Walking across the leach pad, stepped and sunk down to EE's left knee. when EE pulled it out of the mud EE felt a rip

On November 14th, a mechanic bumped their knee while descending the back of a maintenance truck. Knee became swollen and painful. The employee continued working full duty. On December 3rd the injury became reportable when the employee requested medical evaluation at local hospital and an immobilization brace was applied, and potential fracture was diagnosed.

Unseen all thread was protruding from the bed of a 3rd party truck. Employee made contact just below the left eye.

7/2/2023 While working in the pulverizing room employee was splitting samples and felt a pop in EE's left wrist. The employee did not wish to seek medical attention at the time of injury. On 7/5/2023 employee was seen by medical professionals for continued pain and was placed on restricted work assignment.

While lifting the end of a box employee accidentally dropped the box on top of left foot.

While lifting the drive belt guard on B screen, employee felt discomfort in groin area.

Employee finished repairs and was placing tools back on service truck. Employee turned left while leaving weight on right foot and twisting. They felt pain in their right knee.

Employee felt a pinch in lower back when they bent down to pick up a door mat.

Employee was connecting a water hose onto a fitting. Employee felt a pop in right elbow when connecting the hose. Employee was placed on modified work assignment due to elbow strain.

While operating a floor jack employee went to move file cabinet off floor jack when it shifted and fell striking employees shoulder.

Employee was training in the water truck, riding in the passenger seat. The trainee placed a hand on the door post to adjust position in the passenger seat. At that time, the trainer closed the door, catching the tip of the trainee's thumb in the hinged door jam, causing crush injury that included fracture and cut.

Operator was checking oil when a loose strand of wire on the dipstick penetrated employee's finger. Employee was placed on modified duty with a splint.

Employee was manually sliding a vehicle gate shut. While pulling the gate, EE felt a pop in the back side of EE's right knee, followed by pain. Soft tissue injury (strain resulted). Specific diagnosis is pending results of an MRI.

After replacing loader teeth, employee felt irritation in eye. A metallic foreign body had to be removed.

While climbing ladder on loader employee felt a pop in left elbow resulting in medical treatment and one day of restricted duty.

Employee noticed discomfort in lower back after picking up sample bags. Employee requested medical evaluation the following day and was placed on restricted duty.

Driver experienced pain in back after offloading dyed diesel. Driver stated after handling a fuel hose driver noticed pain in back.

Employee reported that his left knee popped as he started to descend the ladder of a 785 Cat Haul Truck November 8 at the end of shift. Employee did not report accident to supervision per company policy until November 11.

Employee was loading a drill hammer onto a pickup flat bed. The hammer slipped pinching his right hand between the hammer and bed of truck.

Employee was assisting to lift a drill bit and sub into a pick up from the ground. The object rotated slightly when placed in the pickup bed and a metal splinter from the threads punctured his right ring finger. A small piece was lodged in his finger. Note report is required due to medical treatment.

Employee was attempting to remove a roller from a pick-up bed. As he began to lift he felt a "pop" in his right elbow. Report is do to restricted duty.

Employee was in the process of removing the fuel wiggens fitting from the loader e-z valve. The e-z valve stuck open causing diesel fuel to spray in employee's right eye. This is a reportable due to prescribed medication.

Employee was assigned the task of numbering and collecting approximately 200 blast hole drill samples each weighing between 1 and 5 pounds. As he was traveling back to the sample lab he noted his back began to hurt and get stiff. By the end of the shift the pain and discomfort remained and he reported his condition to his supervisor.

Employee was collecting rock samples on steep terrain when he felt pain in his right ham string area. Employee did not wish medical evaluation initially until 8/23/11. Injury did not become reportable until 10/28/11 when he was prescribed physical therapy.

EE was asked to inspect a haul truck tire (3300X51). EE noticed the tread leaving casing. EE installed a spare, the scrap tire was laying on the ground. EE released the tire hand from the tire, a flap of rubber then hit EE on the left knee.

While washing/cleaning under #11 belt at the Crusher employee had a water hose slip from his grasp. Employee attempted to re-gain control of the hose spraining his left wrist in the attempt.

Employee lacerated his right ring finger in the process of repairing a torque to transmission drive line as he rotated the shaft with his right hand. His finger was lacerated when it was pinched in the u joint.

The employee was pulling a steam hose off of a steam truck and hyperextended his left elbow. The elbow is currently strained and the impairment is unknown at this time.

Employee was working the south ditch project, cleaning the ditch. He bent down to lift a load of mud and vegetation and popped his upper back out between his shoulder blades. He lifted too much weight with his shovel.

Employee was in the process of installing an arm rest cover on a loader seat. While pushing and pulling he strained his stomach muscle.

Hazardous procedure

When backing off the take up drum on 21 conveyor belt the adjustment bolt got stuck. It released and slamed back to the orignal position pinching his finger between the bolt head and bearing slide rail.

EE WAS CLIMBING LADDER OF 777 WATER TRUCK AND STRUCK HIS KNEE ON THE BUMPER. THE FOLLOWING DAY HE WAS SEEN BY A DOCTOR AND DUE TO EXTREME SWELLING IT WAS NECESSARY TO DRAIN THE FLUID FROM HIS KNEE.

EMPLOYEE WAS REMOVING A CHOKER FROM THE MAIN HOIST ON 90 TON CRANE. ANOTHER EMPLOYEE WAS HOLDING THE SAFETY LATCH OPEN. HE RELEASED THE SAFETY LATCH BEFORE INJURED EMPLOYEE HAD HIS HANDS CLEAR AND LATCH SMASHED TIP OF RIGHT RING FINGER. INJURY REQUIRED 6 STITCHES TO CLOSE.

EE WAS ASSEMBLING A CONVEYOR BELT TAIL PULLEY TAKE-UP AND ASSEMBLY SHIFTED PINCHING. HIS LEFT RING AND LITTLE FINGER. INITIAL X RAYS SHOWED NO SERIOUS INJURY AND EE WAS RELEASED BACK TO WORK. A FOLLOW UP APPT WITH AN ORTHOPEDIC SPEICALIST RESULT IN RECONSTRUCTIVE SURGERY OF THE NAIL BED OF THE LITTLE FINGER 9-15-99.

EE WAS REMOVING BARREL OF OIL FROM SERVICE TRUCK AND INJURED WRIST. INITIALLY FELT WRIST WAS JUST SPRAINED BUT BY MIDDLE OF MARCH STILL FEELING PAIN & STIFFNESS. WAS SEEN BY A DOCTOR ON 3/23/9 9 AND PUT OFF WORK PENDING SURGERY TO REPAIR LIGAMENT DAMAGE.

EMPLOYEE PICKED UP A SAMPLE BAG AND FELT A PAIN IN HER NECK, WENT TO A DR ON 11-30-98 AND HE RELEASED HER FROM WORK ON 12-1-98 PENDING MRI.

EE REPORTED BACK PAIN AFTER OIFTING A BOX OF BOOSTERS FROM THE POWDER TRUCK. HE DID NOT FEEL MEDICAL ATTENTION WAS NECESSARY AT THE TIME. HE CONTINUED WORKING UNTIL MID DECEMBER WHEN HE REQUES TED TO SEE A DR. DUE TO CONTINUED PAIN AND STIFFNESS. HE WAS SEEN BY A DR. ON 12-15-98 WHILE ON A SCHEDULED VACATION. THE DR. RELEASED HIM FROM WORK STARTING 12-21-98, PENDING AN MRI & THERAP

EE PICKED UP A ROCK TO REMOVE IT FROM THE HAUL ROAD AND FELT A PAIN IN HIS GROIN AREA. UPON MEDICAL EXAMINATION HE WAS FOUND TO HAVE A INGUINALHERNIA. HE WAS RELEASED BACK TO WORK ON RESTRICT ED DUTY PENDING SCHEDULING OF SURGERY

EMPLOYEE WAS WACKING ON A WATER TRUCK #3635 WHEN A PIECE OF FOREIGN MATERIAL FELL INTO HIS EYE. HE WAS TAKEN TO THE DOCTOR WHERE THE PIECE WAS REMOVED.

EE HAD DONE SOME LIFTING OF BARRELS CONTAINING ORE SAMPLES AND REPORTED HE HAD BACK PAIN AFTER COMPLETING WORK DID NOT SEEK MEDICAL ATTENTION UNTIL 12-8-97 AND WAS RELEASED FROM WORK FROM 12-8 - TO 12-22 DUE TO WHAT WAS INITIALLY DIAGNOSED AS A CONGENITAL CONDITION NOT WORK RELATED ON 12-27 A FOLLOW UP EXAMINATION REVERSED INITIAL DIAGNOSIS TO WORK RELATED BACK SPRAIN.

10' ROD HUNG UP, LEANING AGAINST THE TABLE. EE GRABBED THE TOP OF THE ROD, MOVED THE ROD AND IT SLID, PINCHING HIS HAND AGAINST THE TABLE.

EMPLOYEE WAS PUTTING THE HAMMER BACK TOGETHER AND HIS FINGER GOT CUT BETWEEN BARREL AND HAMMER.

EMPLOYEE REACHED ACROSS CENTER CONSOLE OF HAUL TRUCK TO REPLACE THE COVER OF THE CAB AIR FILTER WHEN SHE FELT A PAIN IN RIGHT SHOULDER.

AS EMPLOYEE WAS REMOVING A LARGE WASHER (25 LBS) HE TURNED TO SET IT DOWN AND THE WASHER PARTIALLY SLIPPED FROM HIS HAND AND CAUGHT HIS RIGHT INDEX FINGER BETWEEN THE WASHER AND THE WORK PLATF ORM. THE TIP OF HIS FINGER WAS SMASHED AND HIS FINGER NAIL PARTIALLY TORN OFF. THE DR. HAD TO REMOVE THE FINGERNAIL AND RELEASED HIS BACK TO WORK.

EMPLOYEE WAS REMOVING AN EMPTY OXYGEN BOTTLE FROM THE RACK ON A PICKUP.HE LIFTED THE BOTTLE AND TURNED TO SET IT DOWN.AS HE SET IT DOWN HE FELT A PAIN IN HIS LOW BACK.

UNKNOWN CONTRIBUTING CONDITION TO INJURY 10-17-93. INJURY WAS NOT REPORTED UNTILFOLLOWING DAY. 10-18-93, WENT TO THE DR ON 10-21-93 AND AN ADJUSTMENT WAS MADE TO LOWER BACK.
