While on ladder, EE was pulling slack out of the new skirting being installed on the conveyor belt #1 and strained or pulled lower back. Continued to do light duty work rest of the day.
Cleaning up the discharge blower chute on the Mill. EE attempted to empty the discharge flap, failed to notice a bunge cord that was securing the flap closed and pushed the flap open which caused the bunge cord to break and slip free from the flap striking EE in the lower nose and upper lip area.
Strained/pulled muscles in lower back while emptying 50 lb bags of product. Went to Custer Regional Medical Emergency Room, Custer, SD.
Employee was stacking 50# bags on a pallet and pulled muscles in lower back. Employee was released to come back to work on 8/25/08 but chose not to return until 8/26/08. Treated at Custer Regional Clinic by Dr for lower back strain.
At home in evening he removed a flake of mica from his R eye. On Sun, 8/10 went to ER & was treated (found nothing in eye) - given antibiotics. 8/11 went to Advanced Eye Care. Was told eye was scratched & was given eye drops. 8/11 reported issue to Pit Mgr. 8/15 went back to Advanced Eye Care for more treatment - did not work, Returned to work 8/18/08 able to see out of eye.
Employee was stacking 50# bags on a pallet and over-extended his right elbow. Treated at Custer Regional by doctor for lateral epicandylitis
Employee stacked 48 50# bags on a pallet. He then went to get forklift. He removed the portable tank heater and hung it on a hook on the wall. He experienced a sharp pain in his lower back. He tried to return to stacking bags but the pain persisted in his lower back. He went to the Custer Regional Medical Clinic were he received 4 lidocaine injection in his lower back and med.
EE WAS AT THE EXPLOSIVES MAGAZINE. HE WAS TRYING TO OPEN A JAMMED DOOR (THE WOOD ON THE BACK OF THE DOOR EXPANDED DUE TO MOISTURE). HE STRAINED HIS LOWER BACK. HE REPORTED THE INCIDENT BUT DID NOT SEEK TREATMENT. ON 5/22 HE WAS BARRING ORE IN THE CRUSHER PIT AND REINJURED HIS BACK. HE IS NOW BEING TREATED BY A CHIROPRACTOR & MAY BE SCHEDULED FOR FURTHER MEDICAL TREATMENT.
BENT TO SLIDE AND LIFT A BOX OUT OF A VEHICLE AND PULL OR STRAINED BACK MUSCLES. PROGRESSIVELY GOT WORSE BY NEXT DAY. HE MISSED NO TIME OFF WORK. HE WENT TO LAMMERS CHIROPRACTIC CLINIC ON 12/6 /01, 12/7/01 AND 12/10/01.
EE WAS LOADING 50 LB BAGS OF FELDSPAR. HE BENT OVER THE HELP FOLD A TARP THAT WAS COVERING THE MATERIAL AND PULLED THE MUSCLES IN HIS LOWER BACK.
EMPLOYEE WAS CLEANING MUD FROM PCV PIPE HE SEPARATED THE TOP & BOTTOM SECTIONS OF PCV PIPE THE TOP SECTION OF PIPE BROKE LOOSE & HIT HIM IN THE CALF OF THE LEFT LEG WENT TO EMERGENCY ROOM FOR CLEANING OUT & STITCHING OF WOUND OFF WORK 10-14DAYS
EMPLOYEE WAS LAYING BRICK ON INSIDE OF BALL MILL AT THE FELDSPAR PLANT HE KNEELED PART OF THE TIME TO DO THIS ACTIVITY HE RECEIVED CONCRETE BURNS ON HIS KNEES EMPLOYEE VISITED THE CLINIC IN CU STER ONE TIME & RECEIVED FIRST AID & MEDICATION 09-23-94 HAD A RECHECK AT CLINIC ON 09-26-94
EE WAS WORKING ON AN ENGINE AT WORK. HE WAS REACHING UNDERNEATH TRYING TO REPAIR THE FUEL LINE. HIS HEAD WAS AGAINST THE ALTERNATOR AND HE WAS REACHING QUITE FORCEFULLY UNDERNEATH THE ENGINE T O REACH THE FUELING. HE FELT STRAIN AND SHARP PAIN SENSATION IN HIS UPPER BACK AND BETWEEN THE SHOULDER BLADES. EE UNDER CARE OF CHIROPRACTOR EE TWO POSSIBLE THREE VISITS.
EE WAS PULLING BELTED DRIVE BUCKETS BY HAND TO INSPECT THE BELT AND BUCKETS. HE STRAINED BACK AND EXPERIENCED PAIN IN HIS LOWER BACK JUST ABOVE THE HIPS.
EMPLOYEE WAS WORKING ON REMOVING OLD SILICONE FROM A V-SCREEN. HEW AS IN A CROUCHED POSITION WHILE DOING THIS TASK. LATER WHEN HE WENT INTO THE OFFICE AND REACHED FOR DOOR TO OPEN IT, HE EXPE RIENCED A SHARP PAIN IN HIS LEFT LOWER BACK. PAIN CONTINUED ON AND OFF.
EMPLOYEE WAS STACKING 50# BAGS OF MATERIAL ONTO A PALLET. HE TWISTED TOO QUICKLY AND STRAINED BACK. HE EXPERIENCED PAIN BETWEEN SHOULDER BLADS AND HAD NUMBNESS IN HANDS.
EMPLOYEE WAS STACKING 50# BAGS OF BRITE-X ONTO A PALLET.THE EMPLOYEE TURNED FROM THE WAIST TO PLACE A BAG ON THE PALLET AND PULLED MUSCLES INHIS LOWER BACK.EMPLOYEE HAD THREE TREATMENTS AT DR LAMMERS CHIROPT\RACTIC IN CUSTER SD.
EMPLOYEE WAS SHOVELING BRIST-X INTO BOBCAT BUCKET TO CLEAN UP THE PLANT AREA. EMPLOYEE LIFTED SHOVEL FULL OF MATERIAL AND STRAINED MUSCLE IN LOWER BACK. EMPLOYEE RECEIVED SIX CHIROPRACTIC TREA TMENTS.
EMPLOYEE WAS SHORTENING A CHAIN BY REMOVING LINKS. A PIN ON ONE OF THE LINKS SLIPPED AND IT DROVE A PIECE OF METAL INTO THE EMPLOYEE LEFT THUMB.
