Employee was setting up crushing plant. Employee was pulling heavy electrical wire over head assembly. Strained right shoulder and left leg.
3 employees were installing a roller on a conveyor. One employee lost his grip and the injured employee tried to maintain his grip, resulting in pinching of finger between the roller and conveyor frame.
Accident was NOT reported to Concrete Nor'West until 3/25/09. On approx. 2/25/09 EE was carrying bags of cement up a hill. On 3/20/09 he was having muscle spasms and saw a chiropractor. He saw the Chiropractor again on 3/24/09 and the doctor indicated he has compression fracture. EE informed us on 3/25/09 that he injured himself in February 2009.
Worker was pulling on a bar when the bar broke. Sudden weight shift caused worker to run back of hand across steel guard.
Installing an I-beam in the shaker deck. Had to do very heavy lifting and standing in an awkward position. Strained abdomen. This incident did not become reportable until his first doctors visit on 3/24/08.
Employee was pulling ladder from the rear of the service truck and felt a sharp pain in lower back. Employee first sought medical attention on May 1, 2007.
Employee was replacing a safety guard in the tunnel. Employee was lifting the safety guard and turning. Employee felt back go out.
Incident occurred on 6/5/04 and treated as First Aid only until doctor visit on 6/21/04. Left brake caliper was partially disassembled and on mounting pedestal. He bent down to pick up a fallen Allen wrench and caliper fall off and hit top of knee.
WHILE HOLDING A 3 FOOT LENGTH OF STEEL PLATING IN PLACE TO WELD, IT SLIPPED, STRIKING LEFT EAR.
HE WAS HOLDING THE BECKET BY IT'S EYES AND WAS TAPPING THE SHORT LENGTH OF PROTRUDING CABLE AGAINST THE DECK USING THE WEIGHT OF THE BECKET TO FORCE THE CABLE THROUGH. CABLE RELEASED AND TRAP PED 2 FINGERS.
WORKING WITH 3 OTHER PERSON ON REPAIR OF TOP SHAKER DECK. SHOULD HAVE HAD 2 PERSONS ON BOTH SIDES OF SHAKER TO BALANCE IT OUT. TIGHTENING BEARING PLATE TO SHAKER BOX, WHEN THE SHAFT TURNED AND COUNTER WEIGHT PINCHED TIP OF FINGER.
EE WAS ON LIGHT DUTY AND WAS HELPING WITH GENERAL MAINTENANCE ON THE DREDGE.
CLIMBING LADDER UP TO WINCH HOUSE, STRUCK LEFT KEE ON NEXT STEP UP.
CHANGING BRAKE ROTOR ON DREDGE, ROTOR SLIPPED OFF HUB AND PINCHED FINGER AGAINST OTHER HALF OF HUB ON MOTOR SHAFT.
