While changing a belt on a roller, employee was checking alignment of belt by pulling through roller, momentum pulled fingers of right hand in contact with roller causing injury.

Employee was working on number 1 tray in unit 345 when caustic sprayed into employees eyes. Full proper PPE was on at the time of the incident.
Employee was moving a 2 inch diameter shaft, 16 foot long and fell on left hand.
OP4 bent over to lift a six inch hose so it could be relocated for the portable sump pump. When EE attempted to resume an upright position, EE felt a sharp stinging pain in the middle of EE's lower back.
Employee was late reporting a job-related injury (6/11/17) to plant nurse. On the morning of 6/18/2017 employee reported infected callus to nurse. Upon observation, caustic burn to outside of right big toe noted.
Operator was closing 14" vapor valve on outside of heater deck on west side of 340 area during a routine heater swap. EE felt a strain in left groin area. Nurse came to plant site and a decision was made to bring ee to the emergency room to get further evaluated.
OP3 was in area 335 turning a valve on #8 press when they felt a pain in the right abdomen that radiated down into groin. Employee seen by doctor for evaluation.
White side supervisor was in the process of switching fill pumps when the surge tank overflowed. The sump pump could not keep up with the flow. The liquor level rose quickly and entered EE's boots resulting in thermal and caustic burns to both lower extremities. Transported by RN to Baton Rouge General ER and hospitalized on Burn Unit.
Operator was performing task of changing out condensate filters & was in the process of putting the condensate pot back into service. When operator started opening the condensate valve to fill the pot, hot condensate started spraying out from the pot gasket & made contact with right abdomen, right hip, both thighs & buttocks. Initial diagnosis was 2nd degree burns to all areas.
EE used improper rigging technique when rigging a lever hoist with 2 sling chokers spread & connected to hook at lever. Tightened up on load choker slipped to safety latch causing it to break. Block & lever sprung & hit ee in mouth leaving a minor laceration & chipping 2 teeth. EE was prescribed an antibiotic to prevent infection inside mouth.
GRM was working on ground floor of area 350 cold end. He was on a clean-up project following a turnaround when a pan filter support roller fell down a tugger well and struck him on the right foot. X-ray at Gulf Coast Occ Med revealed a distal tuft fracture to the big toe on the right foot. Returned to work regular duty with no restrictions.
GRM was at rod mill #2 washing the screen on the north end. When he got splashed with caustic water, he immediately went to eyewash to rinse, When he removed his goggles, caustic dripped in the corner of his right eye. Washed in medical dept by RN for 45 minutes and returned to work. Seen by Dr. on morning of 3/27/15 for redness & swelling. Prescription medication ordered.
Employee had chemical contact to right eye.
While working, employee was opening clam shell on reclaim chute. He got his left small finger smashed between the clam shell handle and the flop gate handle resulting in left little finger getting smashed with small laceration to the tip. X-ray revealed finger tip fracture.
Store clerk was in yard behind stores stacking pallets, when he dropped a pallet and it smashed left ring finger. Employee was not wearing his proper PPE at the time of the accident.
Employee was removing filter pan side shield pulling upward, when side shield broke free the employee hand moving in an upward motion struck a 1/2 pipe nipple; causing a puncture wound. Note:Employee received stitches and pain medication.
CAH was in calcination area shutting down #2 injection pump from caustic cleaning. When shut down, he opened the drain, and the drain on the suction sprayed the left lower leg. Rinsed underneath #1 hytank for 5 minutes before going to dispensary for 20 minute shower. Evaluated by RN and seen by Dr. for pain medication. Returned to work on 09/30/14.
GRM was changing filter sock at #1 baghouse on 350 hot end when he felt pain in right elbow & forearm area. Taken to Dr. for right arm strain and given OTC medication and returned to work with no restrictions. Primary doctor ordered physical therapy on 10/27/14-11/28/14. Taken out of work 12/02/14 through 2/1/15. RTW on 2/2/15. 40 LTA days.
Employee had caustic splashed in both eyes when coworker dropped impact tool in puddle of caustic on ground. Washed eyes at safety shower, then went to medical dept. where eyes were washed more by supervisor. RN called out and transported to hospital ER. Employee treated and released with prescription for pain medication.
Employee reported he was working as a trayman under the precipitator getting ready to use the Bobcat to remove hydrate from Area 345. When he stepped upward, he missed the cross member step, resulting in the inner aspect of his lower right leg slipping between the bucket and the step. Two inch superficial abrasion resulted from the contact with the step. Seen by RN & MD on 8/4/14.
While on the press floor attempting to unplug a floor drain, OP3 repositioned his air lance, thereby causing caustic on the floor to shoot upwards, knocking off his hard hat(with face shield) and goggles.Immediately rinsed at nearby eyewash station,then supervisor accompanied to dispensary for eye washing & shower, and he was transported to hospital for treatment.
