On Friday 1/21/11, miner' s final day at the company(retired thereafter), miner complained of knee inflammation & sought physician evaluation. Knee strain/sprain diagnosed. Miner told HR director that he had ongoing arthritis in this knee & that the carrying of bags likely inflamed it.
EE was loading palletts into bagging pallet dispenser and pinched middle finger between pallet and pallet dispenser frame, bruised and swollen, treating Dr prescribed 3 days of light hand use.

CLEANING OUT A FLOW METER, EMPLOYEE REACHED UP TO PULL DOWN LEVER TO FLUSH THE LINE. THE LEVER WAS STUCK OR HARD TO OPERATE.

WAREHOUSE WORKER WAS ATTEMPTING TO REPLACE SMALL METAL HOPPER ON WHEEL ASSEMBLY WITH CO-WORKER. ATTEMPTED TO BEND OVER IN CROUCHED POSITION TO LIFT HOPPER. STRAINED LOWER BACK ( AGGRAVATED PRI OR INJURY FROM MOTORCYCLE ACCIDENT WHICH WAS UNKNOWN TO OPERATOR). TREATED WITH MEDICATION AND PLACED ON LIGHT/ RESTRICTED DUTY. RELEASED 1-31-96 BY TREATING PHYSICIAN.
MINER WAS INSTALLING METAL GUARD WITH COWORKER. AS THEY WERE SETTING IT IN PLACE, GUARD SLIPPED AND FELL ON RIGHT HAND OF MINER CUTTING HIS RIGHT RING FINGER. MINER WAS WEARING LEATHER GLOVEWS AT TIME OF INCIDENT PREVENTING FURTHER INJURY.
WORKER DROPPED PIECE OF PIPE ONTO METAL PLATFORM AT APPROX. EYE LEVEL. PIPE STRUCK CERAMIC GRINDING MEDIA WHICH FLEW AND STRUCK LEFT EYE. TREATED AND RELEASED BY PHYSICIAN. REPORTED TO SUPERVI SOR ON 2-8-95.
WORKER WAS REMOVING SWING GATE (DOOR) ON EXCAVATOR TO INSPECT HYDRAULIC LINES. HEAVY METAL GATE SWUNG SHUT AND CAUGHT FINGER BETWEEN GATE AND EXCAVATOR. THIS FRACTURED RING FINGER ON LEFT HAND .
EE WAS ATTEMPTING TO CLOSE A VALVE AND REAHCED OVER AN OBJECT IN THE WAY. THIS AWKWARD POSITION CAUSED A STRAIN IN HIS SHOULDER. EE CHOSE NOT TO SEE DR. AND WAS ORDERED TO SEE ONE ON 11-23 WHN E HE COMPLAINED OF PAIN.

WORKERS TURNED QUICKLY AND STRUCK ELBOW AGAINST CHICAGO FITTING PIPE ON 11-17-93. HE DID NOT REPORT INCIDENT THINKING IT WAS NOT SERIOUS. ONE MONTH LATER HE STILL FELT PAIN WHEN LIFTING AND RE PORTED TO ME ON 12-15-93. AT THIS TIME ACCIDENT BECAME REPORTABLE TO MSHA. SENT TO DR DIAGNOSED AS TENDONITIS CAUSED BY TRAUMA. WORKER REINSTRUCTED ON ACC. REPORTING POLICY TIMELY.
EMPLOYEE ALLEGES THAT FOR A PERIOD OF APPROX. 4 WEEKS HIS BACK WAS SORE AND FATIGUED FROM PERFORMING JOB DUTIES OF LIFTING AND SHOVELING. FIRST REPROTED TO SUPERVISOR ON 10-25-93 AT WHICH TIME THIS INCIDENT BECOMES REPORTABLE. LUMBAR STRAIN DIAGNOSED BY TREATING PHYSICIAN.
