Stone block broke at drier, the piece that fell cut employees left leg below the knee.

On Super 72, spinning spool to line up ware liners with right hand, somehow thumb on left hand got caught between spool and ware liner.

Cutting straps from bundles of landscape stone, when 2nd strap was cut the stone pieces fell all over pinching employees finger between stones.

Moving power lines and shoulder popped.

Stacking 8-9" cut wall stone onto pallet, stone piece he was stacking broke apart which then caught his finger tip between the stones and cut it off. It is his left hand middle finger.

Flipping stone, finger got caught between stone and table. Open fracture to right-hand pinky finger from top joint to tip of finger; bone crushed, stitches to close and re-attach fingernail.

On 7/25 EE said he was going to chiro. He did not say he got hurt during work. Not convinced this is work related injury because of what ee did over weekend on July 28, but found out on 8/22 that work. comp. insurance will be treating injury as work related until further investigation.

Pushing stone off of stone cutter that had broke down, thus injuring his back.

PICKED UP A PIECE OF STONE WHICH CRACKED WITH ONE PIECE DROPPING ON FOOT WITH SLIGHT FRACTURE. WILL BE BACK TO WORK IN 2-3 WEEKS WITH NO FURTHER OR FUTURE IMPAIRMENT.
