Employee was doing a brake inspection of a Trailer when EE was measuring the slack adjuster a small chunk of dirt fell between EE's safety glasses and EE's eye causing scratches on EE's eye. we took EE to urgent care where the prescribed EE antibiotics.

Mostrando los 35EMPLOYEE WAS WIPING DOWN HUB SURFACE OF THE LEFT FRONT BRAKE ON 13060. ANOTHER EMPLYOEE WAS REMOVING DUO-CONE SEAL AT THE SAME TIME. THE DAMAGED METAL RING SPUNG FORWARD AND HIT THE EMPLOYEE IN THE RIGHT THUMB KNUCKLE CUTTING IT OPEN, RESULTING IN STITCHES.
Employee was picking up a chock block that was froze down and felt discomfort to their left shoulder. They continued to work and did not want to be seen by a doctor. On April 18th they reported that they were feeling a lot of pain and would like to be seen by a physical therapist. They then received dry needling.
Employee was working over on another shift on 3/19/2024 changing a hydraulic hose under a 785 haul truck. When the employee stood up the employee felt a pop in knee. Employee was scheduled to return to work 3/22/2023.
Employee was doing a shop inspection and seen a tire out of place, when EE bent down and picked it up to return it to its proper location EE felt pain in EE's lower back.
Employee strained their left wrist and forearm back on 08/08/2018, pushing on the door handle of a 785 CAT haul truck. Surgery had to be performed on 04/21/2021 to remove some scar tissue from the wrist.
Employee was shoveling frozen mud from the track frame of a D-10 dozer when EE felt a strain in right shoulder. Employee went to the doctor for observation two days later. EE was referred to an orthopedic surgeon. This injury went LTA on March 14, 2019.
Employee was removing a Lock Ring from a 785D wheel assembly. The Lock Ring began to fall off the wheel assembly. The employee tried to stop the Lock Ring from falling resulting in the employee getting thumb smashed.
Employee was lowering a piston into a ram of a track adjuster for a D 10 dozer. When piston was in place employee grabbed it and was lowering the overhead crane to unhook lifting straps. The crane hook came in contact with left pointer finger, requiring stitches.
Employee was helping install Hilfickers around the inlet and outlet of a fifteen foot culvert. Employee stepped over the four foot rise to retrieve a rake that was stored inside the culvert. Employee lost footing and slipped puncturing upper leg with a 3/8 inch wire located on top of the hilfickers requiring two to three stitches.
Employee was unloading oak blocking timbers from his trailer to his ramps to load a piece of equipment when he felt a sharp pain in his shoulder (strain).
Employee was removing blow bar keeper blocks from the shales crusher when the block slipped out of their hand, went to grab it and wedging left thumb between bar and rotor of the mill. Employee went to doctor and was diagnosed with a cracked bone in thumb.
Employee was pulling on an old spool of three inch fuel hose, did not see the wire reinforcement sticking out the end of rubber. He was moving hand to get better grip and wire caught middle finger on right hand. Employee received 6 stitches.
Employee was cleaning work area at close of shift, was using a pressure reducer on an air hose to clean up work bench. There was an accumulation of dust and metal shavings that was hidden between items on the bench; employee had a foreign object taken out of left eye as a result.
Employee was opening a can of welding rods by pulling on the pop top tab, it hung up and employee grabed the can to get a better hold. Employee was not wearing gloves and caught right index finger on edge of lid, cutting index finger. Required four stiches.
Employee was working in the B pit area on the date of incident and around 2:00 PM. He started to have some irritation in left eye, he filled out an accident packet at end of shift. The next morning he was taken to the hospital and a small foreign body was taken out of eye and antibiotic was prescribed in eye drop form.
Employee was running a loader, employee stepped off on to a small rock. He got back on the loader for 30 minutes. Later he complained of knee pain. He was taken to the doctor. Doctor found no damage but there was arthritis and bone spurs. Employee has a history of previous work and non-work related knee injuries. Workers compensation is investigating this incident.
Employee was rotating a drive pump to get the timing right, the pump moved unexpectedly and smashed his hand between the flange and pump. Employee received a cut in the palm of hand and a bruise on the top of hand resulting in three stitches
Mechanic was changing out a welding gas bottle when he felt a sharp pain in lower left groin. He went to the doctor for diagnosis and returned to work until 2/6/2007 when he went in to have a small hernia repaired. He reported this to his supervisor on 2/20/2007.
Employee was lifting the chock block into the holder when something popped in the left shoulder. Felt some numbness in left hand. Employee went to a doctor during days off. Doctor did not see anything. Employee continued to work for about a month. Then went in for exploratory surgery on 3/13/06.
The employee was loading a pump impeller in to a bucket of a skid steer loader. The impeller tipped over and pinched employee's finger between impeller and I-Beam.
Employee was removing a small air conditioning unit from an office window. It was frozen to the frame. When the unit broke loose it slipped out the window. Employee grabbed the cord to catch it from falling. The cord wrapped around employee's ring finger on the left hand, breaking the finger when the cord pulled tight.
Employee was crawling under a haul truck to change the oil in the tranmission. Twisted the right knee on uneven ground. Was not accepted by state workmans comp until 8-31-05.

USING AN OVERHEAD CRANE TO MOVE AN ELECTRIC MOTOR, EMPLOYEE WAS PUSHING MOTOR INTO PLACE AND STRAINED RIGHT SIDE OF BACK.
REMOVING OIL PLUG TO ADD OIL IN PIVOT SHAFT, PLUG WAS TIGHT. WHEN PLUG BROKE LOOSE EMPLOYEE'S LEFT HAND SMASHED INTO TRACK FRAME.
EMPLOYEE RAN INTO FORK ON LIFT TRUCK, CUTTING LEFT EYE BROW.
LIFTING A HEAVY BAG OF GARBAGE SHE TURNED TO THROW IT IN BIN. EMPLOYEE RETURNED TO WORK ON 6/22. EMPLOYEE WENT ON DISABILITY ON 6/30/02.
CLEANING WHITE BOARD SPRAYED CLEANER IN EYES.
EE WAS SHOVELING SNOW WITH A HAND SHOVEL AND EXPERIENCED STOMACH DISCOMFORT. A FEW DAYS LATER HE WAS DIAGNOSED WITH A HERNIA WHICH BECAME REPORTABLE ON 3/19 WHEN HE WENT IN FOR SURGERY.
INDIVIDUALS WERE PULLING THE PIN OUT OF THE NOSE CONE ON A 150-TON HAUL TRUCK. THE PIN HUNG UP, SO THE INJURED EE LIFTED THE END WHILE ANOTHER MECHANIC PUSHED THE PIN FROM THE OTHER END. THE P IN CAME OUT, SMASHING THE EE'S THUMB JOINT AND CUTTING THE THUMB-REQUIRING SEVERAL STITCHES.
INDIVIDUAL WAS CLIMBING A LADDER TO FILL A GENERATOR GAS TANK. WHILE LIFTING A 5-GALLON GAS CAN,SHE TURNED WRONG & PULLED A BACK MUSCLE. SHE REPORTED TO WORK THE NEXT DAY BUT COULD NOT PERFORM HER DUTIES. SHE WAS TAKEN TO THE HOSPITAL, TREATED FOR MUSCLE SPASMS & MISSED ONE DAY OF WORK.
INDIVIDUAL WASPUTTING AWAY A SHIPMENT OF DRILL BITS. DRILL BITS WEIGH APPROX 120 LBS. BITS HAD TO BE STOOD UP AND PUSHED UNDER A SHELF. DURING THIS PROCESS, EMPLOYEE STRAINED HER MIDDLE BACK.
HE WAS CHANGING BATTERIES ON CAT DOZER, WHEN HE STRAINED HIS LOWER BACK. INDIVIDUAL WAS TAKEN TO DOCTOR, GIVEN MEDICATION AND RESTRICTED FROM WORK FOR FOUR DAYS.
HE WAS WORKING UNDERNEATH CAT-DOZER, TIGHTENING BOLTS. WHEN HE GOT OUT FROM UNDER EQUIPMENT, S PLACED ON TWO WEEKS RESTRICTED DUTY.
