Employee was moving crusher break plates in storage building. One of the plates started to fall over and EE pushed it back with EE's foot and felt a pop in EE's knee. EE went for treatment on 03/19/2026 and was diagnosed with a right knee injury and primary osteoarthritis.

IP was attempting to find a leak in a hydraulic system on EE's repump truck. While moving a line, hydraulic fluid was injected into right hand index finger.

Employee did not seek treatment until 02/25/2025. A pin for the excavator bucket was being knocked out and when it came out, instead of letting it fall, the employee held onto it, not realizing the weight. Employee's left ring finger was smashed in between the pin and the excavator bucket. The employee received a fractured finger and two sutures.

One employee was handing a screen to another employee and the screen slipped from employee's hands. The injured employee was struck with the screen in the right pectoral suffering a chest wall sprain.

While changing steels on the roof bolter, the steel slipped from the employee's hand. It struck EE on the top of EE's left foot behind the steel toed portion of EE's boot. EE was diagnosed with a fracture of the 4th metatarsal bone.

Employee was installing a 7' roof bolt when EE felt a pain in the arm. EE was diagnosed with a left rotator cuff tear.

Employee opened the door to enter the water truck and a piece of metal fell into their eye. Employee had to have the imbedded piece of metal removed.

Employee strained back while cutting and resetting the hoist cable on the dragline. The employee did not notify foreman about the injury until 04/02/2021 after they went to see a physician. The employee was scheduled off on 04/02/2021. Employee finished shift on 03/31/2021 and worked on 04/01/2021 without notifying anyone of the injury.

Employee was helping to remove a pin from a CAT 990 loader. When the pin released from the bell crank, it hit the frame, upended, and slid out of the supporting sling. It landed on the employees toes of left foot.

Employee was moving the belt guard for the scalping screen. When they turned with the guard, they felt a pop in the lower back. Employee suffered a back strain.

Employee was pulling a rope attached to a water suction hose and they felt their wrist pop. They were diagnosed with a wrist sprain.

Employee was lifting a 5 gallon bucket of oil and EE felt a pop in EE's left arm. EE was diagnosed with a torn bicep muscle.

Employee was climbing down the ladder, exiting the haul truck when EE felt a pain in EE's chest. EE was diagnosed with a strained chest muscle.

Employee was unloading material from the back of the plant truck when employee felt pain in groin. Employee was diagnosed with a hernia. This was not a restricted duty or lost time injury until employee had surgery on 06/15/18.

Injured was installing tail pulley guard on the dump belt under the crusher. As they were trying to position the guard, employee suffered low back strain.

The employee was installing a transmission guard on a piece of Caterpillar equipment and caught employee's right foot between the guard and frame. Employee broke bone in top of foot. Employee did not seek medical attention until 3-5-2017.

Employee was lifting a bracket for a conveyor and EE felt pain in EE's abdominal area. EE went to see a physician on 04/06/2016 and a small sliver of metal was removed. EE was also diagnosed with a small abdominal hernia.

Employee was changing gates on the dry screen when he suffered an umbilical hernia.

Employee was changing screens at the plant when he went to turn while standing, his knee twisted and he felt a pop.

The employee was sliding a barrel onto the forks of a telehandler when he felt a pain in his lower back and right leg.

Employee was pulling the suction hose for a water pump when he felt a pain in his shoulder.

Stone was being lifted out of a bin using a crane and sling. The employee was placing the sling around the stone to be lifted. When the stone began to be lifted, it slipped out of the sling and rolled into the employee's left knee causing a patella fracture.

The employee turned a water valve on to a hose so that he could rinse off his truck. The pressure from the water caused the nozzle to separate from the hose. The nozzle or hose clamp struck him in the back of the right hand, causing a laceration which required 8 sutures.

Employee was changing slide gates when he lifted one and felt a pain in his left groin. He went to the Emergency Room and diagnosed with a hernia. He returned to work on regular duty the following day and will have surgery to have the hernia repaired at a later date.

Employee was using a flat hoe to clean material from the 2A chute when a piece of stone came up and hit him in the mouth, chipping his front tooth. He went to the dentist and had a cap put on the chipped tooth.

EE was getting ready to install a new battery terminal on the customer's PC400LC-5L. He was cleaning the corrosion off of the existing terminal, using a wire brush, when a piece of corrosion flew into his eye underneath his safety glasses.

Employee was lifting a cabinet into a loader bucket when he injured his bicep. He went to a physician who referred him to an orthopedic specialist.

Employee was lifting trail cable and he felt a snap in his right shoulder.

Employee was sliding return roller off of shelf and it began to fall, when he caught the roller he felt a pain in his right shoulder. Employee attended physical therapy and then had surgery to repair damage.

While picking up 5 gallon buckets of oil and putting them into the manlift basket, the worker injured his right shoulder. The worker went to a physician on 10/9/2009 and was given a prescription for physical therapy. The worker had surgery on 12/23/09.

While unloading an oxygen tank from a pick-up truck the worker felt a pain in his right hamstring. The worker went to a physician on 1/8/2009 and was diagnosed with a right hamstring strain. This injury did not result in any lost time or restricted duty.

While perfoming maintenance on our drill, the injured was working on a brake drum, it slipped and came down on right index finger and cut tip of finger off.

Loosening a filter when it quickly came loose the man strained right hand.

While dumping a bucket of limestone, he pulled a muscle in lower back. He required pain medication.

The man was loading an oxygen bottle on a truck. His right wrist gave way. Went to emergency room and was put on light duty for a strained wrist. If light duty persists too long, he may miss some time as we do not have much light duty oportunities.

While repairing the air line on a 100 ton rock truck a blockage came free and struck the man on the arm. Was treated at emergency room and released to work. The next day pain and redness occurred and he went back to the emergency room. He was in hospital 1 day and released back to work.

The injured man felt a pull in his shoulder when loosening a very tight bolt. He reported the incident, but felt that no medical attention was required. He has not missed any work, but went to Dr. 7/14/06, and was diagnosed with tendinitis and he has lifting restrictions until further notice.

While changing screens the employee felt pain in right elbow. She will receive therapy and was prescribed pain medication. No lost time.

EMPLOYEE WAS TAKING AN AGLIME SAMPLE FROM A SAMPLE POINT. THE POINT WAS PLUGGED WITH LIME, HE PUSHED A HOSE IN THE POINT TO UNCLOG INSTEAD OF TAPPING THE POINT. THE HOSE CAUGHT HIS FINGER AND PULLED IT IN THE POINT & HE LOST HIS RT. INDEX FINGERNAIL. EE WAS TREATED AT THE E.R., SAMPLING POINT MOVED TO DECREASE RISK.

The man was removing a shaft when the shaft moved and pinched his little right hand finger against the frame. The finger required stitches but there was no lost time.

THE INJURED MAN WAS MOVING A 55 GAL OIL DRUM. PINCHED RIGHT INDEX FINGER BETWEEN DRUMS TAKEN TO EMERGENCY ROOM FOR X-RAY. NOTHING REQUIRED BUT APRESCRIPTION PAIN MEDICATION WAS GIVEN.

THE INJURED MAN WAS REPAIRING A PALLET WHEN HE SUFFERED A LUMBAR LIFTING CAPACITY AND IS RECIEVING THERAPY.

ON 10/13/93 EE WAS DIAGNOSED WITH A HERNIA FROM LIFTING OIL DRUMS. NO ACTION WAS NECESSARY UNTIL NOW. HE REQUIRES AN OPERATION TO REPAIR HERNIA.

THE MAN TWISTED HIS BACK LIFTING A 50# BAG OF ROCK DUST. NO TREATMENT WAS SOUGHT UNTIL 6-10-02. HE WAS PUT ON RESTRICTED DUTY WHICH DOES NOT INTERFERE WITH HIS REGULAR WORK.

THE MAN WAS REMOVING THE ACCUMULATOR BOTTLE FROM A CRUSHER WHEN THE BOTTLE MOVED UNEXPECTEDLY AND PINCHED THE LITTLE FINGER ON HIS LEFT HAND. HE WAS TREATED AT THE ER THAT DAY, MISSED WORK THE FOLLOWING DAY AND RETURNED TO REGULAR DUTY THE NEXT DAY.

THE MAN'S HAND SLIPPED ON A FUEL PUMP CAUSING A LACERATION TO THE RING FINGER OF THE RIGHT HAND.FOUR STITCHES WERE REQUIRED.

THE INJURED MAN WAS REMOVING ICE FROM A 35 GAL DRUM WHICH HE WOULD BE USING. WHEN HE DUMPED THE BROKEN ICE FROM THE DRUM HE EXPERIENCED PAIN IN HIS LOWER BACK. HE WILL REQUIRE THERAPY BEFORE R ETURNING TO WORK.

THE INJ MAN WAS LIFTING WHEN HE EXPERIENCED PAININ RIGHT KNEE WHICH WAS DIAGNOSED AS A SPRAIN. HE RETD IMMEDIATELY ON LIGHT DUTY STATUS

THE MAN WAS WALKING OUT TO THE SCALE HOUSE DOOR WHEN HE REACHED BACK TO CLOSE THE DOOR HE TWISTED HIS BACK. HE WAS TAKEN TO EMERGENCY ROOM FOR TREATMENT AND WAS GIVEN A SHOT FOR PAIN. HERETRNE D TO WORK WITH NO FURTHER COMPLICATIONS.

THE INJURED MAN'S RIGHT THUMB WAS BROKEN WHILE HE WAS UNHOOKING A HOIST HOOK FROM A SCREEN.
