Ee was removing driveline from a D10 dozer. While removing the bolts from the U joint fasteners, the driveline pivoted pinching the ee's right thumb between the yoke and the driveline. Seen at ER, diagnosed with a fracture and a laceration requiring stitches. Released to restricted duty.

While working on a cooling pump of a 4 yard loader, the access door came unlatched hitting the employee on the back of EE's hard hat. When hit on the hard hat the force thrust EE forward causing EE to hit EE's nose on the loader frame.

Ee was carrying a box on shoulder when it started to slip. Ee reached with right arm to catch it and pulled shoulder. Ee was seen at the clinic, diagnosed with a right rotator cuff strain, and released to restricted duty which the site did accommodate.

Ee was working on radiator grill, it slipped and struck the machine, and hit the ee in the right ear. Laceration required stitches. Ee released to full duty.

While adding 12' screen casing, the casing being put into the derrick hung up on the elevator ring of the previous piece of casing at the table and the casing being added bounced up striking the employee on the point of the jaw.

Ee was moving metal fittings with two coworkers. As EE bent down and began to lift, EE felt a sharp pain in EE's low back. EE stopped the lift and was taken to the ER for evaluation. EE was diagnosed with an acute muscle strain and spasm, and was released with restrictions that the site could not accommodate.

Ee was pulling bolts, one of the bolt heads came loose and spun, cutting EE's thumb. Seen at clinic and stitches were required to close the wound. Ee released to full duty.

Ee stepped off dozer, foot caught in grating and ee hyperextended left calf/Achilles when ee landed on left toe. Seen at ER, diagnosed with strained left calf. Follow-up confirmed diagnosis, ee released with restrictions site could not accommodate.

Ee was rolling up a cable reel and left thumb became caught and was crushed. Seen at ER, diagnosed with open displaced fracture of distal tuft, distal phalanx of thumb. Referral to orthopedic surgeon resulted in surgery. Ee lost time.

Ee was pulling on hose and it broke free, striking ee in the mouth. Seen at the dentist, diagnosed with 3 broken/chipped teeth, one of which had to be extracted. Ee released to restricted duty for two days.

Interwest Supply employee (contractor) was working with a Barrick employee unloading 2x2 steel tubing from forklift onto steel rack hitting contractor on the forearm causing a cut. The contractor was seen at the ER and stitches were required to close the wound. EE was released to full duty effective 03/20/2017.

Ee was cleaning an anode when it started to slip. Ee reached for the anode and cut EE's hand on the sop edge of the anode plate. Seen at the clinic and stitches were required to close the wound. Ee released to restricted duty which the site can accommodate.

Employee was changing the position of the return hose flange and when the flange came off the last bolts holding it, it slid back and pinched thumb between flange and cyclone brace.

Employee walked into water to shut off a valve and felt lt. knee "pop"

Ee was replacing left rear suspension, left finger caught between collar and axle box frame. Sutures required and ee released to restricted duty.

Putting lifter on BQ pipe and sub with lifter tipped over catching employee's little finger between the sub and the BQ pipe.

Ee was climbing ladder on equipment when he felt a pop in the upper right side of his back. He was seen at the ER, diagnosed with a strain and muscle spasm, and released to full duty. On follow-up on 12/01/15, ee was put on restricted duty, making this reportable for restricted duty.

Ee was observing a job when a tech hit a rail with a hammer, dislodging a piece of metal that struck the ee in the abdomen. The ee was seen at the ER where the metal was removed a stitches were required. Full duty release.

Ee was throwing crushed air filters into metal bin when a sharp edge caught his left pinky finger, cutting it. Ee seen at ER and received stitches to close the wound. Released to full duty.

Ee was pushing a sample catcher, it slipped and hit the ee's finger. Laceration to right index finger needed stitches. Ee released to full duty for next shift.

A slack cable came under tension suddenly striking the employee resulting in four broken ribs and a splenectomy.

The worker was struck in the mouth with the end of a 4" discharge hose fitting when it recoiled while removing it from an electric pump. The worker received a small laceration and a chipped tooth.

Ee was changing a fitting on an emulsion hose and his hand slipped, cutting his right pinky finger. He was seen at the clinic where sutures were required to close the cut. The ee was released to restricted duty.

Ee was holding a nut between two header pipes and his hand was pinched between two pipes when one of the pipes moved. Ee was seen at the clinic, diagnosed with a contusion to the hand, and released with the restriction that he must wear a splint. No fracture identified.

Ee was lifting a heater to move to work area when he felt pain in his groin area. Seen at hospital, diagnosed with possible hernia. Seen at surgeon, told hernia was healing on it's own and retracting, ee released to full duty effective 01/06/15. Ee worked one restricted duty shift and then had scheduled days off.

Ee bent over to pick up his socket set weighing about 50 pounds, and when he lifted he felt pain in his low back. He was taken to the ER, diagnosed with a strain, and released to sedentary duty which the mine site was able to accommodate.

Employee was working underground and was struck on the upper lip by a bolt that fell over. He was taken to the clinic and received stitches to close the wound. He was released to full duty.

Employee was loading 5' core barrel onto rod truck when core barrel fell on fingers. Employee sustained an open fracture to the tip of the ring finger on his left hand.

While using a water lance to clean the scale from a pipe, the water stream contacted the employees right forearm, causing some abrasions and a small injection wound.

Employee was lifting boxes out of totes. The employee states after lifting a box he felt pain in his groin. After medical treatment the pain was revealed to be coming from his hip.

Ee was removing an air drop from a line and was struck in the face by the drop. He was seen by a doctor and sutures were required for the laceration to the face. Other diagnostic testing negative, employee released to full duty.

Ee was installing a light fixture. He mis-stepped and grabbed the light fixture, cutting his right hand on the sheet metal frame. Stitches required and released to full duty.

AS employee was exiting loader she smashed her finger in the cab door from the outside while trying to close it.

Ee was doing a pre-op inspection of a transmixer. He pushed the top hatch to inspect the inside of the drum and the hatch fell, landing on his right hand. Seen by doctor, no fracture, but stitches required to close the wound. Ee released to full duty.

Ee was climbing the ladder onto piece of equipment and felt pain in his lower back. He was seen by medical provider and diagnosed with lumbar spasms and a muscle strain. Released to restricted duty.

Ee climbing up the ladder of a loader, missed a step and caught himself with his right arm. Heard a pop, felt pain in shoulder. Seen at ER, released with restrictions pending follow-up with orthopedist. Orthopedist continued restrictions and ordered physical therapy. Ee noted previous issue with this shoulder.

Employee was removing a latch and pinched his right ring finger when the lock bar caught the finger. He was seen at the ER and sutures were required to close the laceration to the finger. Released to restricted duty.

Employee was lifting sample bags and twisted when she felt pain in her low back. She reported to her supervisor and the following day she was seen at the clinic. She was diagnosed with a lumbar strain and released to restricted duty.

Crew was working on changing teflons on the upper deck, when they went to slide the upper block out the employee thought he had the plate, it slid out at a different angle than anticipated and he did not get his hand fully out of the way. His left index finger was pinched between the block and the feed frame.

Employee was lifting a pin to place into rear bed pin hole of a 795F haul truck bed when the pin slipped out of his right hand and fell on his left hand, smashing the left middle finger causing a laceration requiring two sutures.

After the Helper unlatched the overshot and began lowering it to employee, the overshot slipped out of the helper' s hand and landed on the employee's right foot.

The employee was lowering a screen into a tank and his hand was resting on a handrail. A hook on the screen pinched his hand. He was seen at the ER where he received stitches to close the wound. Released to full duty.

Employee was closing door to drill and caught his finger in the door. Seen at the ER where xrays were negative and the wound was closed with sutures. Employee put off work until 02/07/2013.

Two employees where working in a man basket removing pendent line cables from the boom of a P&H shovel. The employees lost control of the pin that was holding the pendent line causing the pin to fall into the man basket and strike one employee in the left foot. This caused two transverse fractures to the employees 2nd metatarsal bone.

An employee's wrist was caught between the excavator and ring steel. Claimant initially refused medical treatment, but later disclosed his wrist was still bothering him. On 01/12/13, X-Rays confirmed a hairline fracture to the wrist.

Employee was taking apart 4" hoses that led from the Swaco pump to the sump. He unlocked the camlock and as he pulled the hoses apart the female end of the hose flipped up and struck him in the left nostril.

While removing a steel cylinder, the cylinder moved and pinched the left palm at the base of the thumb between the pin and the jack. Seen at ER, received stitches and released to full duty. On follow up, released to restricted duty.

As the employee was lifting a piece of grating to set it on top of an existing piece he lost his balance and he dropped it. As the piece of grating was falling back to its place it caught the employees finger between it and the other piece of grating that was already in place.

Employee was hit in right forearm by stabilizer bar. Was seen at the ER and directed to follow up. On follow-up was released to restricted duty.

While driller was loosening pressure bolts on the actuator plate, the plate broke free from the bolts and struck the driller's left pinky finger causing a laceration below the fingernail.
