Employee stated his back was hurting. Employee is a roof bolt operator. Loads bolt machine daily with bolt supplies, and installs roof support daily.

Employee was pulling a water hose and felt a pain in his belly button area. Doctor placed employee on restrictive duty on 06/21/13 and is on restrictive duty until his surgery is scheduled.

Employee was hanging miner cable in the #5 Entry to clean and felt a pain in his back.

Roof bolter operator was preparing to install an outside bolt in the row. He placed the bolt over his canopy and pulled down to bend it and felt a sharp pain in his lower left abdomen.

Belt man was connecting a plastic rock dust line and the line slipped and started to slide down the slope. He lunged forward to grab it and felt a sharp pain in his lower abdomen. He was diagnosed with a hernia that required surgery.

While plastering a stopping the employee got plaster (B-Bond) under the plaster glove he was wearing on his right hand that extended up his forearm. The next day he told his foreman about the incident. The following day his skin was red and swollen and he went to the doctor and was diagnosed with a chemical burn to his right forearm. This is to report medical treatment only.

While using a chain hoist to pull slack in the 2-C belt take-up the eyelet attached to the take-up cross member broke and the miner was struck in his left knee resulting in a contusion. He was placed on restricted duty.

While pulling belt together to pin the splice slack came in the belt and the employee struck his right hand against the belt structure. The employee received medical treatment in the form of a splint/cast and arm sling.

Employee was removing a band from a stack of blocks and cut his finger.

Employee was constructing a seal and while removing gob from a seal site a small (softball size) piece of gob rolled off a pile of gob approximately 24 inches and struck the employee on the thigh area causing a pulled hamstring.

Employee was crossing the tall piece and put his left hand down on a hot down-drill bit. This resulted in a 2nd degree burn to his left hand. He returned to his regular duty as a scoop operator on his next scheduled shift 31*

Employee was carrying 3" lay-flat waterline to a pump in #4 entry near the shaft bottom when he felt pain in his lower back.

A crew was trying to assemble a belt drive system. Discharge boom needed adjusted to align into drive frame. While adjusting, the discharge boom dropped striking an employee's left foot (fracture).

Two 16' steel jacks were being moved from the bottom of shaft 1 to the rib by IEE and two other men when EE felt a sharp pain in his groin area. He continued to work and said that he would get it checked out. EE continued to work until he was taken off work ( 8/18/2011)by his doctor for surgery to repair hernia in groin area.
