Employee picked up a bag containing training materials and felt a twist in her back which didn't present pain at the time of the incident. The next she day wasn't sure if the pain was from cramps or kidney. She did not seek medical attention until June 19, 2011 and she reported the back pain issue to her immediate supervisor on June 20, 2011

Mostrando los 4Employee was moving stabilizer from pipe truck to drill utilizing the winch on the drill. Employee had hold of stabilizer, and the momentum of the tool forced him into the rig, causing him to hit his fingers on the hammer that was stationary in the rod table.
EE was working on the 1800 screening plant when he removed a rock from the tail pulley. The belt rolled back and caught his left hand in the pulley. He then used his right hand to push another employee out of the way, straining his right shoulder. EE started receiving physical therapy on his right shoulder 07/28/2010. Turning this into a medical treatment.
Employee was unloading scrap metal out of a 10 wheel truck. The metal stuck to tailgate as it was being unloaded. Employee lowered the truck's bed and went to loosen the tailgate to release the stuck material. The tailgate released and pinned the employee's left arm between the tailgate and bed of the truck, breaking his left arm.
