At approx. 9:20 AM, EE was installing a side shift slide on FL177. While reattaching the carriage to the mast, it slipped and fell, pinching R index finger between the backrest and mast. EE sustained a fractured n lacerated finger, requiring sutures, x-ray, splint, and medication. Able to work-restricted from using (r) hand at work but later changed to limited use of (r) hand.

While climbing down on fixed ladder of dump bed, EE got their finger caught on nonslip bar of the rungs and tore the skin on EE's left ring finger. EE stated took gloves off because it was hard for EE to get nuts and bolts out of EE's jean pocket. EE got stitches for injury and released back for work.
7am n 11am on 10 29 21 EE W was working with EE J performing maint. on an excavator bucket, pin wgt 80# EE W decided to lift the pin n not use equipment provided. Witness warned EE of weight but decided to lift anyway, while lifting EE twisted, heard a pop. EE kept working then later reported to lead. EE taken to med facility, lumbar strain, given medication and work restriction
At approx. 8:30am EE reported to supervisor EE's back was hurting from lifting Rotex lid. EE lifted an aluminum Rx screen lid during a maint. activity by 4 operators at the time. This is a common activity typically done by four teammates. This teammate was experiencing back spasm following this event. Received medical attn.,given meds and light duty. Thoracic back strain.
Feeling discomfort in lower back. EE went up to load railcar, got a screener basket put it in the hole and started loading railcar. When pulling the basket out EE's back started to feel funny. When finished sealing the railcar EE told supervisor. EE was removed from task to office type work. EE requested medical attention on 1/27/21 and Dr. stated to remain on office type work.
While setting rings, the ring sprung and hit hardhat causing neck sprain.
EE's R thumb was pinched monitoring FF equipment. EE was unaware of where EE placed hand resulting in the pinch occurring. After investigating the area it was clear that there were no moving parts in the location of the injury. It is believed that EE's thumb was pinched between the clasp on the electrical control panel door. EE was not wearing gloves. EE got 5 stitches to R thumb
EE's were replacing the Rotap assembly with a rebuilt assembly. When pulling on old Rotap IE felt a burning/ stinging sensation in IE's left lower side. EE and IE carried the used Rotap to a table approximately 20 feet away. Then both teammates installed the new assembly.
EE reports EE was kneeling to get to a grease fitting on cv303. Before EE got to the grating EE felt a pain in EE's left knee. EE stood back up immediately.
While lifting a 35 pound bag of cat litter to be run through a splitter for gathering test samples, EE felt a pulling/popping sensation in EE's right shoulder area. Able to work- Sprain of shoulder rotator cuff- no lifting 5 -10# - take meds. -home exercise program -refer to physical therapy.
While assisting another EE closing gate EE put(r) hand in path of traveling gate and finger became pinched under guide roller causing fracture to 4th finger on (r) hand.
Temporary worker attempted to lift a case & stack it on a pallet when EE felt a sharp pain in EE's back. Diagnosis: lower thoracic pain & spasm. Restrictions of may lift up to 10 lbs, no twisting & bending.
Changing hose, without required assistance, from one tank to another. Unhooked hose & it started sliding off pipe, so EE had to catch it. When EE caught hose EE heard EE's back pop. Completed hose change with one hand, held EE's back with the other. Diagnosis: acute lumbar pain & radicular symptoms. Restrictions: may lift up to 10 lbs., no climbing, bending or stooping.
Assisted with removing door pins & door shifted pinching/crushing left index & middle fingers between washer & mill base. Both fingers fractured & both nail beds significantly disrupted. Surgery performed w/ both fingers the distal phalanx was pinned with K-wire & repaired both nail beds with small suture.
EE was turning disconnect back on for rotary valve at DC102. EE was extending left arm upward & struck metal attached to disconnect causing a laceration that required 9 sutures. No restrictions.
Conveyor belt not running, so employee tightened tail pulley end. He reached in with his right middle finger to check the tension when belt started turning & his finger was caught between head pulley and belt causing a laceration requiring sutures.
Power failure, blower motor filled with dust. Cleaning dust from fan motor, glove caught between housing & fan blade edge causing fingertip/fracture of right middle finger. Required 6 sutures.
Employee was bending and lifting material during the work day.
Lowering spacer bars off of Wms Mill 1 platform to another EE at ground level. EE lowering bar hands slipped, caught bar weight on left thigh when EE on ground pulled bar. Burr on bar cut thigh. Required sutures & tetanus. EE did not notice burr & did not make accommodation for wet conditions.
Was assisting bags loading into load to tilt chamber of OMW. He opened guard reached into chamber and the tilt chamber came down catching his right arm between tilt chamber and conveyor. Laceration requiring 4 sutures. No work for 1 day.
EE was transferring powder. Disconnected hose & was looking inside hose when suddenly powder started shooting out of hose. Right eye corneal abrasion requiring prescription.
Removing roll up door spool. When he cut door support loose it struck him on top of left arm & wrist. Sustained left arm & wrist fracture. Taken to ER & was admitted.
Wrapped receptacle with electrical tape. Used thumb to break tape when tape broke his right hand was cut by tab sticking out on the receptacle. Diagnosis: right hand laceration requiring sutures. Restriction - no use of right hand for physical activity for 10 days.
Employee crushed his right 4th finger when closing rear door of spot truck. Diagnosis: open fracture, wearing splint, no use of rt hand & scheduled with Orthopedic doctor.
Performing maintenance on bulk loading spout. Adjusted cable & when spout began traveling up cable, the attachment came apart & eye bolt scraped right middle finger, causing laceration. Required 4 sutures and restricted with no use of right hand.
Climbed up platform to clean high bin eyes, reached under light conduit, when he felt sharp pain in lower back. Diagnosis low back strain and has restrictions.
Removing a stuck pallet. Employee notified supervisor on 9-10-10 that he had been diagnosed with a hernia on 9-1-10. He said it was caused by an injury on 7-30-10, which injury had not been reported to the company.
Bending over to lift empty boxes felt sharp pain in lower back. Diagnosis hip flexor muscle strain. Light duty.
Turning conduit and felt a sharp pain run through his right wrist. Also, connecting hoses and felt sharper pain in same area of same wrist. Diagnosis: wrist ligament/tendon strain with lifting restriction.
Two employees had made adjustments to regulator for spray nozzles. Both were watching gages and spray nozzles when the accumulator came apart and acid fell. This employee received 1st & 2nd dgree burns to shoulder, both arms and leg.

Removing bags, pushed e-stop to disable machine. Accidently bumped & activated limit switch to pusher plate. Left hand & arm caught between pusher cylinder plate & guide rails. Left wrist sprain. Restricted with splint.
Injury had a 6 ft cross tie raised & he was holding it. He was sliding cross tie under raised tie when he caught his left 3rd finger between the 2 cross ties. Open fracture left 3rd finger w/ nail bed reconstruction.
EE placed his hand under trash bin while he was attempting to unlatch the dump lever. When the lever unlatched, the bin dropped down and struck two fingers. Laceration of left index and middle finger.
EE opening chute on railcar with left hand and placed his right hand down on moving conveyor. Right hand was pulled between conveyor and roll wheels. Another employee unplugged the conveyor. Right ring finger fracture.
He reached down and lifted a 50 lb bag and felt pain in his lower back. Diagnosis was acute paralumbar muscle strain w/ spasm and radicular symptoms into the left extremities. No work until better.
EE was carrying a 5hp electrical motor for replacement on a piece of equipment. He felt a dull pain in the right side groin area and did not initially want to see doctor. Pain did not subside, so on 12 11 08 the employee decided he wanted to see a doctor. He is on light duty with no lifting more than 5lbs. Recheck on 12 19 08 - hernia.
Ridgeway was cleaning a dust line and dust got into his right eye even though he had safety glasses. He flushed the dust out of his eye. It was irritated, so he was sent to the Dr. He was treated for conjunctivitous in the right eye and given eye drops for seven days. no restrictions.
EE was hepling to remove a mill roll attached to crane ball & positioned half way on platform. They tried to push the roll it begain to turn clockwise. He tried to move but roll hit him on his left leg & left foot. Bruised left leg & foot. Knee swollen but there is no evidence of fracture, dislocation,or subluxation. He is on light duty and scheduled for knee MRI.
EE was carrying an air cylinder on his shoulder. He lowered cylinder to work bench and right index finger got caught between cylinder and work bench. Laceration requiring sutures. Limited use of hand. No lost time.
Employee had a prior incident on 2/27/08 that has now become a reportable on 10/1/08. In Feb 08 he was pushing a lever on a hopper when he felt a pop in his right elbow. He was complaining of pain so he was sent to the Dr on 10/1/08 and diagnosed was tendernitis in the right elbow with restrictions of no lifting pushing or pulling greater than 10lbs with the right arm.
Employee was clearing a jam at a bag kicker. The conveyor belt had worn a sharpe edge on the belt scraper. The employee cut the 3rd and 4th finger on his right hand on a sharpe edge of the belt scraper as he was removing the jammed bag.

Employee attempted to reposition a mobile compressor without assistance. Lost control and ran compressor into a stack of wooden pallets, pinching right index finger between the pintle hitch on the compressor and the stack of pallets.
Employee was restacking pallets when he felt a pain in his lower groin. Treating physician diagnoised a strained groin muscle and placed employee on 3 weeks of restricted duty until condition improves. The employee lost no time.
Employee was walking a basket of invoices from a cabinet overhead with her right arm and felt pain in right upper arm and shoulder area.

WHILE ADJUSTING A TAIL-PULLEY DOWN, EE STARTED HAVING SOME DISCOMFORT ON LEFT SIDE OF CHEST
MOVING BULKHEAD DOOR ON RAILCAR. GOT HAND PINCHED BY DOOR.
RAILCAR DOOR WAS DIFFICULT TO OPEN. THE EE ALONG WITH ANOTHER TRIED TO OPEN IT TOGETHER. EE FELT A SHARP PAIN IN RIGHT UPPER ARM.
